Monocular involuntary eyelid closure.

OBJECTIVES: Monocular blepharospasm (MBLS) is precipitated by ipsilateral ocular pathology and becomes bilateral over time. Two patients are reported with monocular visual disturbance exhibiting monocular involuntary eyelid closure (MIEC), resembling MBLS. MATERIAL AND METHODS: A 31-year-old female with hyperthyroidism developed monocular polyopia with left eye vision on binocular fixation followed by ipsilateral visual extinction. Monocular vision was otherwise normal. A 57-year-old male with renal failure developed monocular blindness secondary to retinal hemorrhage. RESULTS: The first pat

Monocular band optic atrophy.

Band or "bow tie" optic atrophy is characterized by well-described ophthalmoscopic findings in the optic nerve and nerve fiber layer and homonymous hemianopia. It is typically associated with compressive lesions of the pregeniculate postchiasmal visual pathway or, less commonly, congenital malformations affecting the postgeniculate radiations or cortex. A unique case with band optic atrophy is described because of the unilateral visual defect. The optic atrophy is strictly unilateral and without an obvious structural lesion that could explain the optic disc damage. However, incidental cerebral

Unfixed reference, monocular occlusion, and developmental dyslexia--a critique.

Stein and Fowler have proposed that poor binocular control of vergence eye movements is responsible for reading problems in a subset of dyslexic children, and that this subgroup is characterised by unstable performance on Dunlop's reference eye test. Four predictions from this hypothesis are evaluated in the light of published evidence. First, it is shown that a substantial minority of good readers have unfixed reference. Second, the evidence for a raised prevalence of unfixed reference in dyslexics is reviewed and contradictory findings are discussed. Third, it is argued that there is little

Comparative analysis of structural modifications induced by monocular retinal inactivation and monocular deprivation in the developing cat lateral geniculate nucleus.

During a critical period of postnatal life, monocular deprivation (MD) by eyelid closure reduces the size of neurons in layers of the dorsal lateral geniculate nucleus (dLGN) connected to the deprived eye and shifts cortical ocular dominance in favor of the non-deprived eye. Temporary inactivation of the non-deprived eye can promote superior recovery from the effects of long-term MD compared to conventional occlusion therapy. In the current study, we assessed the modification of neuron size in the dLGN as a means of measuring the impact of a brief period of monocular inactivation (MI) imposed

Effects of monocular deprivation on the visual fields of squirrel monkeys.

Five squirrel monkeys were monocularly deprived at birth for 3 years. Visual field testing for the deprived eye revealed no responses to visual stimuli at any position, including the monocular segment. These results are similar to those obtained in macaque monkeys after long-term neonatal monocular deprivation and indicate that lack of patterned visual input to an eye during development in primates can produce functional blindness throughout the visual field. Because the monocular segment of the visual field was not spared, binocular competition cannot be the only mechanism underlying this los

Is the phakic eye normal in monocular pediatric aphakia?

This retrospective review of 97 pediatric patients who underwent monocular surgery for congenital or developmental cataracts studied the incidence of abnormalities in the contralateral phakic eye. Fifty-nine percent of patients had a normal phakic eye. However, 40 patients showed at least one abnormality in the phakic eye: reduced vision (21%), nystagmus (19%), cataract (15%), iris heterochromia (9%), myopia (6%), microphthalmos (6%), pupillary miosis (2%), congenital glaucoma (2%), optic nerve abnormality (2%), aniridia (1%), and corneal opacity (1%). Not all abnormalities were detected at th

Blackout: Understanding transient vision loss.

BACKGROUND: Transient vision loss may indicate underlying disease of the eye, optic nerve, orbit, brain or heart. Detailed history-taking followed by a complete ocular and neurological examination is therefore a crucial part of any consultation. OBJECTIVE: It is important to determine whether a patient with transient vision loss can be reassured or requires urgent referral for further investigation. This review examines monocular and binocular transient vision loss and provides a structured approach to the examination of a patient with transient vision loss. The aim of this article is to prov

Daily mixed visual experience that prevents amblyopia in cats does not always allow the development of good binocular depth perception.

