Tuberous sclerosis and fibrous dysplasia.

We present the case of an 11-year-old female patient with tuberous sclerosis who had a right nasal mass. CT examination revealed fibrous dysplasia involving the frontal, ethmoid, sphenoid, and vomer bones. Biopsy findings of the mass confirmed this diagnosis, and follow-up revealed marked expansion of these lesions. The authors emphasize the association of bone abnormalities and tuberous sclerosis and discuss the consideration of fibrous dysplasia as a component of this syndrome.

The influence of different types of hard palate closure in two-stage palatoplasty upon palatal growth: dental cast analysis.

We investigated the influence of different types of hard palate closure in two-stage palatoplasty. In 12 cases the lip and soft palate were closed at the age of 3 to 7 months; these 12 were then divided into two groups. The hard palate was closed at the age of 1 year 5-11 months by the vomer flap with skin graft in the vomer flap (VF) Group (Osada's two-stage palatoplasty), and by push-back procedure of the mucoperiosteal flap in the push-back (PB) Group. Dental casts of the two groups were measured and compared at the ages of 0, 1, 3-4, and after 10 years. 1) In spite of apparent differences

Nasal Septal Anatomy in Skeletally Mature Patients With Cleft Lip and Palate.

IMPORTANCE: Septal deviation commonly occurs in patients with cleft lip and palate (CLP); however, the contribution of the cartilaginous and bony septum to airway obstruction in skeletally mature patients is poorly understood. OBJECTIVES: To describe the internal nasal airway anatomy of skeletally mature patients with CLP and to determine the contributors to airway obstruction. DESIGN, SETTING, AND PARTICIPANTS: This single-center retrospective review included patients undergoing cone-beam computed tomography (CBCT) from November 1, 2011, to July 6, 2015, at the cleft lip and palate division

Computed tomography in the evaluation of choanal atresia.

Nine patients with choanal atresia or stenosis, ranging in age from two days to 6 years were evaluated by computer tomography (CT). The clinical, conventional radiographic and surgical findings were correlated with CT findings. In all patients, the complete anatomic abnormality and the thickness of the obstruction were better defined with CT than with a choanogram. Findings on CT included: thickening of the vomer, bowing of the lateral wall of the nasal cavity and fusion of bony elements in the choanal region. This information is essential in planning surgical correction.

New Technique in Nasal Tip Plasty: Sandwich Technique Using Cartilage and Septal Bone Complex.

OBJECTIVES: It is extremely difficult to obtain ideal tip projection and nose lengthening by septal extension graft using only septal cartilage in Asians, because their nasal septal and alar cartilages are small and weak. Therefore, we introduce a new septal extension graft using a cartilage-bone complex with the vomer and the perpendicular plate of the ethmoid bone as well as cartilage to obtain optimal outcomes. METHODS: Participants included 30 patients who underwent surgery by external approach. The septal cartilage, vomer, and perpendicular plate of the ethmoid bone harvested by septopla

Surgery for correction of bilateral choanal atresia.

A transnasal approach was employed in 28 neonates (26 blacks and 2 whites) with bilateral choanal atresia. In each patient the atresia was perforated microsurgically with the drill, followed by insertion of a Portex endotracheal tube for a period of three months. By this method a satisfactory nasal respiration was established immediately. Removal of the posterior part of the vomer is crucial to the successful result. During the follow-up, one death was recorded in a microcephalic neonate, unrelated to the operation and another four patients required repositioning of the tube under general anes

A Radioanatomical Study of Nasal Septal Cartilage: Influencing Factors and Clinical Implications.

BACKGROUND: The septal cartilage is the most common and advantageous donor site for autogenous cartilage grafts in rhinoplasty. OBJECTIVES: We aimed to examine the parts of nasal septum of Turkish patients and assess the association between the size of these structures and patient characteristics such as age and sex. Through this, we investigated whether some external features of the nose could be useful in predicting the septal cartilage area (SCA). METHODS: Paranasal sinus computerized tomography (PNCT) images of 174 patients were examined with Radiant Dicom Viewer 2024.1. The "multiplanar

Repair of complete bilateral cleft lip with severely protruding premaxilla performing a premaxillary setback and vomerine ostectomy in one stage surgery.

