The Alzheimer's Disease Brain, Its Microvasculature, and NADPH Oxidase.

The deterioration of the brain's microvasculature, particularly in the hippocampus, appears to be a very early event in the development of Alzheimer's disease (AD), preceding even the deposition of amyloid-β. A damaged microvasculature reduces the supply of oxygen and glucose to this region and limits the production of energy, ATP. The damage may be a function of the rise with age in the expression and activity of NADPH oxidase (NOX) in these microvessels. This rise renders these vessels vulnerable to the effects of oxidative stress and inflammation. The rise in NOX activity with age is even m

A bibliometric analysis and visualization of normal pressure hydrocephalus.

BACKGROUND: Normal pressure hydrocephalus (NPH) has drawn an increasing amount of attention over the last 20 years. At present, there is a shortage of intuitive analysis on the trends in development, key contributors, and research hotspots topics in the NPH field. This study aims to analyze the evolution of NPH research, evaluate publications both qualitatively and quantitatively, and summarize the current research hotspots. METHODS: A bibliometric analysis was conducted on data retrieved from the Web of Science Core Collection (WoSCC) database between 2003 and 2023. Quantitative assessments

Direct nose to the brain nanomedicine delivery presents a formidable challenge.

This advanced review describes the anatomical and physiological barriers and mechanisms impacting nanomedicine translocation from the nasal cavity directly to the brain. There are significant physiological and anatomical differences in the nasal cavity, olfactory area, and airflow reaching the olfactory epithelium between humans and experimentally studied species that should be considered when extrapolating experimental results to humans. Mucus, transporters, and tight junction proteins present barriers to material translocation across the olfactory epithelium. Uptake of nanoparticles through

Modeling Posthemorrhagic Hydrocephalus of Prematurity in Rats.

Posthemorrhagic hydrocephalus of prematurity (PHHP) is a serious sequela of severe intraventricular hemorrhage (IVH) in very preterm infants less than 32 weeks gestational age (GA). PHHP is defined by the accumulation of cerebrospinal fluid (CSF) associated with clinical symptoms of elevated intracranial pressure (ICP). Infants with PHHP suffer lifelong shunt dependence, with half requiring repeat surgery in the first year of life and many requiring multiple additional surgeries throughout the lifespan. Prenatal chorioamnionitis predisposes preterm infants to severe IVH and the need for surgic

Neuroinflammation as a Final Common Pathway in Systemic Disease - How Systemic Disease Becomes Brain Disease.

PURPOSE OF REVIEW: This chapter reviews evidence that neuroinflammation constitutes a final common pathway linking systemic disease of diverse origin to a shared spectrum of CNS injury, and synthesizes its mechanistic, biomarker, and therapeutic implications. RECENT FINDINGS: Autoimmune, infectious, metabolic, malignant, and age-related diseases converge on five shared CNS mechanisms-blood-brain barrier disruption, microglial priming, complement-mediated synaptic injury, microvascular and thromboinflammatory injury, and autoantibody-mediated neuronal targeting-that interact as a self-perpetua

Vascular Microforamina in Adult Human Braincase: A Survey on Dry Skulls.

OBJECTIVES: Microforamina are small vascular passages on the endocranial surface of the braincase, connecting the diploe of the vault bones with the endocranial space. Recently, they have been hypothesized to be involved in brain immune response and inflammation. At present, they have been described through biomedical imaging, but information on their general anatomy is still missing. MATERIALS AND METHODS: A sample of 94 dry skulls from a mid-20th-century Northern-Spanish collection has been examined to supply a morphological description of the dimensions and distribution of these channels i

Brain Injury: Response to Injury Wound-Healing Mechanisms and Enlarged Perivascular Spaces in Obesity, Metabolic Syndrome, and Type 2 Diabetes Mellitus.

Embryonic genetic mechanisms are present in the brain and ready to be placed into action upon cellular injury, termed the response to injury wound-healing (RTIWH) mechanism. When injured, regional brain endothelial cells initially undergo activation and dysfunction with initiation of hemostasis, inflammation (peripheral leukocytes, innate microglia, and perivascular macrophage cells), proliferation (astrogliosis), remodeling, repair, and resolution phases if the injurious stimuli are removed. In conditions wherein the injurious stimuli are chronic, as occurs in obesity, metabolic syndrome, and

Regional Associations Between Ocular Vascular Parameters and Enlarged Perivascular Spaces in the Basal Ganglia and Centrum Semiovale.

