Wrist and Distal Radioulnar Joint Arthroplasty: Maximizing Results in Difficult Conditions.

Traditional management of wrist arthritis consists of proximal row carpectomy, partial carpal fusions, or, in the event of pancarpal arthritis, total wrist fusion. Although proximal row carpectomy and partial wrist fusions preserve some motion at the wrist while relieving pain symptoms, the quality of results obtained from these procedures is not predictable or optimal in many instances. Management of hip, knee, ankle, and shoulder joints has evolved from arthrodesis to arthroplasty. The wrist joint is following the same pattern of evolution with the advent of reliable designs.

Dorsal wrist joint pain in tetraplegic patients during and after rehabilitation.

In a study of 42 tetraplegic patients, physiological, neurological, electrophysiological and radiological examinations were made in 11 patients with complete tetraplegia who had wrist pain after rehabilitation. Pain relief produced by a selective, posterior interosseous nerve lidocaine block indicated distal posterior interosseous nerve syndrome. This syndrome can sometimes be treated conservatively, but surgical excision was required after nerve scarification. Repetitive dorsiflexion, as in wheelchair handling, transfer and tenodesis-like movement, compresses the distal posterior interosseous

Quantifying forearm and wrist joint power during unconstrained movements in healthy individuals.

BACKGROUND: Wrist movement-related injuries account for a large number of repetitive motion injuries. Remarkably little, if any, empirical data exist to quantify the impact of neuromuscular disorders affecting the wrist or to validate the effectiveness of rehabilitation training programs on wrist functions. The aim of this project was to develop a biomechanical model for quantifying wrist and forearm kinetics during unconstrained movements, to assess its reliability and to determine its sensitivity. METHODS: Twenty healthy subjects with no history of upper arm and wrist pain volunteered for t

Distal radio-ulnar joint instability in children and adolescents after wrist trauma.

UNLABELLED: This study retrospectively evaluated the medical records and radiographs of patients younger than aged 25 that were referred for a second opinion due to ulnar-sided wrist pain and persistent distal radio-ulnar (DRU) joint instability. We identified 85 patients with a major wrist trauma before the age of 18. Median age at trauma was 14 years. Median time between trauma and diagnosis of DRUJ instability was 3 years. Sixty-seven patients (79%) had sustained a fracture at the initial trauma. The two most common skeletal injuries related to the DRUJ instability were Salter-Harris type I

Joint-angle-dependent neuromuscular dysfunctions at the wrist in persons after stroke.

OBJECTIVE: To evaluate the joint-angle-dependent neuromuscular functions at the affected wrist in hemiplegic subjects after stroke while doing isometric maximal voluntary wrist flexion and extension across different wrist angles. DESIGN: We investigated torques during isometric maximal voluntary wrist flexions and extensions at 8 different wrist angles, ranging from -45 degrees to 60 degrees. We used the associated electromyographic activities of 2 agonist and antagonist muscle pairs related to wrist and elbow joints for the analysis of muscular coactivations. We compared the data obtained fr

Calcium pyrophosphate dihydrate crystal deposition disease of the wrist: trapezioscaphoid joint abnormality.

OBJECTIVE: To determine whether trapezioscaphoid (TS) joint alterations are associated with calcium pyrophosphate dihydrate (CPPD) crystal deposition disease and, if so, to determine the nature of these alterations. METHODS: Radiographs of 160 wrists with evidence of chondrocalcinosis were evaluated with regard to TS joint abnormalities, and findings were compared with a similar number of radiographs in an age and sex matched control population in whom no evidence of chondrocalcinosis or other calcification in the wrist was seen. Two radiologists in consensus recorded radiographic findings in

Functional Relationships Between Wrist Joint Morphology and Ulnar Deviation in Strepsirrhine Primates.

OBJECTIVES: Ulnar deviation is a fundamental hand movement reflecting different positional behaviors that characterize primates and other arboreal mammals. Few experimental data exist on the relationship between wrist joint morphology and ulnar deviation of the hand in living primates. This study tests functional relationships between carpal joint anatomy and the degree of ulnar deviation for eight strepsirrhine species representing major locomotor groups. METHODS: Passive ranges of ulnar deviation were measured from radiographs of the hand of 25 anesthetized subjects using a motion analysis

A wrist-inspired suspended tubercle-type tensegrity joint with variable stiffness capacity.

In complex and unpredictable environments or in situations of human-robot interaction, a soft and flexible robot performs more safely and is more impact resistant compared to a traditional rigid robot. To enable robots to have bionic features (flexibility, compliance and variable stiffness) similar to human joints, structures involving suspended tubercle tensegrity are researched. The suspended tubercle gives the joint compliance and flexibility by isolating two moving parts. The variable stiffness capacity is achieved by changing the internal stress of tensegrity through the simultaneous cont

Estimation of Finger Joint Angles Based on Electromechanical Sensing of Wrist Shape.

