Premature Ventricular Complexes With 2 QRS Morphologies: Cardiac Crux Insights and an Endocardial Ablation Strategy.

BACKGROUND: Premature ventricular complexes (PVCs) from the cardiac crux often have intramural or epicardial substrates, complicating localization and ablation. CASE SUMMARY: A 53-year-old man presented with symptomatic, flecainide-refractory PVCs with a 24-hour Holter burden of 41.8%. An electrocardiogram showed frequent PVCs with 2 alternating QRS morphologies (right and left bundle branch block), both with a superior axis and subtle V1 to V2 and/or precordial differences, suggesting a single crux focus with preferential exits. Proximal coronary sinus mapping did not reveal early activation

The effects of catheter ablation of outflow tract premature ventricular complexes on atrial electromechanical delay.

BACKGROUND: Consistent data from several studies have shown that catheter ablation of frequent premature ventricular complexes (PVCs) results in substantial improvement in left ventricular ejection fraction (LVEF), left ventricular diastolic function, and left atrial volume and mechanics. However, the effects of catheter ablation of PVCs on atrial electromechanical properties have not been documented yet. AIMS: In the present study, we investigated the short-term effects of radiofrequency catheter ablation (RFCA) of outflow tract PVCs on atrial electromechanical delay (EMD). METHODS: A total

Clinical significance of myocardial scar in patients with frequent premature ventricular complexes undergoing catheter ablation.

BACKGROUND: Frequent premature ventricular complexes (PVCs) can result in PVC-induced cardiomyopathy (PICM). Scarring has been described in patients with frequent PVCs in the absence of apparent heart disease and in patients with known cardiomyopathy. OBJECTIVE: The purpose of this study was to determine the impact of focal myocardial scarring as detected by cardiac magnetic resonance imaging (CMR) on PICM, procedural outcomes, and recovery of left ventricular function in patients with frequent PVCs. METHODS: A total of 351 consecutive patients (181 men; age 53 ± 15 years; ejection fraction

Newer diagnostic criteria for identification of epicardial origin of ventricular tachycardia/premature ventricular complex.

Aim: The study defines criteria for localization of ventricular arrhythmias. Materials & methods: Data from 40 premature ventricular complex (PVC)/ventricular tachycardia (VT) were studied. The interval from PVC QRS onset to His potential and ratio of this interval to PVC/VT QRS duration was analyzed. Patients were divided into epicardial and endocardial groups based on mapping/successful ablation site. Results: The interval from PVC QRS onset to His potential in the epicardial versus endocardial group was 99 ± 21 ms and 64 ± 37 ms (p = 0.002). A cut off of 88 ms had a sensitivity of 73% and s

[From premature ventricular complexes to sustained ventricular tachycardia : An overview of innovations in the 2022 ESC Guideline on the therapy of ventricular arrhythmias].

The recent 2022 European Society of Cardiology Guidelines for the management of patients with ventricular arrhythmias and the prevention of sudden cardiac death are an update of the former 2015 European guidelines. With multiple tables, algorithms, and comprehensive integration of underlying study data, the new guideline is a user-oriented reference book for clinical practice that also covers special clinical situations such as cardiac arrhythmias in pregnancy or in the context of sports. Regarding the acute treatment of ventricular arrhythmias, cardioversion is now recommended in case of hemo

The value of NT-pro-brain natriuretic peptide and left ventricular ejection fraction for prediction of premature ventricular complexes burden.

OBJECTIVE: We aimed to investigate the relation of NT-pro BNP level and left ventricular ejection fraction with premature ventricular complex burden. PATIENTS AND METHODS: A total of 94 patients with PVC burden > 5% (age 45.9+12.9 years, 53 males, 41 females) were included in the study. The primary outcome was PVC burden % and main prognostic factors were LVEF% and NT-Pro BNP level. Gender, age, DM, HTN, presence of symptoms, symptom duration and heart rate were used as adjustment predictor variables. We created four different linear multivariable models to compare performance measures of pro

Radiofrequency catheter ablation for unifocal premature ventricular complexes triggering recurrent ventricular fibrillations in a young man without structural heart disease.

