Journal of voice : official journal of the Voice Foundation2016eng
BACKGROUND: Friedreich Ataxia (FRDA) is the most common hereditary ataxia, with dysarthria as one of its key clinical signs.
OBJECTIVE: To describe the voice profile of individuals with FRDA to inform outcome marker development and goals of speech therapy.
METHODS: Thirty-six individuals with FRDA and 30 age-matched controls provided sustained vowel and connected speech samples. Speech and voice samples were analyzed acoustically using the Analysis of Dysphonia in Speech and Voice program and perceptually using the Consensus Auditory-Perceptual Evaluation of Voice form. Correlations between

Aasthma is one of the most common chronic diseases with a prevalence of 5% in Germany. Nearly half of the patients complain about permanent voice disorders. Mucosal changes due to the obstructive respiratory disease as well as mucus abnormalities and regularly accompanying chronic rhinosinusitis may explain these symptoms. The additional influence of laryngopharyngeal reflux is discussed controversially. Additionally, dysphonia may as well occur due to side effects of the therapy with inhaled corticosteroids: the ingredients as well as physical effects may be responsible for the development of

Pediatric patients with voice or speech problems usually should receive a team assessment in which communication between the pediatrician or primary care physician, the otolaryngologist, and speech pathologist occurs. Although speech or voice problems may prompt an otolaryngologic evaluation, the voice or speech problem simply may be the manifestation or symptom of a larger or more complex disease process. Whether that is the case of hypernasal speech, eventually leading to the diagnosis of velocardiofacial syndrome, or bilateral vocal fold paralysis, eventually leading to the diagnosis of hyd

This report describes a voice evaluation procedure that in some way parallels the audiologic tests used for hearing and has multiple uses both clinically and in research. It uses a simultaneous eight-channel input, is not difficult to use, requiring between 12 and 25 minutes to administer, and provides the physician with a printout in standardized form before the patient leaves the room. This three-page report includes 15 abstracted or calculated values, normal ranges by sex for each value, notes that draw attention to deviations from the normal, a summary profile, a graphic representation of

Although various methods of alaryngeal communication after laryngectomy have been described, little attention has been given to voice restoration after laryngopharyngoesophagectomy. This article deals with our experience reconstructing the hypopharynx and cervical esophagus in five patients by the use of microvascular free tissue grafts. Voice restoration procedures with use of a Blom-Singer prostheses were used uniformly and resulted in the attainment of functional voice in each patient. It is our feeling that this method should be used equally in patients undergoing both traditional laryngec

Surgical voice restoration is possible years after laryngectomy by using hypopharyngeal puncture and cricopharyngeal myotomy if necessary. We have developed a technique of insertion of the ESKA-Herrmann tracheostoma valve for fingerfree speech. The patient moistens and cleans the tracheostoma valve every day. The quality of fingerfree speech is the best of all known techniques.

OBJECTIVE: To determine differences in voice disorders in patients after thyroidectomy, with recurrent laryngeal nerve preservation, divided into 2 groups, those who underwent lobectomy (LO) vs patients who underwent total thyroidectomy (TT).
METHODS: In our prospective study, we compared subjective and objective parameters of voice disorders. Our study included 61 subjects who underwent thyroidectomy (n (LO) = 31, n (TT) = 30) at the Department of ENT and Head and Neck Surgery University Hospital Center Zagreb. The research was conducted at four time points: 1st measurement-7 days before sur

The primary movement disorders affecting the voice are laryngeal dystonia (including spasmodic dysphonia), essential voice tremor, and Parkinson disease. Diagnosis of these conditions is clinical and based on a detailed history, voice evaluation, and physical and laryngoscopic examination. Laryngeal dystonia and essential voice tremor are hyperfunctional disorders and are treated most commonly with botulinum toxin injections. Parkinson disease is a hypofunctional disorder that may affect the voice and most commonly is treated with Lee Silverman Voice Treatment.

