Polyneuropathy in Cerebrotendinous Xanthomatosis: Diagnostic Challenges and Potential for Therapeutic Intervention.

Cerebrotendinous xanthomatosis (CTX) is a rare metabolic disorder caused by mutations in the CYP27A1 gene, leading to cholestanol accumulation in various tissues, including peripheral nerves. Polyneuropathy is an underrecognized feature with considerable variability in clinical presentation and neurophysiological findings in CTX. This review assesses the prevalence, clinical manifestations, and diagnostic methodologies of polyneuropathy in CTX, exploring its underlying mechanisms and potential treatment outcomes. A literature review was conducted using PubMed, Embase, and the Virtual Health Li

Optic Neuropathy with Features Suggestive of Optic Neuritis in Cerebrotendinous Xanthomatosis.

We describe our encounter with a 39-year-old man who exhibited acute painless visual loss and progressive gait disturbance. He had tendinous xanthoma and several neuroophthalmological findings indicative of optic neuropathy in the right eye, including afferent pupillary defect, cecocentral scotoma, and optic disc swelling. Neurological examination showed cerebellar ataxia and pyramidal weakness. Brain magnetic resonance imaging revealed bilateral swelling in the optic nerves with gadolinium-enhancement suggesting optic neuritis, an enlarged fourth ventricle, atrophy of the cerebellum, and hype

Clinical imaging and neuropathological correlations in an unusual case of cerebrotendinous xanthomatosis.

Cerebrotendinous xanthomatosis (CTX) is a rare autosomal recessive lipid storage disorder due to a deficiency of the mitochondrial enzyme sterol 27-hydroxylase (CYP 27) with reduced or no chenodeoxycholic synthesis. This deficiency leads to an accumulation of cholestanol in different sites such as the eye lens, central nervous system or tendons. We report a 64-year-old female patient with a progressive gait disorder associated with cognitive decline since the age of 59. The patient had no mental retardation, cataract or chronic diarrhea. Her family reported increasing behavioral modifications

Segmental xanthomatosis of the small intestine. A case report and review of the literature.

Intestinal xanthomatosis is a rare, nonneoplastic lesion that may involve the small bowel in a localized or generalized way. It most probably represents a nonspecific response to a previous injury. Most cases are not suspected clinically. They are diagnosed by endoscopy/biopsy, surgical specimen, or autopsy. It may be a cause of clinically significant obstruction and should be included in the differential diagnosis with other causes of bowel obstruction, particularly in patients with history of radiation therapy or chemotherapy. We report a case of segmental xanthomatosis involving 50 cm of th

[Perifollicular xanthomatosis as a key histological finding in Fox-Fordyce disease].

Fox-Fordyce disease is a rare skin condition characterized by the presence of multiple pruritic follicular papules in areas rich in apocrine glands, such as the axillae, mammary areolae, or genital regions. There is a high degree of variability in the histological findings seen in Fox-Fordyce disease. In addition to those described as typical of this entity, such as dilation of the infundibulum and hyperkeratosis and spongiosis of the infundibular epithelium, many other histological changes can be observed. We report the case of a 21-year-old woman with Fox-Fordyce disease and highlight the im

Presence of diarrhea and absence of tendon xanthomas in patients with cerebrotendinous xanthomatosis.

BACKGROUND: Cerebrotendinous xanthomatosis (CTX) is an autosomal recessive disorder of bile acid synthesis. A diagnosis of CTX should be considered in patients with premature bilateral cataracts, intractable diarrhea, neurological signs and symptoms, and tendon xanthomas, especially in the Achilles tendons. The prevalence of these signs and symptoms increases with age. OBJECTIVES: To investigate signs and symptoms, age at onset, and age at diagnosis in 32 patients with biochemically and genetically confirmed CTX, and to compare this clinical spectrum with reports in the literature. METHODS:

The neuropathology of cerebrotendinous xanthomatosis revisited: a case report and review of the literature.

Cerebrotendinous xanthomatosis (CTX), a rare autosomal-recessive lipid storage disease, has been well characterized clinically and biochemically, and recently also from the molecular biological aspect. However, only a very few publications deal with its neuropathology, and views on its pathogenesis vary. Based on a recently examined case, we propose that central-peripheral distal axonopathy is the major pathogenetic mechanism of nervous system injury in CTX. The latter is characterized by white matter pathology, typically in form of long tract involvement with the more distal parts of the trac

Utilization of screening for cerebrotendinous xanthomatosis in adult patients with idiopathic ataxia.

INTRODUCTION: Cerebrotendinous xanthomatosis (CTX) is an autosomal recessive lipid storage disorder. Early diagnosis and bile acid replacement therapy can improve prognosis. While clinical presentation typically occurs at a young age, recent data suggests patients presenting as adults may predominately have neurological symptoms, including ataxia. CTX prevalence among adults with idiopathic ataxia is unknown. METHODS: In this study, we examined the prevalence of CTX among adult patients with idiopathic ataxia referred to a specialized ataxia clinic. We used serum blood 7α,12α-dihydroxy-4-chol

A Rare Case of Bilateral Xanthomatosis of Tendon of Achilles: A Case Report.

INTRODUCTION: Xanthomatosis is an atypical pseudotumor of connective tissue characterized by the accumulation of lipid-laden histiocytes, often manifesting as encapsulated yellowish masses. It is commonly associated with familial hypercholesterolemia (FH). This case report aims to present a rare instance of bilateral xanthomatosis affecting the Achilles tendons in a 42-year-old male, which underscores the importance of identifying clinical and radiological findings of this condition to facilitate early diagnosis and intervention for related musculoskeletal and metabolic disorders. Clinical, electrophysiological, and genetic characteristics of cerebrotendinous xanthomatosis in South Korea.

Cerebrotendinous xanthomatosis (CTX) is an autosomal recessive lipid storage disorder caused by 27-hydroxylase deficiency. We report the clinical characteristics of six Korean CTX patients. The median age of onset was 22.5 years, the median age at diagnosis was 42 years, and the diagnostic delay was 18.1 years. The most common clinical symptoms were tendon xanthoma and spastic paraplegia. Four of five patients exhibited latent central conduction dysfunction. All patients carried the same mutation in CYP27A1 (c.1214 G>A [p.R405Q]). CTX is a treatable neurodegenerative disorder; however, our res

Chronic diarrhoea as a dominating symptom in two children with cerebrotendinous xanthomatosis.

The objective of this study was to describe diarrhoea as a dominating symptom of cerebrotendinous xanthomatosis (CTX), a lipid storage disease, and investigate its cause. Two children with chronic diarrhoea as the dominating symptom of CTX are presented. Before and after therapy with orally administered chenodeoxycholic acid (15 mg kg-1 24 h, in three divided doses) bile alcohol excretion in urine, serum cholestanol level, serum bile acid patterns and faecal bile acids were measured. All routine gastro-intestinal investigations before therapy were normal. Diarrhoea ceased immediately after sta

Cerebrotendinous xanthomatosis in the Israeli Druze: molecular genetics and phenotypic characteristics.

Cerebrotendinous xanthomatosis (CTX) is an autosomal recessive lipid-storage disease caused by mutations in the sterol 27 hydroxylase gene (CYP27). Clinically, a multitude of neurological, skeletal, and vascular manifestations are usually present. Premature atherosclerosis has been reported in CTX and may be related to the metabolic derangement caused by the deficiency of the enzyme. A CYP27 nonsense mutation created by the deletion of cytosine376 has been identified in four Israeli Druze CTX patients residing in the same village. Molecular screening for this mutation in families of two proban