A 17-year-old man was referred for aborted sudden cardiac death. Ventricular fibrillation (VF) was recorded by automated external defibrillator. Post-resuscitation electrocardiograms showed frequent monomorphic premature ventricular complexes (PVCs), with left bundle branch block configuration and inferior axis. Cardiac arrest due to VF recurred twice within the initial 42 hours. Rhythm monitoring revealed multiple episodes of sustained VF triggered by a triplet of monomorphic PVCs having similar morphology with isolated PVCs. Comprehensive cardiologic workup revealed no structural heart disea

Very High-Power Short-Duration Ablation for Premature Ventricular Complexes From Sites With Suboptimal Catheter Stability.

INTRODUCTION: While very high-power short-duration (vHPSD) ablation has been shown to be safe and effective for ablation of atrial fibrillation, the utility of vHPSD ablation for targeting premature ventricular complexes (PVCs) remains unclear. We aimed to describe our experience of PVC ablation using vHPSD ablation targeting areas with suboptimal catheter contact. METHODS AND RESULTS: We included 8 patients (mean age 66.5 ± 11.3 years, 77% female gender, mean LV ejection fraction 52.8 ± 8.2%, baseline PVC burden 23.3 ± 10.1% [range 9-41%]) with PVCs originating from intracavitary structures

Calcium-mediated force-interval relationship drives post-extrasystolic potentiation in premature ventricular complexes: a computational study.

Premature ventricular complexes (PVCs) are common cardiac arrhythmias that can lead to cardiomyopathy when frequent. Post-extrasystolic potentiation (PESP), which is the transient increase in contractility following a PVC, may serve as a predictive marker for heart failure risk; yet, the underlying calcium-mediated mechanisms and their relative contribution compared to loading conditions remain poorly understood. We integrated a mechanochemical model coupling intracellular calcium dynamics to sarcomere mechanics within the CircAdapt closed-loop cardiovascular framework. A novel calcium source

A Novel ECG-Based Deep Learning Algorithm to Predict Cardiomyopathy in Patients With Premature Ventricular Complexes.

BACKGROUND: Premature ventricular complexes (PVCs) are prevalent and, although often benign, they may lead to PVC-induced cardiomyopathy. We created a deep-learning algorithm to predict left ventricular ejection fraction (LVEF) reduction in patients with PVCs from a 12-lead electrocardiogram (ECG). OBJECTIVES: This study aims to assess a deep-learning model to predict cardiomyopathy among patients with PVCs. METHODS: We used electronic medical records from 5 hospitals and identified ECGs from adults with documented PVCs. Internal training and testing were performed at one hospital. External

Prevalence of atrial fibrillation and procedural outcome in patients undergoing catheter ablation for premature ventricular complexes.

INTRODUCTION: Atrial fibrillation (AF) and premature ventricular complexes (PVC) are common arrhythmias. We aimed to investigate AF prevalence in patients with PVC and its impact on PVC ablation outcomes. METHODS: Consecutive patients undergoing PVC ablation at a single institution between 2016 and 2019 were included and prospectively followed for 2 years. Patients with severe valvular heart disease, hyperthyroidism, malignancy, alcohol use disorder and advanced renal/hepatic diseases were excluded. Twelve-lead electrocardiograms were used to diagnose AF and assess PVC morphology. All PVCs we

Diagnostic utility of early premature ventricular complexes in differentiating atrioventricular reentrant and atrioventricular nodal reentrant tachycardias.

BACKGROUND: His-refractory premature ventricular complexes perturbing a supraventricular tachycardia (SVT) establish the presence of an accessory pathway (AP). Earlier premature ventricular complexes (ErPVCs) may perturb SVTs but are considered nondiagnostic. OBJECTIVE: The purpose of this study was to test the hypothesis that an ErPVC will always show a difference >35 ms in its advancement of the next atrial activation during atrioventricular nodal reentrant tachycardia (AVNRT). During atrioventricular reentrant tachycardia (AVRT), a PVC delivered close to the circuit can result in greater a

Bepridil Inhibits Premature Ventricular Complexes Induced by Cardio-Sympathetic Nerve Stimulation in a Canine Experimental Model.

