Fever of unknown origin and the role of Plasmodium vivax in Sao Paulo.

Malaria is a disease that hits the northern part of Brazil in an endemic form. Recorded cases outside this region are unusual and, in most cases, limited to travellers. The southeast of Brazil has an extensive wilderness area with a type of vegetation similar to the one found in the equatorial rainforest. We present the case of a patient living in the southeast, with no reported travel history, who evolved to periodic fever, associated with myalgia and chills. With the positive test result for Plasmodium, the treatment for malaria was started. This case enabled the study in the Anopheles mosqu

[Clinicopathological analysis of 20 cases of giant cell granuloma of the jaw].

The pathological features of 20 cases of giant cell granuloma and 12 cases of giant cell tumor of the jaw were compared. Specimens of 8 cases of giant cell granuloma and 5 cases of giant cell tumor were stained with Wilder's reticular fiber stain and trichromic stain. The results showed that reticular fibers and collagenous fibers were more abundant and more focal in distribution in giant cell granulomas than in those of giant cell tumors. It is concluded that the two staining methods adopted are helpful in the differential diagnosis between these two kinds of diseases.

Travel Medical Kit.

"The traveler's medical kit is an essential tool for both the novice and expert traveler. It is designed to treat travel-related illness and injury and to ensure preexisting medical conditions are managed appropriately. Travelers are at increased risk for common gastrointestinal issues during travel. Respiratory illnesses make up approximately 8% of the ailments present in returned international travelers. Approximately 12% of travelers experience a travel-related skin condition. First aid treatment for minor injuries is essential to all travel medical kits. The complexity ranges from a small,

Futility and Termination of Resuscitation Variation for Pediatric Out-of-Hospital Cardiac Arrest.

OBJECTIVES: To evaluate agency-level differences in determinations of futility (defined as withholding resuscitative efforts by emergency medical services [EMS]) and termination of resuscitation (TOR) during prehospital encounters for pediatric out-of-hospital cardiac arrest (OHCA). METHODS: We conducted a retrospective cross-sectional study of EMS activations for children (aged <18 years) with nontraumatic OHCA from the 2018-2023 National EMS Information System. Our outcomes of interest were futility and TOR. We constructed mixed-effects models including patient- and agency-level covariables

Leptospirosis Presenting as Sepsis with Pancytopenia in an Outdoor Enthusiast.

We discuss the case of a 39-y-old outdoor enthusiast who developed sepsis with pancytopenia and transaminitis following backcountry camping in the southeastern United States and adventure travel in Costa Rica. Initial evaluation revealed persistent high fevers, leukopenia, thrombocytopenia, and transaminitis, with extensive infectious testing initially negative. Convalescent serology later confirmed leptospirosis. Febrile illness in wilderness travelers and outdoor athletes presents a diagnostic challenge, particularly when initial testing is unrevealing and exposures span both local and tropi

Maggot Therapy for Wound Care in Austere Environments.

The past 25 years have seen an increase in use of maggot therapy for wound care. Maggot therapy is very effective in wound debridement; it is simple to apply and requires very little in the way of resources, costs, or skilled personnel. These characteristics make it well suited for use in austere environments. The use of medical-grade maggots makes maggot therapy nearly risk free, but medical grade maggots may not always be available, especially in the wilderness or in resource-limited communities. By understanding myiasis and fly biology, it should be possible even for the nonentomologist to

Transmission Dynamics of Rhodesian Sleeping Sickness at the Interface of Wildlife and Livestock Areas.

Many wilderness areas of East and Southern Africa are foci for Rhodesian sleeping sickness, a fatal zoonotic disease caused by trypanosomes transmitted by tsetse flies. Although transmission in these foci is traditionally driven by wildlife reservoirs, rising human and livestock populations may increase the role of livestock in transmission cycles. Deciphering transmission dynamics at wildlife and livestock interface areas is key to developing appropriate control. Data are lacking for key parameters, including host distributions, tsetse density, and mortality rates, and the relative roles of l

Genetic Structure and Antimicrobial Resistance of Escherichia coli and Cryptic Clades in Birds with Diverse Human Associations.

The manner and extent to which birds associate with humans may influence the genetic attributes and antimicrobial resistance of their commensal Escherichia communities through strain transmission and altered selection pressures. In this study, we determined whether the distribution of the different Escherichia coli phylogenetic groups and cryptic clades, the occurrence of 49 virulence associated genes, and/or the prevalence of resistance to 12 antimicrobials differed between four groups of birds from Australia with contrasting types of human association. We found that birds sampled in suburban

Analysis of emergency medical services activations in Shenandoah National Park from 2003 to 2007.

BACKGROUND: Wilderness activities continue to be popular in the United States, but may lead to both direct injuries and exacerbations or complications from chronic diseases. Appropriate response planning requires information on the type and location of emergency medical services (EMS) activations in large outdoor areas with many visitors. OBJECTIVES: To describe EMS calls in Shenandoah National Park in Virginia and explore the resultant implications for EMS resources and staging, medical provider training, and potential public health interventions in similar wilderness recreation areas. First insights into antimicrobial resistance among faecal Escherichia coli isolates from small wild mammals in rural areas.

