Reflux and Voice Disorders: Have We Established Causality?

A trend of attributing abnormal voice changes to reflux has gained momentum among medical professionals over the last few decades. Evidence supporting the connection between reflux and voice and the use of anti-reflux medication in patients with dysphonia is conflicting and deserves careful examination. In the current health care environment, it is important that medical decisions be based on science rather than anecdote and practice patterns. The goal of this review is to investigate the evidence linking reflux and voice changes. Specifically, this association will be examined in the context

An update in thinking about nonorganic voice disorders.

OBJECTIVE: To describe and evaluate psychosocial factors in nonorganic voice disorders (NVDs). Nonorganic voice disorders are presumed to be the result of increased muscular tension that is caused to varying extents by vocal misuse and emotional stress. It is therefore necessary to include both of these in the diagnosis and treatment of patients with voice disorders. DESIGN: Clinical survey. SETTING: Academic tertiary referral center. PATIENTS: To evaluate psychosocial factors in NVDs, a sample of 74 patients with NVDs was examined psychologically using the Giessen Test and Picture Frustrat

Personality and voice disorders: a superfactor trait analysis.

To determine whether personality factors play causal, concomitant, or consequential roles in common voice disorders, a vocally normal control group and four groups with voice disorders-functional dysphonia (FD), vocal nodules (VN), spasmodic dysphonia (SD), and unilateral vocal fold paralysis (UVFP)-were compared on measures of personality and psychological adjustment. Superfactor group comparisons revealed that the majority of FD and VN subjects were classified as introverts and extraverts, respectively. Comparisons involving the SD, UVFP, and control subjects did not identify consistent pers

pH monitoring in patients with benign voice disorders.

The aim of this study was to compare oesophageal pH-metry with laryngeal signs and symptoms in patients suspected of laryngeal reflux disease. A total of 60 patients with voice disorders, who were suspected of laryngeal reflux, were tested by single probe oesophageal pH monitoring. Thirty-two suffered from reflux laryngitis. A comparison of symptoms in patients with proven reflux to patients with no reflux was made. The symptoms were more frequent in the patients in the reflux group than in the non-reflux group. There was a significant difference between the groups with regard to dysphonia, so

Personality and voice disorders: a multitrait-multidisorder analysis.

To examine whether personality has causal, concomitant, or outcome status in common voice disorders, a vocally normal control group and 4 groups with voice disorders--functional dysphonia (FD), vocal nodules (VN), spasmodic dysphonia (SD), and unilateral vocal fold paralysis (UVFP)--were compared using the Multidimensional Personality Questionnaire (MPQ). Univariate and discriminant analyses confirmed that the FD and VN groups differed significantly from each other, the controls, and the other voice-disordered groups at both a superfactor and lower-trait level of personality description. [Present day diagnosis of voice disorders in children].

Videoendostroboscopy (VESS) was employed for diagnosis of vocal dysfunction in 369 children aged 6 to 16 years. Effects of flexible and hard endoscopes were compared. Vibratory cycle was analyzed in 55 healthy children of different age. Diagnosis and differential diagnosis of soft and hard nodules, physiological and long-term mutation, chronic laryngitis are described as well as a videoendostroboscopic picture in functional dysphonias, vocal grooves and polyps. VESS is a modern, informative and safe diagnostic procedure.

[Voice disorders in children with velopharyngeal valving problems].

The purpose of this study was to examine if velopharyngeal valving problem can affect voice production (phonation) to some extent. Seventeen cleft palate children, mean age 7.0 had developed hoarseness for years without any treatment. In this study, each subject received a complete voice evaluation and the velopharyngeal (VP) function assessment. Voice evaluation included vocal cords condition and four voice parameters (quality, intensity, fundamental frequency and phonation behaviors). V-P function, classified into 3 levels (adequate, marginal & inadequate), was based on speech evaluation and

[Pharmacotherapy of voice and articulation disorders in aphasia].

