We present the case of a non-dialyzed patient with chronic kidney disease and biopsy-proven calciphylaxis who presented with painful cutaneous ulcers on both legs. The skin ulcers drastically improved within 6 months after the initiation of hemodialysis, aggressive wound care, the control of a mineral and bone disorder, and the administration of sodium thiosulfate and hyperbaric oxygen therapy. Notably, the patient's serum levels of C-reactive protein and calciprotein particles decreased and her serum albumin and fetuin-A levels increased in parallel with the alleviation of her calciphylaxis.

In order to investigate whether the immune system is involved in the degradation of hydroxylapatite as implant material the adsorption behavior of several serum proteins to hydroxylapatite was studied and compared with the adsorption to other calcium phosphate compounds (Ca3(PO4)2, CaHPO4), calcium carbonate, aluminium oxide, and zinc oxide. Arterial stiffness is a prominent feature of vascular ageing and strongly predicts cardiovascular and total mortality. The β2-microglobulin, (β2M) a newly identified biomarker of peripheral arterial disease (PAD), is related to renal insufficiency, inflammatory and neoplastic diseases, but may also play a role in vascular dysfunction. However, the relationship between arterial stiffness and β2M has not been previously studied in patients with atherosclerosis. In the present study we examined a possible association between β2M and arterial stiffness in patients with PAD and in healthy subjects.

BACKGROUND: Risk stratification during the acute kidney disease period remains suboptimal with existing staging systems.
METHODS: We used prospectively collected clinical data and samples from 130 patients with acute kidney disease to evaluate the association between a novel biomarker, urinary post-translationally modified fetuin-A (uPTM-FetA), and clinical outcomes.