Wild rodents can be carriers of antimicrobial resistant Escherichia coli. As rodents are known to be involved in the transmission of bacteria of human and animal health concern, they could likewise contribute to the dissemination of antimicrobial resistant bacteria in the environment. The aim of this study was therefore to get first insights into the antimicrobial resistance status among E. coli isolated from wild small mammals in rural areas. We tested 188 faecal isolates from eight rodent and one shrew species originating from Germany. Preselected resistant isolates were screened by minimal

The impact of epidemics on inland development in Qing Taiwan, 1684-1895.

During the early Qing dynasty, Taiwan remained largely undeveloped, with vast areas covered in dense wilderness and forests. The terms zhangli and zhangyi, which appear frequently in historical documents, most likely cover a variety of epidemic diseases such as malaria, dysentery, cholera, and scarlet fever. Following the Opium Wars, as Western powers increased pressure on Taiwan, the Qing government sought to intensify inland development. However, persistent epidemics in the mountainous regions hindered efforts to "open the mountains and pacify the aborigines." These health challenges signifi

Clinical aspects and consequences of envenoming by a captive rhinoceros viper (Bitis nasicornis) in Hungary.

A case of Rhinoceros viper (Bitis nasicornis) bite is reported. The bitten hand distended to the wrist, which was tense and painful, with only mild local livid discolouration manifested around the fang mark. Slight hypertension with moderate tachycardia and temporary coagulopathy were observed. The patient received analgesic and intravenous fluids, antibiotics and anti-tetanus therapy. Use of antiserum was not necessary. The bitten person was treated in the main centre for snakebite first aid: in the Toxicological Ward of Erzsébet Hospital of Budapest. We attach importance to the implications

West Ridge Forerunner: Thomas F. Hornbein and the Development of the Maytag Oxygen Mask for the 1963 American Mount Everest Expedition.

Those with an interest in the history of mountaineering are aware of the visionary achievement of the first ascent of the West Ridge of Everest-and first traverse of the mountain-by Tom Hornbein and Willi Unsoeld during the 1963 American Mount Everest expedition led by Norman Dyrunfurth. In many respects, this climb defined Hornbein (1930-2023) in the eyes of the international mountaineering community. One aspect of his career in high altitude mountaineering that also reflected his professional interests has received scant attention. This largely overlooked contribution was that of the design

Successful Defibrillation at a Core Temperature of 18.2 Degrees Celsius.

Both the temperature at which defibrillation can be effectively used and how often it should be repeated in severe accidental hypothermia have not been definitely established. Current recommendations are based mainly on expert opinion and suggest withholding defibrillation after 3 shocks when the core temperature is below 30°C (86°F). However, growing evidence supports the effectiveness of defibrillation in patients with a core temperature below 30°C (86°F). We present a case of successful defibrillation of a 54-y-old, severely hypothermic patient with a core temperature of 18.2°C (64.8°F). Th

Far from Home: Heat-Illness Prevention and Treatment in Austere Environments.

Austere environments present unique challenges concerning the prevention and treatment of exertional heat-illness patients that may greatly increase the risks of morbidity and mortality. For athletes, occupational groups, and others who may work, train, or compete in austere environments, proper preparation and planning may be lifesaving. The roles of acclimatization and hydration are often emphasized in the literature, but other important risk factors may be overlooked. Work capacity, especially aerobic work capacity, will always be reduced in hot environments, and individuals should understa

Regional Anesthesia in the Austere Environment: Lessons Learned from Current Out-of-Hospital Practice.

INTRODUCTION: Pain management for trauma in the extreme environment is vital for both casualty comfort and aiding safe extrication. However, adequate pain management in a resource-limited environment can be challenging and is often limited. We conducted a scoping review of the use of regional anesthesia in the prehospital environment, evaluating which regional anesthetic procedure was performed for various indications, their efficacy, and the type of healthcare provider delivering the anesthetic. METHODS: A PRISMA-guided systematic literature review was conducted of Medline, Embase, and Cochr

On botulinum neurotoxin variability.

The rapidly growing number of botulinum neurotoxin sequences poses the problem of the possible evolutionary significance of the variability of these superpotent neurotoxins for toxin-producing Clostridium species. To progress in the understanding of this remarkable phenomenon, we suggest that researchers should (i) abandon an anthropocentric view of these neurotoxins as human botulism-causing agents or as human therapeutics, (ii) begin to investigate in depth the role of botulinum neurotoxins in animal botulism in the wilderness, and (iii) devote large efforts to next-generation sequencing of

The Mt. Tyndall incident.

The authors describe the 53-hour rescue of a 6-foot, 1-inch tall, 250-pound hiker in the face of harsh environmental conditions in Sequoia National Park. This 43-year-old man fell 25 feet, injured his leg, and was noted to be hypothermic and hypovolemic. Weather, altitude, and the patient's size delayed and complicated his evacuation. After being carried down 1,500 vertical feet, he was hoisted into a hovering helicopter and flown to University Medical Center in Fresno, California. On arrival, the patient was determined to have a comminuted subtrochanteric right femur fracture, which was ultim

Developing Cricothyroidotomy Skills Using a Biomaterial-Covered Model.

INTRODUCTION: Cricothyroidotomy is an advanced and life-saving technique, but it is also a rare and a difficult procedure. The purpose of the present study was to produce a low-cost simulation model with realistic anatomic features to investigate its effectiveness in developing cricothyroidotomy skills. METHODS: This study was performed at a university simulation center with 57 second-year student paramedics and a cricothyroidotomy simulation model. Total scores were assessed using a checklist. This consisted of 13 steps and was scored as misapplication/omission=0, correct performance and tim