BACKGROUND: Enlarged perivascular spaces (EPVS) represent a hallmark imaging feature of cerebral small vessel disease. The ocular vasculature is an anatomical extension of cerebral vessels, constituting part of the microvascular system; however, the relationship between ocular vascular characteristics and EPVS remains poorly understood. In this study, we examined the association between ocular vascular parameters and the regional distribution of EPVS in patients with minor stroke. METHODS: Patients diagnosed with minor stroke between 2021 and 2024 were prospectively enrolled. Ocular vascular

Retinal Müller glia alterations and their impact on ocular glymphatic clearance in an Alzheimer's disease mouse model.

BackgroundRetinal amyloid-β (Aβ) accumulation has been detected in Alzheimer's disease (AD) and correlates with brain Aβ deposition, suggesting that the retina may reflect central disease processes. Impaired Aquaporin-4 (AQP4)-mediated glymphatic clearance contributes to Aβ accumulation in AD brains, but whether similar mechanisms affect the retina remains unclear.ObjectiveThis study investigated glymphatic transport and Müller glia cell (MGC) remodeling in 3-month-old female 5xFAD mice.MethodsFluorescent immunostaining of 5xFAD and wild-type (WT) retinas (n = 5 for each) of AQP4, glial fibril

Relationship between cerebral small vessel disease and proteinopathies in the medial temporal lobe.

The medial temporal lobe (MTL) is strategically important for cognition and the pathogenesis of Alzheimer's Disease (AD). Cerebral small vessel disease (CSVD) independently contributes to cognitive impairment and is believed to play a role in AD. CSVD and proteinopathies related to AD often coexist in the MTL but neither the severity of CSVD, nor the associations between these pathologies have been quantitatively addressed in this brain region. We hypothesized that the severity of CSVD in the MTL is associated with the local burden of proteinopathies implicated in neurodegeneration (tau-tangle

Comparison of Enlarged Perivascular Spaces in Early-Onset and Late-Onset Alzheimer Disease-related Cognitive Impairment: A Single Clinic-based Study in South Korea.

We examined whether there were differences in the presence of centrum semiovale-enlarged perivascular spaces (CSO-ePVS) and basal ganglia-ePVS (BG-ePVS) among patients with Alzheimer disease-related cognitive impairment (ADCI) based on their age of onset. Out of a total of 239 patients with cognitive impairment, 155 with positive amyloid-PET results were included. Among these, 43 had early-onset ADCI (EOADCI) and 112 had late-onset ADCI (LOADCI). Patients with LOADCI exhibited a higher prevalence of hypertension, lacunes, white matter hyperintensities, and BG-ePVS than those with EOADCI. BG-eP

Enlarged perivascular spaces and their association with motor, cognition, MRI markers and cerebrovascular risk factors in male fragile X premutation carriers.

FMR1 premutation carriers (55-200 CGG repeats) are at risk of developing fragile X-associated tremor/ataxia syndrome (FXTAS), a neurodegenerative disorder associated with motor and cognitive impairment. Bilateral hyperintensities of the middle cerebellar peduncles (MCP sign) are the major radiological hallmarks of FXTAS. In the general population, enlarged perivascular spaces (PVS) are biomarkers of small vessel disease and glymphatic dysfunction and are associated with cognitive decline. Our aim was to determine if premutation carriers show higher ratings of PVS than controls and whether enla

Lymphatic outflow of cerebrospinal fluid is reduced in glioma.

Glioblastoma is a malignant brain tumor with mean overall survival of less than 15 months. Blood vessel leakage and peritumoral edema lead to increased intracranial pressure and augment neurological deficits which profoundly decrease the quality of life of glioblastoma patients. It is unknown how the dynamics of cerebrospinal fluid (CSF) turnover are affected during this process. By monitoring the transport of CSF tracers to the systemic blood circulation after infusion into the cisterna magna, we demonstrate that the outflow of CSF is dramatically reduced in glioma-bearing mice. Using a combi

Brain-wide glymphatic enhancement and clearance in humans assessed with MRI.

