A retrospective analysis of 509 consecutive interscalene catheter insertions for ambulatory surgery.

Effective pain therapy after shoulder surgery is the main prerequisite for safe management in an ambulatory setting. We evaluated adverse events and hospital re-admission using a database of 509 interscalene catheters inserted during ambulatory shoulder surgery. Adverse events were recorded for 34 (6.7%) patients (9 (1.8%) catheter dislocations diagnosed in the recovery room, 9 (1.8%) catheter dislocations at home with pain, 2 (0.4%) pain without catheter dislocation, 1 (0.2%) 'secondary' pneumothorax without intervention and 13 (2.6%) other). Twelve (2.4%) patients were re-admitted to hospita

Adjunctive Intravenous Dexmedetomidine's Effect on Surgical Visualization and Analgesia in Arthroscopic Bankart Repair: A Retrospective Cohort Study.

Background/Objectives: Optimizing surgical visualization via controlled hypotension and managing postoperative pain remain challenging in arthroscopic Bankart repair. While interscalene brachial plexus block (ISBPB) is effective, pharmacological adjuncts may further improve hemodynamics and prolong analgesia. The aim of this study was to assess the impact of intraoperative intravenous dexmedetomidine on visual clarity, hemodynamics, and postoperative pain during arthroscopic Bankart repair. Methods: This retrospective, single-center cohort study evaluated 63 patients undergoing isolated anteri

Comparison of the Analgesic Properties of Sevoflurane and Desflurane Using Surgical Pleth Index at Equi-Minimum Alveolar Concentration.

Background: Traditionally, minimum alveolar concentration (MAC) has been used as the standard measure to compare the potencies of volatile anesthetics. However, it reflects the spinal mechanism of immobility rather than the subcortical mechanism of analgesia. Recently, the surgical pleth index (SPI) derived from photoplethysmographic waveform was shown to reflect the intraoperative analgesic component. This study was designed to compare the SPI values produced by equi-MAC of two commonly used volatile anesthetics, sevoflurane and desflurane. Methods: Seventy-two patients undergoing arthroscopi

The ultrasound-guided supraclavicular-interscalene (UGSCIS) block: a case report.

Providing complete anesthesia to the entire upper extremity remains challenging. We present the use of a novel, updated, regional anesthetic technique-an ultrasound-guided supraclavicular-interscalene block (UGSCIS)-to provide anesthesia and analgesia to a patient with end-stage liver disease who required fixation of a pathologic fracture of the humerus.

Delayed onset and long-lasting hemidiaphragmatic paralysis and cranial nerve deficit after interscalene nerve block for rotator cuff repair in beach chair position.

Hemidiaphragmatic paralysis is the most common adverse effect associated with interscalene block. In most cases, it resolves with the resolution of nerve blockade with only an estimated incidence of 0.048% persisting for longer duration. Occasionally, interscalene block is also associated with recurrent laryngeal nerve block and seldom with cranial nerve paresis. We present a case of delayed onset and prolonged hemidiaphragmatic paralysis that was associated with 3 cranial nerve deficits after interscalene nerve block for shoulder surgery performed under general anesthesia in the beach chair p

Editorial Commentary: The Wake of the Dragon: Will the Orthopaedic Community Adopt the Shoulder Arthroscopic Latarjet Procedure as We Adopted the Arthroscopic Rotator Cuff Repair?

The Latarjet procedure is a complex and difficult operation when performed both with an open approach and arthroscopically. The difficulties come from the fact that it is a combined intra- and extra-articular procedure, and that working close to the brachial plexus may be frightening for surgeons. Because of the high complication and reoperation rates reported in the literature, this procedure is, at the moment, rejected by a large part of the orthopaedic community, specifically in North America. The Chinese experience shows, after the European one, that arthroscopic Latarjet is an efficient a

Combined use of dexmedetomidine and propofol in monitored anesthesia care: a randomized controlled study.

BACKGROUD: Although propofol and dexmedetomidine have been widely used for monitored anesthesia care, their adverse effects necessitate the search for better methods. Therefore, we performed this randomized controlled trial to evaluate the combined use of propofol and dexmedetomidine. METHODS: Eighty-seven adult patients undergoing hand surgery under brachial plexus block were randomly allocated to receive 1.6 μg/ml of the target effect site concentration of propofol (P group) and infusion of 0.4 μg/kg/h dexmedetomidine following a loading dose of 1.0 μg/kg for 10 min (D group). Regrown first rib in patients with recurrent thoracic outlet syndrome.

