Is the density of transition zone prostate tissue truly 1 gram per milliliter?-an evaluation of HoLEP specimens.

BACKGROUND: Prostate volume is assumed to equal prostate weight (PW), based on a 1:1 conversion between the milliliter (mL) and gram (g). While the 1:1 conversion holds for pure water, the density of transitional zone prostate tissue has not been studied. The aim of this study was to evaluate prostate density (PD) and the factors that affect it. METHODS: This was an experimental in vitro study of holmium laser enucleation of the prostate (HoLEP) specimens weighed intraoperatively. Morcellated transition zone tissue was collected. The PW was measured by sterile water displacement. The volume d

Establishing an endoscopic enucleation of the prostate practice: a survey of strategies and challenges by successful early career urologists.

INTRODUCTION: Endoscopic Enucleation of the Prostate (EEP) remains a size independent technique for the treatment of Benign Prostatic Hyperplasia (BPH) with many benefits. Adoption has lagged behind other interventions due to a purported steep learning curve and high cost of equipment. The Prostate Enucleation Collaborative (PEC) is a group of early career urologists which formed in 2024 to promote EEP education and research. Here we report the results of a survey of PEC members addressing common practice patterns and member experiences early in their learning curve to help aspiring non-EEP pr

En-bloc HoLEP training and learning curve challenges: a multi-generational survey study.

BACKGROUND: Initial training of Holmium Laser Enucleation of the Prostate (HoLEP) often favored the 3- or 2-lobe techniques, as the en-bloc approach was considered most technically demanding. In recent years, the en-bloc method has become increasingly standardized. This survey aimed to evaluate the subjective perception of primary en-bloc HoLEP training within a structured mentorship network in Switzerland, to identify procedural steps perceived as particularly demanding and to determine whether trainees continue to apply the technique in independent practice. MATERIALS AND METHODS: Swiss Urol

Virtual basket mode in HoLEP: reduced early dysuria and faster continence recovery without loss of efficacy.

OBJECTIVES: Holmium laser enucleation of the prostate (HoLEP) is an established treatment for benign prostatic hyperplasia (BPH), but early postoperative dysuria and incontinence remain common concerns. The Virtual Basket (VB) mode has been proposed to reduce tissue trauma. This study evaluated whether VB use improves early postoperative recovery without compromising HoLEP efficacy. METHODS: We retrospectively analyzed 168 men who underwent HoLEP between September 2023 and September 2024. Patients were categorized into three groups according to laser settings: 100 W Standard (n = 65), 100 W V

Prostatic Artery Embolization: A Contemporary Review of Efficacy, Safety, and Expanding Clinical Indications.

PURPOSE OF REVIEW: Prostatic artery embolization (PAE) has emerged as a minimally invasive treatment for a spectrum of prostatic conditions. Supported by a growing body of randomized and observational evidence, PAE now occupies an established role in the management of lower urinary tract symptoms (LUTS) secondary to benign prostatic hyperplasia (BPH) and is increasingly applied for refractory prostatic bleeding and hematospermia. A structured review of PubMed, Google Scholar, and MEDLINE was performed with priority given to studies published from 2020 onward, including randomized controlled tr

Impact of Moses technology on HoLEP day-case success rate: a monocentric matched paired cohort study.

INTRODUCTION: Moses pulse modulation technology has been shown to improve enucleation efficiency and hemostasis during holmium laser enucleation of the prostate (HoLEP). Whether these technical advantages translate into better outcomes in a day-case setting remains unclear. We aimed to evaluate the impact of Moses 2.0 technology on day-case HoLEP success. MATERIALS AND METHODS: We conducted a single-center matched-pair cohort study comparing day-case HoLEP performed with Moses 2.0 technology to conventional HoLEP. A prospective cohort of 100 consecutive Moses patients (June 2023-April 2025) w

Prostatic morcellation after HoLEP Enucleation: efficiency and safety of Storz DrillCut vs. Jena Surgical Multicut Solo.

BACKGROUNDS: Holmium laser enucleation of the prostate (HoLEP) is a widely accepted technique for treating benign prostatic hyperplasia (BPH). Developed in the 1990s, its safety and efficiency have improved with adenoma morcellation. Multiple morcellation devices exist, with oscillatory systems offering higher efficiency and safety. Notable among these are the DrillCut Storz™ (DC) and the Multicut SOLO Jena Surgical™ (MC). Currently, evidence on the efficiency of MC is limited, and no comparative analysis exists between oscillatory morcellators. The objective of this study is to compare the mo

HoLEP using a 21 Fr internal urethrotomy sheath, a feasible alternative to MiLEP in resource-limited settings.

PURPOSE: Holmium laser enucleation of the prostate (HoLEP) is an effective, size-independent surgical treatment for benign prostatic hyperplasia; however, the use of large-caliber resectoscope sheaths may increase urethral trauma. Dedicated minimally invasive laser enucleation of the prostate (MiLEP) systems aim to reduce this risk but are not universally available. This study aimed to evaluate whether a modified HoLEP technique using a 21 Fr internal urethrotomy sheath (mLC-HoLEP) technique performed with commonly available endourological instruments provides comparable short-term functional
