Add-on efficacy of doravirine in PLWHIV with virological failure and low-level viremia.

BACKGROUND: Doravirine is a non-nucleoside reverse transcriptase inhibitor (NNRTI) with moderate plasma protein binding and good potential for anatomical sanctuary penetration. MATERIALS AND METHODS: Adding doravirine to existing antiretroviral therapy (ART) regimens has led to virological success in two groups of people living with HIV (PLWHIV): those with recent virological rebound (n = 84) and those with prolonged low-level viremia (LLV, n = 39). Doravirine was added without any modification to the other ART drugs. RESULTS: After 6 months, 88% of PLWHIV in the recent failure group had ach

Assessing B19V epidemiology and viremia dynamics in plasma donors: A comprehensive analysis.

BACKGROUND: Parvovirus B19 (B19V) infection is characterized by measurable bloodstream biomarkers, which vary over time indicating different phases of infection. High B19V DNA levels in the bloodstream are associated with infectivity, impacting the eligibility of source plasma donors. OBJECTIVES: Characterize temporal changes in B19V epidemiological rates from January 2018 to April 2025 and to investigate the longitudinal dynamics of the diagnostic biomarkers and infectivity in subclinical infections among US source plasma donors. RESULTS: B19V positivity rates in CSL's US source plasma dono

Effects of COVID-19 mRNA vaccination on HIV viremia and reservoir size.

OBJECTIVE: The immunogenic nature of COVID-19 mRNA vaccines led to some initial concern that these could stimulate the HIV reservoir. We analyzed changes in plasma HIV loads (pVL) and reservoir size following COVID-19 mRNA vaccination in 62 people with HIV (PWH) receiving antiretroviral therapy (ART), and analyzed province-wide trends in pVL before and after the mass vaccination campaign. DESIGN: Longitudinal observational cohort and province-wide analysis. METHODS: 62 participants were sampled pre-vaccination, and one month after their first and second COVID-19 immunizations. Vaccine-induce

Cytomegalovirus viremia and advanced HIV disease: is there an argument for anti-CMV treatment?

INTRODUCTION: The role of cytomegalovirus (CMV) infection as a co-factor in HIV disease has been a topic of considerable interest since the beginning of the HIV pandemic. CMV is believed to function both as a co-factor in the progression of HIV infection, and as a contributor to enhanced disease for other opportunistic infections. AREAS COVERED: In this special article, we review several recent studies that have enhanced our understanding of the role that CMV infection plays in the natural history of other HIV-related opportunistic infections. We review the clinical evidence that demonstrates

Impact of HCV viremia on HBV biomarkers in patients coinfected with HBV and HCV.

BACKGROUNDS: Hepatitis B virus (HBV) biomarkers reflect the status of HBV infection; however, their role in patients with chronic hepatitis B and C (HBV/HCV) coinfection remains unknown. This study evaluated the characteristics of HBV biomarkers in patients with chronic HBV/HCV coinfection. METHODS: One hundred untreated HBV/HCV coinfected patients were enrolled. Active viral infection was defined as viral load above 2000 U/L and 15 U/L for HBV and HCV, respectively. Blood samples were analyzed for HBV biomarkers, including hepatitis B surface antigen (HBsAg), hepatitis B core-related antigen

Factors associated with viremia in people living with HIV on antiretroviral therapy in Guatemala.

INTRODUCTION: Viral suppression prevents HIV transmission and disease progression, but socio-economic and clinical factors can hinder the goal of suppression. We evaluated factors associated with viral non suppression (VNS) and persistent viremia (PV) in people living with HIV (PLHIV) receiving antiretroviral therapy (ART) in Guatemala. METHODS: We conducted a cross sectional analysis using data from an ongoing cohort of PLHIV attending the largest HIV clinic in Guatemala. Univariable and multivariable analyses were conducted between PLHIV with viral suppression and detectable viremia. VNS wa

[Research status and challenges of low-level viremia in chronic hepatitis B].

Persistent low-level viremia during chronic hepatitis B therapy can promote drug-resistant mutations, liver fibrosis progression and even hepatocellular carcinoma occurrence, which further seriously affect the prognosis of patients. Herein, we hope to clarify and analyze the possible occurrence mechanisms and clinical outcomes of low-level viremia by sorting out the key points, so as to assist in the adjustment of individualized treatment plans and provide references for the diagnosis and treatment.

Epidemiology of Multiple Herpes Viremia in Previously Immunocompetent Patients With Septic Shock.

