Video-assisted Abruzzini technique for bronchopleural fistula repair. A pathology study.

Trans-sternal closure of the bronchial stump is an effective procedure to treat bronchopleural fistula after pneumonectomy. The paper reports a modified video-assisted Abruzzini technique that, maintaining the same results, should determine a lower surgical risk. Three simultaneous approaches are used: cervical video-mediastinoscopy, right anterior parasternal mediastinotomy, left parasternal thoracoscopic access. The dissection of the bronchial stump is performed entirely through the mediastinotomy approach after having controlled mediastinal vessels. The bronchial stump reamputation is achie

Diagnosis of Mesothelioma Using Flexible Bronchoscopy With Endobronchial Ultrasound-guided Biopsy of Mediastinal Lymph Nodes.

Malignant mesothelioma is a neoplasm that can arise from the mesothelial surface of the pleura and tends to be locally invasive. Various techniques have been used for diagnosis and include thoracentesis with pleural fluid cytology, closed pleural biopsy, and video-assisted thoracoscopic surgery. Thoracentesis and closed pleural biopsy can often establish the presence of malignancy, but frequently do not provide sufficient tissue to definitively diagnose malignant mesothelioma. Video-assisted thoracoscopic surgery biopsy has higher sensitivity for establishing the diagnosis, but is a more invas

3D-video- and robot-assisted minimally invasive ASD closure using the Port-Access techniques.

BACKGROUND: Video-assisted minimally invasive surgical methods with endovascular-based femoral cardiopulmonary bypass (CPB) and balloon occlusion of the aorta (Port-Access technique) were used to close an ostium-secundum atrial septal defect (ASD) in 7 patients. METHODS: Minor modifications were made to the system to provide drainage of the superior vena cava. The surgery was performed through a small (3.5-5cm) right anterolateral thoracotomy with 3D video and robotic arm assistance. RESULTS: The operative procedures were completely uneventful and the patients were discharged four days posto

Off-bypass coronary bypass grafting via minithoracotomy using mechanical epicardial stabilization.

BACKGROUND: Minimally or less invasive surgical coronary revascularization has gained increasing interest along with new techniques and devices designed for easier and safer procedures. Until recently, it appeared questionable whether grafting techniques with avoidance of cardiopulmonary bypass techniques would allow adequate results compared with conventional techniques using cardioplegic arrest. METHODS: Since June 1996, minimally invasive direct coronary artery bypass grafting procedures without cardiopulmonary bypass were intended in 24 patients (19 male, 5 female; age, 60.5 +/- 10.5 year

Endoscopic parathyroidectomy.

During the last three years, minimally invasive procedures have been adopted for the surgical treatment of primary hyperparathyroidism, because preoperative localization studies such as a high-resolution ultrasonography and sestamibi scintigraphy. guidance by intraoperative scans and the use of a quick, intraoperative PTH assay, have improved. Therefore, endoscopic parathyroidectomy can be performed for patients with primary hyperparathyroidism. The endoscopic procedures range from the 'pure' endoscopic approach characterized by constant gas insufflation and video-assisted gasless techniques.

Percutaneous video choledochoscopic treatment of retained biliary stones via dilated T-tube tract.

BACKGROUND: Retained biliary stones is a common clinical problem in patients after surgery for complicated gallstone disease. When postoperative endoscopic retrograde cholangiopancreatography (ERCP) and endoscopic sphincterotomy are unsuccessful, several percutaneous procedures for stone removal can be applied as alternatives to relaparotomy. These procedures are performed either under fluoroscopic control or with the use of choledochoscopy, but it is also possible to combine these methods. METHODS: Since 1994, we have used the percutaneous video choledochoscopic technique for the removal of

Video-assisted minimally invasive parathyroidectomy: benefits and long-term results.

