[Peduncular hallucinosis in a young woman with vertebrobasilar insufficiency based on severe hypoplasia of a vertebral artery, demonstrated angiographically].

A clinical case of peduncular hallucinosis due to circulatory insufficiency of the cerebral trunk is reported. Angiography revealed marked hypoplasia of a vertebral artery. The patient presented changes to sleep-arousal rhythms and neurological signs with focus involving the cerebral trunk. The patient took an emotional part in her visual hallucinations even though she was aware that they were only hallucinations. A review of the literature on the subject follows.

Exertional Vertebrobasilar Insufficiency 6 Years after Vertebral Artery Dissection Treated with Occipital Artery-posterior Inferior Cerebellar Artery Anastomosis.

Exertional vertebrobasilar insufficiency (VBI) secondary to the non-atherosclerotic cause is uncommon. We herein report the case of a patient who developed exertional VBI long after extracranial right vertebral artery (VA) dissection. At the time of dissection, the right VA was completely occluded near its origin, but the distal flow was compensated by the collateral flow from the right deep cervical artery (DCA). After conservative management, the patient was discharged without neurologic deficit. Six years later, he developed recurrent VBI in association with the exertion of his right should…

[SERUM CONTENTS OF SUBSTANCE P, ENDOTHELIN-1 AND VASOACTIVE INTESTINAL PEPTIDE IN CHILDREN WITH RECURRENT BRONCHITIS AND VERTEBROBASILAR INSUFFICIENCY].

The recurrent bronchitis (RB) course is caused by the bronchi secretory-evacuation mechanisms state, which provide clearance from pathogens. This mechanism can be disrupted by vegetative reflexes and neuropeptides imbalance that develops in children with the syndrome of the vertebrobasilar arterial system (SVBAS). The objective: study of the neurogenic maintenance of the RB pathogenesis in children with SVBAS by studying the serum content of substances affecting of the bronchial mucosa secretory-evacuation function and inflammatory activity (substance P, vasoactive intestinal peptide - VIP and

Superficial temporal and occipital artery bypass pedicles to superior, anterior inferior, and posterior inferior cerebellar arteries for vertebrobasilar insufficiency.

Eighty-three patients underwent 85 intracranial to extracranial pedicle bypass anastomosis procedures to the posterior circulation. There were 15 patients with occipital artery (OA) to posterior inferior cerebellar artery (PICA) anastomosis, 20 patients with OA to anterior inferior cerebellar artery (AICA) anastomosis, and 50 patients with superficial temporal artery (STA) to superior cerebellar artery (SCA) anastomosis. All patients had transient ischemic attacks (TIA's) suggestive of vertebrobasilar ischemia. Twenty-seven patients had crescendo TIA's or stroke in evolution and were considere

A Forgotten Cause of Chronic Dizziness in the Elderly: Exercise-Induced Vertebrobasilar Insufficiency due to Grade III Subclavian Steal Syndrome.

Chronic dizziness in elderly patients, particularly when reproducibly triggered by upper limb exertion, should prompt evaluation for arm-exertion-provoked vertebrobasilar hypoperfusion due to subclavian steal syndrome. Duplex ultrasonography can identify complete retrograde vertebral artery flow and confirm complete Grade III subclavian steal physiology, highlighting a treatable vascular cause that may otherwise be misattributed to vestibular, medication-related, or neurodegenerative disease.

Carotid Endarterectomy for Vertebrobasilar Insufficiency Caused by Severe Stenosis of Primitive Hypoglossal Artery: A Technical Case Report and Literature Review.

Persistent primitive hypoglossal artery (PHA) originating from the external carotid artery (ECA) is a rare anomaly. Reports of carotid endarterectomy (CEA) for ECA stenosis associated with this anomaly are even rarer. A 76-year-old woman presented to a medical clinic with a major complaint of refractory dizziness. Carotid ultrasound study suggested severe stenosis of the left cervical carotid bifurcation; therefore, she was referred to our department for a possible CEA. The imaging results indicated severe stenosis of the left carotid bifurcation and that the ECA was a PHA and the origin of th

Rotational vertebrobasilar insufficiency due to dynamic compression of the dominant vertebral artery by the thyroid cartilage and occlusion of the contralateral vertebral artery at C1-2 level.

BACKGROUND: Vertebrobasilar insufficiency caused by head rotation is an uncommon event. The mechanism is obstruction of the vertebral artery resulting in flow limitation or, less frequently, thromboembolism. CASE DESCRIPTION: A 41-year-old male developed dizziness and light-headedness during chiropractic manipulation when his head was turned to the extreme right position. Computed tomography angiography (CTA) of the neck and selective digital subtraction angiography (DSA) of the vertebral arteries revealed that when the patient turned his head to the extreme right position the dominant right …

This case emphasizes that recurrent atypical vertigo and brief syncope may be warning signs of vertebrobasilar insufficiency, especially in the context of vertebral artery stenosis with limited collateral flow. Timely vascular imaging and intervention can prevent subsequent debilitating brainstem stroke.

Superficial temporal artery-middle cerebral artery double bypass for vertebrobasilar insufficiency due to multivessel intracranial stenosis: A report of two cases with a network-based hemodynamic concept.