Sympathetic nerve activity has arrhythmogenic potential for ventricular arrhythmias associated with structural heart diseases. However, a sufficient amount of beta-blockers occasionally cannot be prescribed in some patients. An experimental study was performed to clarify the therapeutic effects of bepridil, a multiple ionic current inhibitor that does not affect beta-adrenergic receptors, for premature beats occurring during enhanced sympathetic nerve activity. Cardio-sympathetic nerve activity was augmented via stellate-ganglion (SG) stimulation in a canine model (n = 8), and the arrhythmogeni

Association between premature ventricular complexes during exercise, long-term occurrence of life-threatening arrhythmia and mortality.

OBJECTIVE: Exercise-induced premature ventricular complexes (EiPVCs) are often considered as benign arrhythmias, although they are associated with a high risk of all-cause death in the general healthy population. However, an intermediate pathophysiological process remains unclear, particularly in patients with known cardiovascular disease. The aim of this study was to find an association between EiPVCs, the occurrence of life-threatening ventricular arrhythmias (LACO), and all-cause mortality in patients with cardiovascular disease. METHODS: This was an observational study of a cohort of pati

Bipolar radiofrequency catheter ablation between the left ventricular endocardium and great cardiac vein for refractory ventricular premature complexes originating from the left ventricular summit.

Ablation for ventricular arrhythmias originating from the left ventricular (LV) summit is sometimes challenging. Bipolar radiofrequency catheter ablation (RFCA) is effective for refractory arrhythmias; little is known about bipolar RFCA from the coronary venous system and the appropriate settings. We experienced three cases of ventricular premature complexes (VPCs) originating from the LV summit successfully treated by bipolar RFCA between the LV endocardium (irrigated catheters as active electrodes) and coronary venous system (8-mm-tip catheters as return electrodes). These cases showed that

Transcutaneous Electrical Vagus Nerve Stimulation to Suppress Ventricular Premature Complexes (TREAT-PVC): A Randomized Clinical Trial.

BACKGROUND: Low-level tragus stimulation (LLTS) of the auricular branch of the vagus nerve at the tragus reduces atrial fibrillation burden. Its efficacy on premature ventricular complexes (PVCs) is unknown, however. OBJECTIVES: The goal of this study was to evaluate the efficacy of daily LLTS on PVC burden in patients with idiopathic PVC. METHODS: TREAT-PVC (LLTS to Treat Premature Ventricular Contractions) is a sham-controlled, double-blind, randomized trial. Eligible patients with >10% PVC burden were randomized 1:1 to receive LLTS vs sham stimulation to the earlobe (20 Hz, 1 mA below the

Estimated sodium intake and premature ventricular complexes: data from the population-based Swedish CArdioPulmonary bioImage Study.

BACKGROUND: Premature ventricular complexes (PVCs) predict coronary heart disease, heart failure, atrial fibrillation and death, all of which are also related to sodium intake. We studied estimated sodium intake and PVC frequency in the randomly selected population-based Swedish CArdioPulmonary bioImage Study cohort. METHODS: In our cross-sectional study, we included 5636 individuals with 24-hour ECG registration and fasting morning urine sampling. Sodium intake was estimated using the Kawasaki formula, and the association between sodium intake and PVC frequency was modelled using multivariab

Premature ventricular complexes and risk of atrial fibrillation and stroke in patients without structural heart disease.

BACKGROUND AND OBJECTIVE: Prior studies have suggested that patients with premature ventricular complexes (PVCs) may have an increased risk for atrial fibrillation (AF) and stroke. It is unclear whether frequent PVCs are linked to an increased risk of AF and stroke in patients where structural heart disease (SHD) has been excluded. We aimed to study if PVCs increase the risk of AF or stroke in patients without SHD. METHODS: In this retrospective observational cohort study, we included patients who received a PVC diagnosis at three major hospitals in Stockholm, Sweden. The patients had no hist
