Correlating Dysphagia Severity with Fluoroscopic Parameters in Patients with Zenker's Diverticulum.

Zenker's diverticulum (ZD) is an uncommon condition characterized by formation of a pseudodiverticulum in the hypopharynx that presents with considerable variability in swallowing symptomatology. Identifying radiographic features of ZD most associated with clinical impact could prove useful in counseling patients and predicting treatment response. This study was a retrospective case series of patients undergoing videofluoroscopic swallowing studies (VFSS) for Zenker's diverticulum at a tertiary dysphagia center. Anatomic parameters identified on VFSS of patients with ZD were correlated with su

Zenker's diverticulum: a rare clinical condition with unusual oral manifestation.

Oral mucosa exhibit clinical manifestations of a variety of systemic conditions and can serve as an indicator of overall health of an individual. Various systemic conditions like autoimmune, endocrine and haematological disorders can present with oral mucosal lesions, which can serve as an important diagnostic feature. These oral lesions can vary from oral ulceration, bleeding gums, xerostomia, chronic glossitis, to erosion and sensitivity of teeth. It is important that a clinician must be aware of the oral presentations of systemic conditions, so that timely diagnosis can be made and the nece

Clinical relevance and prognostic value of radiographic findings in Zenker's diverticulum.

The aim of this study was to evaluate the clinical relevance and prognostic value of preoperative and postoperative oesophagography in patients with Zenker's diverticulum. The medical records of 155 patients who underwent surgical treatment (with an endoscopic or transcervical approach) for Zenker's diverticulum between 1992 and 2010 in a tertiary referral centre were retrospectively evaluated. The size of the diverticula on oesophagography, recognizable muscular septum, and protection of the diverticulum were assessed relative to the surgical procedures performed. The incidence of diverticula

Does the hypopharyngeal cavernous body protect the development of Zenker's diverticulum?

OBJECTIVE: The aim of this study was to examine the morphology of the cavernous body at the pharyngoesophageal segment. METHODS: In 47 cadavers the submucosal vascular plexus of the pharyngoesophageal segment and the cricopharyngeal muscle were examined. RESULTS: A vascular plexus which was macroscopically non visible or slightly identified was observed in the majority of the specimens (28 or 59.6%). Fourteen of the cadavers (29.8%) exhibited macroscopically lightly observable blood vessels which were covering a discontinuous area of the hypopharyngeal wall. In the other five specimens (10.6

[Measures for reducing the rate of complications in endoscopic surgery of Zenker's diverticulum].

In the treatment of the pharyngeal pouch diverticulectomy and endoscopic diverticulotomy are generally accepted. The latter can lead to severe bleeding and mediastinitis. --Therefore the authors have extended the preoperative diagnostic procedure to include a DSA of the aortic arch. The simultaneous contrast filling of the pouch enables the exact position of the blood vessels relative to the bar to be established. --With the spreadable diverticuloscope developed by the authors, the use of a CO2 laser and an operating microscope, optimum endoscopic working conditions are assured. Postoperative

Comparative effect between Zenker's peroral endoscopic myotomy and alternatives in the treatment of Zenker's diverticulum: a systematic review and meta-analysis.

BACKGROUND: Zenker's diverticulum (ZD) is a rare outpouching of the pharyngeal mucosa in the upper oesophagus, predominantly affecting elderly patients. Historically, the management for ZD has been surgery, but less invasive endoscopic techniques have also emerged. One technique that has gained traction is Zenker's peroral endoscopic myotomy (Z-POEM), but there remains no clear consensus on the optimal modality. This study aimed to compare the effectiveness and safety of Z-POEM with alternative treatments, including flexible and rigid diverticulotomy. METHODS: A literature search across MEDLI

Evolution of endoluminal treatment of Zenker's diverticulum and complete pharyngo-oesophageal obstruction.

PURPOSE OF REVIEW: Endoscopic approaches to surgery of the foregut, in particular, the upper oesophageal sphincter (UES), continue to develop, providing options for management that were previously unavailable. This review examines endoluminal surgical tunnelling approaches (third space endoscopy) to treat Zenker diverticulum and the more rare condition of complete oesophageal stenosis. RECENT FINDINGS: Endoluminal surgical approaches to the UES such as Z-POEM (per-oral endoscopic myotomy) that utilize gastroscopes and upper gastrointestinal endoscopic instrumentation, are increasingly being u

A preventable medical error: Successful management of iatrogenic Zenker's diverticulum perforation.