An approach to finger motion capture that places fewer restrictions on the usage environment and actions of the user is an important research topic in biomechanics and human-computer interaction. We proposed a system that electrically detects finger motion from the associated deformation of the wrist and estimates the finger joint angles using multiple regression models. A wrist-mounted sensing device with 16 electrodes detects deformation of the wrist from changes in electrical contact resistance at the skin. In this study, we experimentally investigated the accuracy of finger joint angle est

[Initial results of treatment with the new AO wrist joint arthrodesis plate].

Although diagnosis and therapeutic options for post-traumatic, idiopathic, and degenerative wrist arthropathies have improved during recent years, total wrist fusion is still frequently regarded as a popular option of treatment. Fourty-one patients underwent wrist arthrodesis between May 1994 and April 1996 using the recently introduced new AO-plate after earlier wrist trauma or Kienböck's disease. Thirty-five patients were examined. The average follow-up time was 15 months. Hand function and active digital range of motion (ADROM) was assessed clinically, grip strength was measured using a Jam

Effects of intra-articular distal radius depression on wrist joint contact characteristics.

Twelve cadaver extremities were used to study the effect of scaphoid and lunate facet depressions on the contact characteristics of the radiocarpal joint. Pressure-sensitive film was inserted into radiocarpal joints with varying degrees of depression, and the specimens were loaded statically in neutral position, radial deviation, and ulnar deviation. The film was removed and analyzed for contact area and pressure. The only statistically significant effect of a lunate fossa depression was an increase in scaphoid fossa pressure with a 3-mm step-off and the hand in neutral position. Scaphoid foss

Surgical management of acute combined injuries of the ipsilateral wrist and elbow joints.

BACKGROUND: Combined injuries of ipsilateral wrist and elbow joints are rare in clinical practice, characterized by multiple joint dislocations or/and fractures and varying manifestations. As there are still no clinical guidelines and no consensus on the standard treatment, this study aimed to explore the surgical intervention and complications of this kind of combined injuries. METHODS: This retrospective study was conducted in a single center. A total of 13 patients with acute combined injuries of the ipsilateral wrist and elbow joints receiving surgical treatment from August 2013 to May 20

Synovial hemangiomas of wrist and ankle joint: A report of two rare cases.

Synovial hemangioma is a rare, benign, vascular tumor of synovium leading to joint pain and swelling. The most common site is the knee joint, but rare cases involving other sites have also been reported. We report two rare cases of synovial hemangioma, one involving the ankle joint and other involving the wrist joint. Histopathology is the gold standard for diagnosis of these cases. Early treatment is warranted to prevent the risk of permanent joint damage.

In vivo changes in contact regions of the radiocarpal joint during wrist hyperextension.

PURPOSE: Distal radius and scaphoid fractures commonly occur after a fall with the hand outstretched and wrist hyperextended. We investigated contact characteristics of the radiocarpal joint in neutral position, hyperextension, and hyperextension combined with radial deviation in vivo. METHODS: Eight volunteers without a known history of wrist injury were enrolled. We obtained computed tomography scans with 3-dimensional reconstructions of the subjects' right wrists in neutral, hyperextension, and hyperextension with 10° of radial deviation. The contact regions of the radiocarpal joint were m

Synovitis of the wrist joint caused by an intraarticular perforation of an osteoid osteoma of the scaphoid.

Uncommon location and atypical presentation of the osteoid osteomas of the scaphoid can pose a diagnostic challenge. Because of its intraarticular location, scaphoid osteoid osteoma can present with synovitis which is the more commonly reported presentation for other intraarticular locations like in hip and elbow and only rarely reported at the wrist. We report a case of perforation of the osteoid osteoma into the wrist joint, resulting in exuberant synovitis. The clinical significance of this report is to reinforce that synovitis can be a presentation of osteoid osteoma and it should be consi

The palmar radiocarpal ligaments: a study of adult and fetal human wrist joints.

The palmar radiocarpal ligaments were studied with use of fifty-four dissected adult cadaver wrists and serial sections on twenty-three wrists from fetuses ranging in size from 23 to 230 millimeters crown-rump length. Three palmar radiocarpal ligaments were clearly identified: the radioscaphocapitate, long radiolunate, and short radiolunate ligaments. The radioscaphocapitate ligament originates from the radial styloid process and inserts into the radial aspect of the waist of the scaphoid, hemicircumferentially around the distal pole of the scaphoid and interdigitates with fibers from the palm

[Cinematography, a new diagnostic procedure in evaluation of the injured painful wrist joint].

By the X-ray Cineradiografie we are able to examine and to judge the dynamic of the wrist bones by 50 pictures/sec. in comparison to one another and also depending on their ligaments. We did an investigation of 170 patients with painful wrist. With the method we were able to make up a clear diagnosis and to propose the therapy. I.e.: If consecutive shortening of the radius after distal radius fracture resulting ingruency of the wrist joint is relevant, or a scaphoid pseudarthrosis is fixed elastically, or a scaphoic dissociation is effective. The variations were shown in comparison to normal c