The phonation and articulation disorders in a group of aphasics with aphonia/dysarthrophonia could be considerably reduced by the use of medication. The group consisted of 10 apoplectic patients whose resulting aphasia could not be classified because of the vocal impairment. Extrapyramidal motion disorders were proved by laryngoscopy and stroboscopy. A definite improvement of phonation and articulation was observed after L-dopa medication.

Vocal efficiency and aerodynamic aspects in voice disorders.

The variation of vocal efficiency for the mean values for normal subjects was investigated in patients with laryngeal disease. The relative contributions of mean flow rate and intrapulmonic pressure to the variation of efficiency were explored to explain aerodynamic aspects in voice disorders. Vocal efficiency was determined by use of expressions involving simultaneous values of sound pressure level, mean flow rate, and intrapulmonic pressure. The intrapulmonic pressure was noninvasively obtained by plethysmographic and pneumotachographic methods. Values of vocal efficiency were generally abno

Functional voice disorders. A review of 109 patients.

The records of 109 patients, presenting with functional voice disorder during the years 1977-1981, have been reviewed. The mean age was 45 years and the F : M ratio is 2 : 1. Excess voice use was not obviously an aetiological factor. Sixty-one of the 109 patients (56 per cent) were cured by speech therapy, in that their voices returned to their pre-morbid state, and a further 28 (26 per cent) were improved by therapy; seven patients (6 per cent) did not improve and 13 (12 per cent) were lost to follow-up. Of the patients that were 'cured', 54 per cent were discharged after three months' treatm

Conventional Versus Telepractice Delivery of Eclectic Voice Therapy in Hyperfunctional Voice Disorders: An Efficacy Study.

OBJECTIVES: Literature supports the use of eclectic voice therapy in the treatment of Hyperfunctional Voice Disorders (HFVD). Nevertheless, limited studies have assessed and compared outcomes of conventional and telepractice approaches in the management of HFVD. Therefore, the current study aimed to examine and compare the efficacy of an eclectic voice therapy program through conventional versus telepractice mode in HFVD. METHOD: In total, 40 individuals with HFVD aged 18-60 years attended 15 sessions of the eclectic voice therapy program, with each session lasting 40 minutes. About 20 partic

Checking for voice disorders without clinical intervention: The Greek and global VHI thresholds for voice disordered patients.

Voice disorders often remain undiagnosed. Many self-perceived questionnaires exist for various medical conditions. Here, we used the Greek Voice Handicap Index (VHI) to address the aforementioned problem. Everyone can fill in the VHI questionnaire and rate their symptoms easily. The innovative feature of this research is the global cut-off score calculated for the VHI. Therefore, the VHI is now capable of helping clinicians establish a more customizable treatment plan with the cut-off point identifying patients without normal phonation. For the purpose of finding the global cut-off point, a gr

The Minimal Important Difference of Acoustic Voice Quality Index in the Treatment of Voice Disorders.

OBJECTIVES: The acoustic voice quality index (AVQI) is a reliable tool that objectively assesses dysphonia levels using six acoustic parameters. Despite its high criterion-related concurrent validity, diagnostic accuracy, and minimal detectable change derived from test-retest reliability, the minimal important difference (MID) of the AVQI has not been tested before. This study aimed to estimate the MIDs of AVQI for improvement audibly perceived by clinicians and self-reported improvement by patients. METHODS: A retrospective study was conducted on 110 patients who received treatment for voice

Clinical Psychology and Voice Disorders: A Meta-Analytic Review of Studies Assessing Psychological Characteristics Across Individuals With and Without Voice Disorders.

OBJECTIVES: Clinical voice disorders are heterogenous conditions capturing problems with voice production and control. Psychological conceptualizations of voice disorders posit that mood, anxiety, and personality characteristics contribute to the development and maintenance of voice symptoms. This review brings together research comparing these psychological characteristics across groups with and without voice disorders, with the aim of profiling group differences. METHODS: A systematic search of PubMed, CINAHL, Ovid (PsycInfo, MEDLINE, Embase), and Web of Science databases was conducted, wit