INTRODUCTION AND SIGNIFICANCE: Zenker's diverticulum is a rare condition characterized by a false diverticulum, as a true diverticulum involves herniation of all wall layers outward. Dysphagia, difficulty in swallowing, is the most common symptom. Diagnosis is primarily made through X-ray studies using contrast material during swallowing. Treatment options include open surgery or endoscopy. CASE PRESENTATION: We present the case of an adult male who experienced progressive dysphagia for approximately two years. A diagnosis of Zenker's diverticulum was confirmed by radiography following unsucc

Does residual wall size or technique matter in the treatment of Zenker's diverticulum?

OBJECTIVES/HYPOTHESIS: We aimed to compare three surgical techniques (open approach for diverticulectomy with cricopharyngeal myotomy [OA], endoscopic laser-assisted diverticulotomy [ELD], and endoscopic stapler-assisted diverticulotomy [ESD]) for treatment of Zenker's diverticulum with regard to validated swallowing outcomes, radiographic outcomes, complications, and revision rates. We statistically analyzed whether the size of residual postoperative party wall or the specific surgical technique correlates with swallowing outcomes. [Endoscopic treatment of Zenker's diverticulum with soft diverticuloscope: a first Tunisian experience].

INTRODUCTION: Zenker's diverticulum is an acquired hernia from the posterior pharyngeal mucosa developed in the pharyngo-esophageal junction. The gold standard for diagnosis is pharyngo-esophageal barium swallow study. Open surgery with cricopharyngeal myotomy has long been the conventional treatment. Actually, endoscopic treatment is an efficient alternative to resolve the problem with shorter surgery duration and less complication. AIM: We report 3 cases of patients with Zenker's diverticulum treated with endoscopic approach in our department between 2013 and 2014. CASE REPORT: There were

Endoscopic mucosal incision and muscle interruption (MIMI) for the treatment of Zenker's diverticulum.

BACKGROUND: In peroral endoscopic myotomy for Zenker's diverticulum (Z-POEM), the cricopharyngeus muscle is divided within a submucosal tunnel started in the hypopharynx. We aimed to evaluate the safety and preliminary outcomes in patients who underwent a modified version of the Z-POEM where the tunnel is made directly overlying the cricopharyngeus, the mucosal incision and muscular interruption (MIMI) approach, and to compare these with patients who underwent a non-tunneled flexible endoscopic approach. METHODS: All patients with ZD who were treated by flexible endoscopy at our institution b

Prognostic variables for the clinical success of flexible endoscopic septotomy of Zenker's diverticulum.

BACKGROUND AND AIMS: Flexible endoscopy septotomy for Zenker's diverticulum (ZD) is an alternative to endostapling; however, long-term data are sparse and studies are heterogeneous. The aim of this study was to assess the clinical success of flexible endoscopy diverticuloscope-assisted septotomy according to all ZD-related symptoms and to identify potential prognostic variables. METHODS: A prospective database of all patients with ZD undergoing septotomy and followed up for 24 months or longer was analyzed. Septotomy was conducted by using a diverticuloscope-assisted technique. Dysphagia, reg

Long-term experience with endoscopic staple-assisted esophagodiverticulostomy for Zenker's diverticulum.

The authors have previously described the endoscopic staple-assisted esophagodiverticulostomy (ESED) technique for treatment of Zenker's diverticulum. In the initial series of six patients, ESED was shown to be safe and effective in the short term, with a significant reduction in hospital stay and convalescence as compared with other surgical techniques. This report documents the authors' long-term experience with ESED (average follow-up, 9.3 months; range, 1.5 to 25 months) and discusses the long-term results of this therapy, technical issues, and variations they have utilized, as well as the

Massive bleeding from a Zenker's diverticulum: case report and review of the literature.

Zenker's diverticulum is an unusual site of origin for clinically significant upper gastrointestinal hemorrhage. Only three such cases have been previously documented. We report a case of massive upper gastrointestinal bleeding from a Zenker's diverticulum seen as hemoptysis and associated with chronic ingestion of one aspirin tablet every other day. Because of the propensity for ingested tablets to lodge in the diverticulum, caution is urged when prescribing ulcerogenic agents in pill form to a patient with a known Zenker's diverticulum.