To what extent does the subarachnoid cerebrospinal fluid (CSF) compartment communicate directly with the extravascular compartment of human brain tissue? Interconnection between the subarachnoid CSF compartment and brain perivascular spaces is reported in some animal studies, but with controversy, and in vivo CSF tracer studies in humans are lacking. In the present work, we examined the distribution of a CSF tracer in the human brain by MRI over a prolonged time span. For this, we included a reference cohort, representing close to healthy individuals, and a cohort of patients with dementia and

Proteostasis breakdown and aggregate diversity in aging and neurodegenerative disease.

Protein aggregation and proteostasis decline are central features of aging and neurodegenerative disease, arising from progressive impairment of protein quality-control systems and transitions into aggregation-prone states. During aging, diverse proteins, including metabolic and proteostasis-related factors, gradually accumulate in aggregated forms as proteostasis capacity declines. Although these aggregates often remain compatible with cellular function, increasing aggregate burden progressively challenges proteostasis resilience. Neurodegenerative diseases are characterized by the emergence

Pilot Study: Lymphatic Brain Decongestion: A New Surgical Technique in Alzheimer Disease.

BACKGROUND: Alzheimer disease (AD) is characterized by progressive cognitive function decline and neurodegeneration. Accumulation of amyloid-beta peptides forms plaques leading to brain inflammation and neuronal death. Glymphatic and meningeal lymphatic systems regulate brain fluid homeostasis and affect neurodegeneration by transferring amyloid-beta peptides from the brain to the cervical lymph nodes. We aimed to assess the feasibility of a novel operation in a rat model, to enhance the meningeal lymphatic system and consequently increase the clearance of amyloid-beta peptide. METHODS: Ten 1

Alterations in MRI-visible perivascular spaces precede dementia diagnosis by 18 years in autosomal dominant Alzheimer's disease.

INTRODUCTION: Perivascular space (PVS) alterations are traditionally linked to cardiovascular risk factors and aging, but may also play a direct role in Alzheimer's disease (AD). To reduce confounding from age-related comorbidities, we examined PVSs in autosomal dominant AD (ADAD). METHODS: In this cross-sectional study of 96 non-demented individuals (62 mutation carriers), we quantified PVS count fraction and mean diameter in white matter and basal ganglia using automated magnetic resonance imaging analysis. Linear mixed models assessed group differences along the disease course, adjusting f

Regional Burden of Enlarged Perivascular Spaces and Cognition and Neuropsychiatric Symptoms in Drug-Naive Patients With Parkinson Disease.

BACKGROUND AND OBJECTIVES: Although the potential role of enlarged perivascular spaces (EPVSs) in Parkinson disease (PD) is increasingly recognized, whether EPVSs located in different anatomical regions exert differential effects on clinical manifestation remains uncertain. We investigated the regional EPVS burden and its association with cognition and neuropsychiatric symptoms (NPSs) in newly diagnosed PD population. METHODS: In this retrospective, cross-sectional study, EPVS in the temporal lobe (T-EPVS), centrum semiovale (CS-EPVS), and basal ganglia (BG-EPVS) were visually rated in drug-n

Reexamining the Role of Amyloid β Clearance from the Brain: Exporting Labile Iron from the Interstitial Fluid Performs a Protective Function.

Advantageous functions have been attributed to amyloid β, which helps explain its expression despite a propensity to aggregate. Besides supporting cognitive processes, it has antimicrobial activity, e.g., amyloid β can entrap pathogens or disrupt their membranes. Since iron is an essential element for invading organisms, limiting its availability is an antimicrobial strategy. This can be achieved by various means, such as reducing circulating iron, as is the case for anemia of inflammation or anemia of chronic disease, which may occur in Alzheimer's disease. The protein lactoferrin both seques

Redefining intranasal brain delivery: from nasal entry to meaningful parenchymal and cellular exposure.

Nose-to-brain (N2B) delivery is a promising noninvasive strategy to circumvent the blood‒brain barrier (BBB). However, the clinical translation of N2B platforms remains hindered by a conceptual oversimplification that equates crude central nervous system (CNS) exposure with effective therapeutic delivery. This review reconceptualizes the N2B pathway as a stage-resolved sequential transport cascade extending from nasal entry to meaningful parenchymal and cellular access. We identify a critical imbalance in current formulation strategies, which have advanced two extreme poles, namely initial muc