Kittens reared with mixed daily visual input that consists of episodes of normal (binocular) exposure followed by abnormal (monocular) exposure can develop normal visual acuity in both eyes if the length of the former exposure exceeds a critical amount. However, later studies of the tuning of cells in primary visual cortex of animals reared in this manner revealed that their responses to interocular differences in phase were not reliable suggesting that their binocular depth perception may not be normal. We examined this possibility in 3 kittens reared with mixed daily visual exposure (2 hrs b

Endoscopic Endonasal Transsphenoidal Resection of Pituitary Adenomas in Patients Presenting With Monocular Blindness.

BACKGROUND AND OBJECTIVES: Suprasellar tumors, particularly pituitary adenomas (PAs), commonly present with visual decline, and the endoscopic endonasal transsphenoidal approach (EETA) is the primary management for optic apparatus decompression. Patients presenting with complete preoperative monocular blindness comprise a high-risk subgroup, given concern for complete blindness. This retrospective cohort study evaluates outcomes after EETA for patients with PA presenting with monocular blindness. METHODS: Retrospective analysis of all EETA cases at our institution from June 2012 to August 202

Monocular vernier acuity in normally binocular, monocular, and amblyopic subjects.

Previous experiments that have compared monocular vernier acuity in amblyopic, monocularly blind, and normal binocular subjects have been confined to the center of the retina. Based on brain changes that accompany the early disruption of form vision in one eye, monocularly blind subjects (that is, those who have completely lost the ability to process form vision in one eye) and amblyopic subjects are expected to have better vernier acuity in their normally functioning eye than subjects with normal binocular vision. Results from such studies have been ambiguous. This experiment was set up to st

Targeted reaching with monocular depth information and haptic feedback: Comparing between monocular patients and normally sighted observers.

Monocular blindness impairs visual depth perception, yet patients seldom report difficulties in targeted actions like reaching, walking, or driving. We hypothesized that by utilizing monocular depth information and calibrating actions with haptic feedback, monocular patients can perceive egocentric distance and perform targeted actions. We compared targeted reaching in monocular patients, monocular-viewing, and binocular-viewing normal controls. Sixty observers reached either a far or a near target, calibrating reaches to the near target with accurate or false feedback while leaving reaches to

Binocular deficits associated with early alternating monocular defocus. II. Neurophysiological observations.

Experiencing binocularly conflicting signals early in life dramatically alters the binocular responses of cortical neurons. Because visual cortex is highly plastic during a critical period of development, cortical deficits resulting from early abnormal visual experience often mirror the nature of interocular decorrelation of neural signals from the two eyes. In the preceding paper, we demonstrated that monkeys that experienced early alternating monocular defocus (-1.5, -3.0, or -6.0 D) show deficits in stereopsis that generally reflected the magnitude of imposed monocular defocus. Because thes

Compressive optic neuropathy caused by a nonectatic internal carotid artery: illustrative case.

BACKGROUND: Optic nerve compression from direct contact with the internal carotid artery (ICA) is a rarely described cause of monocular vision loss. Thus, little is known about the incidence, optimal management, and outcomes of this particular type of compressive optic neuropathy. OBSERVATIONS: The authors present the case of a 52-year-old female with progressive monocular vision loss. After an extensive workup, including a formal neuro-ophthalmology assessment, her symptoms were attributed to a neurovascular conflict between the optic nerve and a nonectatic supraclinoid ICA. She underwent a

Partial Occlusion of a Third Nerve Palsy, a Shortcut through the Swedish Legal Vision Requirements for Driving.

In this report, we present a case of a young woman with a traumatic third nerve palsy. A substantial practical dilemma for her was that according to the Swedish vision requirements for driving she was not allowed to drive due to diplopia within central 30 degrees of gaze. If she occluded the paretic eye to avoid diplopia, she would need to constantly occlude the eye for an adaptation period of six months before she would be allowed to drive with monocular vision. To get by this problem, the eyeglass of the paretic eye was partially occluded. In her case, this eliminated all diplopia within the