BACKGROUND: Recurrent symptoms of thoracic outlet syndrome (TOS) after first rib resection have varying etiologies. Regrowth of a first rib is a rare event. Recurrent symptoms in the presence of a regrown rib strongly suggest a causal relationship. We report our experience with recurrent symptoms of TOS and regrown first ribs. METHODS: We identified patients with recurrent TOS symptoms and regrown first ribs presented between 1995 and 2012. Details regarding their presentation, evaluation, and treatment were gathered. RESULTS: Eight patients (6 women and 2 men) presenting with recurrent TOS

Optimizing Pain Relief in Refractory Thoracic Outlet Syndrome: The Role of Ultrasound-Guided Injections.

BACKGROUND: Thoracic outlet syndrome (TOS) encompasses a range of symptoms originating from the compression of neurovascular structures, often leading to significant morbidity. Neurogenic thoracic outlet syndrome (N-TOS) frequently manifests as brachial plexus neuropathy, with a subset of patients experiencing refractory pain that does not respond to conservative treatments. OBJECTIVES: This review aims to consolidate current evidence to evaluate the efficacy of available ultrasound (US)-guided injection techniques, including muscle injections, hydrodissection, regenerative therapies, and ner

A safe and efficient analgesic approach for percutaneous transluminal angioplasty: Fentanyl plus targeted local anesthesia.

BACKGROUND: Stenosis is a common complication of arteriovenous access (AV), for which percutaneous transluminal angioplasty (PTA) is a well-established intervention. Nevertheless, balloon dilation commonly induces pain. Common analgesic approaches during PTA encompass moderate intravenous sedation and analgesia, brachial plexus block, and local infiltrative anesthesia, each with its own limitations. A need currently exists for a safe, simple, and effective analgesic approach during PTA. METHODS: This retrospective observational study was conducted between January 2020 and July 2022 at a singl

Opioid-Free Analgesia Provides Pain Control Following Thumb Carpometacarpal Joint Arthroplasty.

BACKGROUND: We hypothesized that an opioid-free (OF), multimodal pain management pathway for thumb carpometacarpal (CMC) joint arthroplasty would not have inferior pain control compared with that of a standard opioid-containing (OC) pathway. METHODS: This was a single-center, randomized controlled clinical trial of patients undergoing primary thumb CMC joint arthroplasty. Patients were randomly allocated to either a completely OF analgesic pathway or a standard OC analgesic pathway. Patients in both cohorts received a preoperative brachial plexus block utilizing 30 mL of 0.5% ropivacaine that

Regional Anesthesia with Spontaneous Breathing for Trans-Axillary Surgery in Thoracic Outlet Syndrome: A Retrospective Comparative Study.

Background: Thoracic outlet syndrome (TOS) is an uncommon condition defined by the compression of neurovascular structures within the thoracic outlet. When conservative management strategies fail to alleviate symptoms, surgical decompression becomes necessary. The purpose of this study is to evaluate and compare the efficacy and safety of regional anesthesia (RA) using spontaneous breathing in contrast to general anesthesia (GA) for patients undergoing surgical intervention for TOS. Methods: We conducted a retrospective comparative study involving 68 patients who underwent trans-axillary first

Local Infiltration Analgesia Versus Interscalene Block for Pain Management Following Shoulder Arthroplasty: A Prospective Randomized Clinical Trial.

BACKGROUND: While providing effective analgesia following shoulder arthroplasty, an interscalene block has known complications. Local infiltration analgesia (LIA) using ropivacaine has been successfully employed in other joint arthroplasties, but its efficacy in shoulder arthroplasty has not been studied extensively. The purpose of this study was to compare pain and opioid consumption between LIA and an interscalene block following shoulder arthroplasty. METHODS: Patients undergoing primary shoulder arthroplasty were prospectively randomized into 2 groups: the block group received an intersca

Paravertebral block for management of acute postoperative pain and intercostobrachial neuralgia in major breast surgery.

BACKGROUND: Several locoregional techniques have been described for the management of acute and chronic pain after breast surgery. The optimal technique should be easy to perform, reproducible, with little discomfort to the patient, little complications, allowing good control of acute pain and a decreased incidence of chronic pain, namely intercostobrachial neuralgia for being the most frequent entity. OBJECTIVES: The aim of this study was to evaluate the paravertebral block with preoperative single needle prick for major breast surgery and assess initially the control of postoperative nausea
