The brainstem contains ocular motor and vestibular structures that, when damaged, produce specific eye movement disorders. In this review, we will discuss three brainstem syndromes with characteristic ocular motor and vestibular findings that can be highly localizing. First, we will discuss the lateral medullary (Wallenberg) syndrome, focusing on ocular lateropulsion, saccadic dysmetria, and the ocular tilt reaction. Second, we will review the medial longitudinal fasciculus syndrome including the ocular tilt reaction, nystagmus, and the vestibular-ocular reflex. Lastly, we will discuss hypertr

We report the case of a patient with episodic rotational vertigo for years. Ear symptoms were negated. The clinical diagnosis was not clear - Menière's disease, vestibular migraine and recurrent vertigo after vestibular neuritis all qualified for differential diagnoses. A locally enhanced inner-ear MRI established clarity by showing an endolymphatic hydrops in the vestibulum. Besides the classical triad of Menière's disease there are other clinical appearances of endolymphatic hydrops, which can be visualized with inner-ear MRI.

Ménière syndrome is an inner ear disorder characterized by spontaneous attacks of vertigo, fluctuating low-frequency sensorineural hearing loss, aural fullness and tinnitus. When the syndrome is idiopathic and cannot be attributed to any other cause (eg, syphilis, immune-mediated inner ear disease, surgical trauma), it is referred to as Ménière disease. This article reviews the physiologic effects of Ménière disease on vestibular function, as measured by caloric, head impulse, and vestibular-evoked myogenic potential testing.

OBJECTIVE: To define the ocular and cervical vestibular evoked myogenic potential (oVEMP and cVEMP) profile in Ménière's Disease (MD), we studied air-conducted (AC) sound and bone-conducted vibration (BCV)-evoked responses in 77 patients and 35 controls.
METHODS: oVEMPs were recorded from unrectified infra-orbital surface electromyography (EMG) during upward gaze. cVEMPs were recorded from rectified and unrectified sternocleidomastoid EMG during head elevation against gravity. Responses to AC clicks delivered via headphones and BC forehead taps delivered with a mini-shaker (bone-conduction vi

The aim of this study was to investigate vestibular evoked myogenic potentials (VEMPs) and their clinical significance in Behcet's disease. Twenty-six patients with Behcet's disease and 25 healthy volunteers were evaluated for pure tone audiometry, caloric response, and VEMPs. Sensorineural hearing loss was found in 53.8% of patients with Behcet's disease, which was significantly higher than controls. Four patients had canal paresis, but no controls; this difference was not significant. Although VEMP recordings were elicited in all study subjects, mean p13 and n23 latencies were prolonged in B

Few reports have appeared in the literature concerning vestibular findings in Behçet's disease. In the present study, extensive vestibular testing, performed in 14 patients (8 male, 6 female; mean age: 32 years; range: 12-51) presenting definite Behçet's disease, revealed a high prevalence of central vestibular dysfunctions (78%). Data reported here suggest that an otoneurological evaluation of Behçet's disease patients may be helpful in identifying unexpected vestibular dysfunctions and central nervous system involvement different from the classical manifestations of the neuro-Behçet's syndro

A total of 62 ears of patients with typical Meniere's disease was examined by the furosemide test to detect endolymphatic hydrops. In 95% of the normal control group, the per cent change in the maximum velocity of the slow phase of caloric nystagmus (MVS) after injection of furosemide was under 10%. Therefore, a positive furosemide test was defined as a change in MVS of more than 10%. Thirty-five (56%) of the 62 ears with typical Meniere's disease showed a positive furosemide test. When the affected ears were divided into two groups according to vestibular symptoms, only 11 (38%) of 29 inactiv

Numerical analysis of the vestibular nerve in Méniè's disease was performed. Vestibular nerve specimens were obtained from three patients with classical clinical findings of Ménière's disease during retrosigmoid vestibular neurectomy. For each patient, an ultrathin section, including an entire cross-section of the nerve specimen, was examined. Fiber counts of nerve specimens with definite pathologic findings were compared with caloric testing results. A significant correlation was found between reduced vestibular response and the incidence of abnormal nerve fibers and also the density of the a

The role of vestibular nerve section (VNS) surgery in the management of Menière's disease is considered by prospective analysis of a large series of 531 patients treated by the author over a period of 11 years. Twenty-seven percent were referred by their primary care physicians, while the rest were secondary (45.6%) or tertiary (27.3%) referrals. Overall, 62 (11.7%) Menière's patients underwent 63 VNS procedures. This surgery was undertaken more frequently in the secondary and tertiary referral patients (14.2%) than in the primary referrals (5.5%). The retrolabyrinthine technique was the prefe

OBJECTIVE: Our objective was to determine the familial incidence of large vestibular aqueduct syndrome (LVAS) detected by CT and MR imaging and to propose the genetic inheritance of LVAS.
MATERIALS AND METHODS: We retrospectively reviewed cases of LVAS revealed by temporal-bone CT and MR imaging at the University of Utah Health Sciences Center. We interviewed 25 patients with LVAS regarding family history of hearing loss. Any family members with onset of hearing loss before 30 years old also underwent CT and MR imaging. The vestibular aqueduct (on CT scans) or the endolymphatic duct (on MR im

In a retrospective study of 475 patients suffering from Meniere's disease, the hearing loss, both in the low and high frequencies, and the loss of vestibular function were studied over time. There were large variations of hearing loss from case to case but, overall, the low were more affected than the high frequencies, regardless of time elapsed since onset of disease. Hearing deteriorated over time and mean values of hearing loss correlated well with the elapsed time since onset of disease. There were also large variations of post-caloric labyrinthine preponderance (lp) from case to case but,

Vestibular nerve dysfunction as the major neuro-otological symptom in one of the osteopetrosis group of bone disorders is unusual. We describe a patient with Engelmann's disease who presented in this manner and who benefited from an eighth nerve decompression procedure. Surgical decompression of the internal auditory meatus to relieve vertigo has to our knowledge not been previously reported in this condition.

The infralabyrinthine approach to vestibular neurectomy was performed in 9 patients with unilateral Menière's disease. According to the AAOO (1972) criteria, 7 of 9 cases were graded as class B and the remaining 2 cases as class C. Otherwise, according to the AAO-HNS (1985) criteria, 6 patients who could be followed over 2 years were all graded as 'complete' at the vertigo control. The compensation of the spontaneous vestibular signs was rapid in the first 2 postoperative weeks, though an occasional imbalance on movement persisted even 3 years after the operation. No specific caloric reaction

Surgical treatment of Meniere's disease is indicated in patients with severe attacks who have failed drug therapy. A decision between a conservative or destructive procedure demands firstly exclusion of patients with non-otogenic diseases, bilateral disease or complete unilateral loss of function, and secondly assessment of the laterality of the disease, of residual labyrinthic function and of vestibular compensation. A total of 185 patients with Meniere's disease were investigated. All of them had a history and clinical signs that indicated surgery. Canal function, otolith responses and vesti

The vestibular ganglia in 11 temporal bones from subjects with known premortem unilateral Meniere's disease were studied for evidence of increased fibrosis. Tissue sections were treated with Gomori's trichrome stain and were examined independently by each of seven persons experienced in histological study. The averaged ratings for the amount of fibrous tissue in the vestibular ganglia showed no significant differences for ears with Meniere's disease, the opposite uninvolved ears, and the controls.