Vertebrobasilar insufficiency (VBI) caused by multivessel intracranial stenosis is associated with a high risk of recurrent ischemia and poor outcomes, and its optimal treatment remains controversial. We report two cases of VBI associated with complex intracranial arterial stenosis that were successfully treated with superficial temporal artery (STA)-middle cerebral artery (MCA) double bypass. In both patients, severe stenosis or occlusion of the vertebral and internal carotid arteries was accompanied by a collateral flow through the posterior communicating arteries. Applying a network-based h…

High-Flow Saphenous Vein Graft Bypass from Common Carotid Artery to Vertebral Artery (V2) for the Treatment of Vertebrobasilar Insufficiency Due to Bilateral Vertebral Artery Origin Stenosis.

Vertebrobasilar insufficiency can lead to devastating brainstem and posterior cerebral infarction without timely treatment.1 A 56-year-old man with a history of hypertension, hyperlipidemia, and diabetes mellitus presented to the clinic with right hemiparesis due to previous left cerebral hemispheric stroke. He also harbored a giant asymptomatic parieto-occipital meningioma incidentally diagnosed 2 years ago. Neuroimaging showed old left cerebral infarcts and a tumor that had remained stable in size. Cerebral angiography identified bilateral vertebral artery stenosis near their origin from the…

Collateral gains from vascular microsurgical intervention on the V1 segment dolicoarteriopathy of the vertebral artery: alleviating restless leg syndrome and urgency urinary incontinence symptoms in refractory vertebrobasilar insufficiency cases.

BACKGROUND: This study assesses the efficacy of vascular microneurosurgical interventions on the V1 segment dolicoarteriopathies (grade 2 and 3 kinks) of the vertebral artery (VA) in patients with refractory vertebrobasilar insufficiency (VBI), manifesting as secondary benefits for restless leg syndrome (RLS) and urgency urinary incontinence (UUI). METHODS: We retrospectively reviewed 78 patients for RLS and UUI symptoms who underwent vascular procedures primarily for refractory VBI due to V1 segment dolicoarteriopathy. Depending on the severity of arterial kinking, the surgical techniques va…

Subclavian steal despite ipsilateral vertebral occlusion.

In the classic subclavian steal syndrome, vertebrobasilar insufficiency is caused by reverse flow in the vertebral artery ipsilateral to a subclavian stenosis or occlusion. We present two patients with vertebrobasilar insufficiency and ipsilateral vertebral and subclavian occlusive disease. The postulated mechanism of vertebrobasilar insufficiency is reverse flow in collateral neck vessels. In both patients, symptoms were relieved by carotid subclavian bypass. Thus, vertebral occlusion ipsilateral to a subclavian stenosis does not preclude subclavian steal syndrome.

Clinical value of MRA analysis of the vertebrobasilar system in vertiginous patients.

Vertebrobasilar haemodynamics correlate closely with syndromal vertigo. The aim of this study is to examine the clinical value of MRA for evaluation of vertebrobasilar haemodynamics in vertiginous patients. We examined 40 individuals (20 with vertebrobasilar insufficiency and 20 diagnosed with other vestibular disorders). MRA results were quantified on the basis of the severity of vascular stenosis in the major arteries of the vertebrobasilar system and we analysed the relationship of these results with otoneurological diagnoses, eye-tracking tests (ETT) and optokinetic nystagmus (OKN). MRA re…

Vertigo secondary to hypotension: the relationship between therapeutic effects and hearing.

Patients with vertebrobasilar insufficiency tend to develop transient, repeated vertigo. However, diagnostic criteria by which to definitively diagnose circulatory disturbance of the vertebrobasilar system have yet to be established. Thus, in order to determine such criteria the relationship between the therapeutic effects on clinical symptoms and hearing in 40 patients with vertigo secondary to hypotension were investigated in the present study. In most patients, vertigo was perceived as a floating or spinning sensation which occurred spontaneously or during positional changes and lasted for …

[Vertebrobasilar circulatory disorders. Diagnosis--pathogenesis--therapy].

Disturbances of the vertebrobasilar circulation are often seen by the general physician and usually described as "vertebrobasilar insufficiency" (VBI). However, the term VBI is only a description of a syndrome and not a definitive diagnosis. It is emphasized that on therapeutic grounds the different etiologies and pathogenesis of this syndrome should be searched for carefully by adequate examination, including clinical findings, extra- and intracranial cerebrovascular doppler sonography, computed tomography, possibly intraarterial digital angiography and nuclear magnetic resonance imaging, and…

Doppler sonography of vertebral-subclavian system.

The ultrasonographic study of vertebrobasilar system can be correctly carried out if based on the knowledge of the peculiar anatomical features of vertebrosubclavian system, of its variants and collateral vessels, of the close anatomofunctional relationship with carotid vessels. In the pathogenesis of vertebrobasilar insufficiency intrinsic and extrinsic steno-obstructive causes are separated. Particular attention is paid to the subclavian steal syndrome. Five types of steal are identified in relation to the involved arteries and to the site of steno-obstructive lesions. Specific aspects of co…