BACKGROUND: In recent years several endoscopic and video-assisted techniques for parathyroidectomy have been described. The role of these techniques, with respect to time-honored conventional surgery, has been largely debated. This paper was designed to review the evidence, and make the recommendations, for the video-assisted/endoscopic approach to parathyroidectomy. METHODS: A database search was conducted in PubMed from which abstracts were screened matching our definition. Publications were further assessed and assigned their respective level of evidence. Additional data were obtained on t

Video-assisted and minimally-invasive open chest surgery for the treatment of mediastinal tumors and masses.

This article reviews the anatomy of the mediastinum as well as indications, limitations, techniques, and results of video-assisted thoracic surgery (VATS) and other minimally-invasive open-chest surgery approaches currently used for the surgical management of mediastinal tumors and masses. It is written by two surgeons with vastly different backgrounds and thoracic surgical experience. One of them is young and very familiar with VATS approaches and technologies while the other is a senior surgeon relatively unfamiliar with minimally-invasive techniques. This combination of authorship is ideal

Is minimally invasive parathyroid surgery an option for patients with gestational primary hyperparathyroidism?

BACKGROUND: Gestational primary hyperparathyroidism is associated with serious maternal and neonatal complications, which require prompt surgical treatment. Minimally invasive parathyroidectomy reduces pain, improves cosmesis and may achieve cure rates comparable to traditional open bilateral neck exploration. We report the clinical course of a woman with newly diagnosed gestational primary hyperparathyroidism and discuss the decision making behind the choice of video-assisted minimally invasive parathyroidectomy, amongst the other minimally invasive parathyroidectomy techniques available. Video-assisted lobectomy in the elderly.

STUDY OBJECTIVES: Video-assisted lobectomy for pulmonary malignancy seems to have potential merits, especially in compromised patients such as the elderly. This study was undertaken to assess the feasibility of this new approach in this special age group. PATIENTS: Video-assisted lobectomy was attempted in eight patients older than 78 years of age with preoperatively diagnosed T1N0 or T2N0 lung carcinomas of peripheral origin. In one patient with squamous cell carcinoma, the tumor was diagnosed after surgery as metastasis from tongue carcinoma. The patients ranged in age from 78 to 85 years (

[Pros/Cons: Airway Management for Separate-side Ventilation: Always Use a Video Laryngoscope?].

The optimal technique for airway management during intubation with a double-lumen tube (DLT) in thoracic anesthesia remains unclear. While efforts are now being made to use a video laryngoscope for routine use with single-lumen tubes, its use with DLTs remains a topic of debate. This article presents an analysis from two perspectives in the style of a debate, in this case with a slight wink as a plea in court, comparing video laryngoscopy (VL) with direct laryngoscopy (DL) for routine use. The "defense position" highlights evidence demonstrating improved visibility of the glottis, higher succe

The expanding role of robotic surgery in thymectomy: a narrative review.

BACKGROUND AND OBJECTIVE: Surgical resection for thymic epithelial tumors has traditionally been performed via a median sternotomy or video-assisted thoracoscopic surgery (VATS). In recent years, however, robot-assisted thoracoscopic surgery (RATS) has been increasingly adopted as a minimally invasive alternative, with a growing number of reports. This review summarizes the progress achieved to date and outlines future perspectives in robot-assisted thymectomy. METHODS: This narrative review was conducted through a structured literature search of PubMed, including articles published up to Nov

Minimally invasive surgical procedures for thymic disease in Asia.

Video-assisted thoracic surgery (VATS) procedures for thymic tumors and myasthenia gravis were introduced in Asia in the middle 1990s in at least two regions, Hong Kong and Japan. To overcome difficulties in obtaining a wide view of the anterior mediastinum, several methods for lifting the sternum or anterior chest wall have been presented, mainly by Japanese surgeons. More recently, single port VATS through a subxiphoid incision was also introduced in Japan. The long-term outcome of a VATS extended thymectomy for myasthenia gravis has been shown to be comparable to that of a trans-sternal ext