Communication changes are an important feature of Parkinson's and include both motor and nonmotor features. This chapter will cover briefly the motor features affecting speech production and voice function before focusing on the nonmotor aspects. A description of the difficulties experienced by people with Parkinson's when trying to communicate effectively is presented along with some of the assessment tools and therapists' treatment options. The idea of clinical heterogeneity of PD and subtyping patients with different communication problems is explored and suggestions are made on how this ma

BACKGROUND: Prevalence of paediatric voice disorders has been reported as 6-9% in children of school age. The appropriate diagnosis and management of paediatric voice disorders is essential for progress in education and psychosocial development. This paper presents a review of a UK tertiary paediatric voice clinic experience of referral patterns, diagnosis, management and socioeconomic variations.
METHODS: An audit of 195 consecutive appointments (154 were new patients) between October 2009 and September 2013 at a monthly paediatric voice clinic based at the Royal Hospital for Sick Children i

INTRODUCTION: Dysphonia is the main symptom of the disorders of oral communication. However, voice disorders also present with other symptoms such as difficulty in maintaining the voice (asthenia), vocal fatigue, variation in habitual vocal fundamental frequency, hoarseness, lack of vocal volume and projection, loss of vocal efficiency, and weakness when speaking. There are several proposals for the etiologic classification of dysphonia: functional, organofunctional, organic, and work-related voice disorder (WRVD).
OBJECTIVE: To conduct a literature review on WRVD and on the current Brazilian

This article reviews the management of voice disorders in children. We describe the relevant anatomy and development of the larynx throughout childhood, which affects voice. We consider the epidemiologic data to establish the size of the problem. The assessment of the patient in the clinic is described stepwise through the history, examination, laryngoscopy, and extra tests. We then review the common voice disorders encountered and their management, concluding with discussion of future directions, which may herald advances in this field.

Voice disorders in the pediatric population are relatively common. The education of families, teachers and clinical staff on etiology and treatment of pediatric voice disorders have led to greater attention being paid to hoarseness in childhood and improving early detection of pediatric voice disorders. Pediatric voice problems can have a number of causes. Most commonly, childhood dysphonia is caused by vocal fold nodules due vocal ab- and misuse. Other reasons might be congenital laryngeal dysplasia, vocal fold cysts and laryngeal papilloma. Medical examination is necessary in order to initia

OBJECTIVE: Patients of laryngeal cancer who have undergone the surgical removal of the entire larynx suffer the loss of phonation. Electrolarynx (EL) speech is the most commonly adopted alaryngeal phonation. However, EL speech is notorious of the sound quality being monotonic and robotic with the lack of pitch control and the presence of the radiated noise. This paper provides a review of modalities in EL speech as well as introducing the technologies to control the pitch and reduce the noise of the device.
METHODS: Improvements of EL speech quality have been divided into two parts: improving

This article reviews the literature on the efficacy of treatment for voice disorders primarily using studies published in peer-reviewed journals. Voice disorders are defined, their frequency of occurrence across the life span is reported, and their impact on the lives of individuals with voice disorders is documented. The goal of voice treatment is to maximize vocal effectiveness given the existing disorder and to reduce the handicapping effect of the voice problem. Voice treatment may be (a) the preferred treatment to resolve the voice disorder when medical (surgical or pharmacological) treat

Speech therapists have traditionally played a major part in voice restoration after laryngectomy. Nurses have a potentially complementary role to play, which is explored in this article. The three main methods of voice restoration are outlined and the nurse's role in the management of laryngectomy patients is discussed.

The pediatric otolaryngologist has an especially important role in the differential diagnosis and treatment of two voice disorders; these are the voice quality problems (dysphony) and the resonance problems (rhinophony). The first step in the examination is to preclude the organic causes. The functional dysphonia is mostly related to voice abuse/misuse, but may be present on a psychosomatic basis; environmental factors can also play a role in the etiology and the personality structure has been found to be very relevant. The perceptual evaluation of voice is of obvious importance. Endoscopy wit