BACKGROUND: Systemic reactivations of herpesviruses may occur in intensive care unit (ICU) patients, even in those without prior immune deficiency. However, the clinical relevance of these events is uncertain. METHODS: In this study we selected patients admitted with septic shock and treated for more than 4 days from a prospectively enrolled cohort of consecutive adults in the mixed ICUs of 2 tertiary care hospitals in the Netherlands. We excluded patients who had received antiviral treatment in the week before ICU admission and those with known immunodeficiency. We studied viremia episodes w

Impaired Nef function is associated with early control of HIV-1 viremia.

UNLABELLED: Host and viral factors influence the HIV-1 infection course. Reduced Nef function has been observed in HIV-1 controllers during the chronic phase, but the kinetics and mechanisms of Nef attenuation in such individuals remain unclear. We examined plasma RNA-derived Nef clones from 10 recently infected individuals who subsequently suppressed viremia to less than 2,000 RNA copies/ml within 1 year postinfection (acute controllers) and 50 recently infected individuals who did not control viremia (acute progressors). Nef clones from acute controllers displayed a lesser ability to downreg

HSV-1 viremia as a potential cause of febrile neutropenia in an immunocompromised child.

Although the standard of care in febrile neutropenic patients includes the initiation of empirical antibacterial and antifungal therapy, many patients do not respond and need further diagnostic work up and treatment. Here, we report on an immunosuppressed neutropenic patient with a prolonged episode of fever unresponsive to empirical antibacterial therapy. Herpes polymerase chain reaction revealed systemic reactivation of herpes simplex virus type 1 (HSV-1) infection and treatment with acyclovir was associated with the prompt resolution of signs and symptoms of infection. Screening for HSV in

Adenovirus type 3 viremia in an adult with toxic shock-like syndrome.

Surveillance by the Unexplained Deaths and Critical Illnesses Project (UNEX) uncovered a novel presentation of adenovirus type 3 infection that satisfied the criteria for toxic shock-like syndrome in a 28-year-old immunocompetent man. Adenovirus may be a cause of toxic shock syndrome; surveillance systems such as UNEX may uncover additional causes of this and other clinically defined infectious syndromes.

[Comparative study of the viremia in tick-borne encephalitis in immune and nonimmune animals].

A long-term viremia (12 days postinoculation, the observation period) of both cellular and plasma type was detected in mice experimentally infected with tick-borne encephalitis virus, the Sophyin strain. Preliminary vaccination was found to prevent viremia in experimentally infected animals. Comparative studies of cellular and humoral immune responses in vaccinated mice showed the cellular immune responses to be most important in eliminating the virus upon subsequent experimental inoculation of mice with tick-borne encephalitis virus.

5' leader defects drive persistent HIV-1 viremia on long-term ART.

Traces of HIV-1 RNA can persist in plasma despite long-term suppressive antiretroviral therapy (ART). Some individuals develop nonsuppressible viremia (NSV), characterized by detectable HIV-1 RNA that raises concerns for virological failure, pathogenesis, and transmission. The sources of NSV remain poorly defined, in part due to limited tools to characterize plasma HIV-1 RNA. Both infectious and defective proviruses, including those with defects in the 5' Leader (5'L), can contribute to NSV, but their relative contributions have not been quantified. Here we show that in over 50 participants, p

Association between cytomegalovirus viremia and long-term outcomes in lung transplant recipients.

Although cytomegalovirus (CMV) viremia/DNAemia has been associated with reduced survival after lung transplantation, its association with chronic lung allograft dysfunction (CLAD) and its phenotypes is unclear. We hypothesized that, in a modern era of CMV prophylaxis, CMV DNAemia would still remain associated with death, but also represent a risk factor for CLAD and specifically restrictive allograft syndrome (RAS)/mixed phenotype. This was a single-center retrospective cohort study of all consecutive adult, first, bilateral-/single-lung transplants done between 2010-2016, consisting of 668 pa

[Natural history of chronic hepatitis B virus infection under low-level viremia].

Low-level viremia (LLV) is a hot and difficult topic that has gradually attracted attention in the field of chronic hepatitis B for evaluating antiviral therapy response in recent years. The presence of LLV may increase drug-resistant mutations, the progression of liver fibrosis, and potentially the development of liver cancer following antiviral therapy. Natural history of chronic HBV infection patients also have LLV, but it is unclear whether these patients are also at risk of disease progression, what the risk is, and whether early antiviral therapy is necessary and beneficial. Therefore, t