BACKGROUND: The authors present a technique for selected cases of CBCL. The primary repair of the CBCL with a severely protruding premaxilla in one stage surgery is very difficult, essentially because a good muscular apposition is difficult, forcing synchronously to do a premaxillary setback to facilitate subsequent bilateral lip repair and, thus, achieving satisfactory results. We achieve this by a reductive ostectomy on the vomero-premaxillary suture. MATERIAL AND METHODS: 4 patients with CBCL and severely protruding premaxilla underwent premaxillary setback by vomerine ostectomy at the sam

Partial duplication of the face: case report and review.

Complete or partial facial duplication is a rare congenital malformation. A spectrum of structural abnormalities varying in degrees of severity has been described in affected individuals. We present discordance for facial duplication between monozygotic twins in which maxillary and mandibular duplication was present in one. The involved twin showed the following findings: ocular hypertelorism, bifidity of the nose, duplication of the maxilla, macrostomia, cleft of the lower lip, hamartoma of the vomer, supernumerary teeth, duplication of the mandibular teeth, bifidity of the tongue, and hamart

The evaluation of choanal atresia by computed tomography.

The radiographic evaluation of choanal atresia has traditionally consisted of a nasopharyngeal contrast study, however, this procedure has certain diagnostic limitations. The contrast nasopharyngogram does not define the type or thickness of the atresia. Computed tomography (CT) accurately characterizes the nature and thickness of the atresia, and visualizes the associated thickening of the vomer and narrowing of the posterior nasal cavity. CT evaluation with three dimensional reconstruction adds an additional diagnostic dimension. Seven patients have been evaluated by CT. The results show CT

Cleft lip and palate treatment with delayed closure of the hard palate. A preliminary report.

A new routine for the surgical treatment of cleft lip and palate patients is presented. Beside the regular lip-nose surgery it includes delayed closure of the cleft in the hard palate and soft palate closure without push-back technique. The effect on early facial growth was evaluated by comparing eight of these patients, three years of age, with a matched group where the hard and soft palate was closed primarily by vomer flap and velar push-back. The new treatment method resulted in significantly less teeth in crossbite occlusion due to a wider maxillary dental arch. As yet no serious disadvan

Congenital choanal atresia.

Bilateral choanal atresia is a medical emergency. With good nursing care and close observation, surgery may be delayed until it is deemed safe and there is a good chance of success. Surgical treatment of unilateral atresia is better delayed to optimize the conditions for successful surgery. There are three surgical approaches for the correction of this anomaly: transnasal, transpalatal, and trans-septal. Each has advantages and disadvantages. The patient's age is an important consideration in the choice of the technique. Partial vomer removal minimizes the likelihood of restenosis. It is widel

Sphenovomerine suture: a useful landmark for locating sphenoid sinus ostium.

AIM: The aim of this study was to determine whether the sphenovomerine suture (SVS) can be used as a landmark to localize the sphenoidal sinus ostium. METHODS: Endoscopic imaging was done on 152 skulls to identify ostium of the sphenoidal sinus, the SVS, and the articulation of sphenoidal process of palatine bone between the body of the sphenoid and the sphenopalatine foramen. The variables were as follows: (1) the distance between the medial border of the ostium and SVS (DSO-SVS); (2) the angle between them (ASO-SVS); (3) the distance between the inferior border of the ostium and the horizon

Carpentering in septorhinoplasty: a novel technique to straighten the deviated nasal septum.

Septal deviation presents a considerable challenge in septorhinoplasty because it leads to functional and esthetic problems. This deformity may displace the nasal tip, disturb nasal valve patency, and affect the final outcomes of septorhinoplasty. This report describes an innovative technique that can be used in patients with C-shaped deviation of the septum in a cephalocaudal direction. Because the procedure is similar to carpentering, it was named a "carpenter spreader graft." In this technique, the deviated dorsal cartilage is separated and displaced to a new straightened position.
