BACKGROUND: Split cord malformation (SCM) and myelocystocele (MC) are rare forms of spinal dysraphism arising from aberrant embryogenesis at distinct developmental stages. Although both lesions share embryological origins, their concomitant occurrence is exceedingly uncommon.
OBSERVATIONS: The authors report the first case of a type II SCM coexisting with an MC in an infant. The patient underwent microsurgical detethering and repair of the MC. Postnatal postoperative MRI revealed a type II SCM of the thoracolumbar spine. At 17 months, the patient demonstrated neurological symptoms consistent

BACKGROUND: Thoracic ossification of the ligamentum flavum (T-OLF) is an important cause of progressive thoracic myelopathy and may be particularly debilitating in professional baseball pitchers, in whom surgical treatment must restore neural function while preserving the spinal biomechanics required for elite pitching. Although open laminectomy has traditionally been the standard treatment, extensive muscle dissection and disruption of posterior elements may compromise postoperative stability and return to play.
OBSERVATIONS: The authors applied a refined full-endoscopic interlaminar decompr

BACKGROUND: Nonmissile penetrating spinal injuries are rare and potentially debilitating. Such injuries can sometimes be complicated by the retention of a foreign body, which is usually part of the assailant's weapon, making their management even more problematic.
OBSERVATIONS: We present a unique case of stab wound to the neck with a retained ice pick, traversing the spinal canal from one intervertebral foramen to the other, yet with no ensuing neurological damage to the patient. After carefully analyzing the weapon's trajectory on computed tomography and ruling out vertebral artery injury v

Brainstem cavernous malformations are frequently surrounded by vital structures, which often makes surgical treatment a challenging task even to the most skilled surgeon. Accordingly, microsurgical excision is preferably offered to symptomatic patients and superficial lesions.1-3 We present the case of a 41-yr-old male presenting with progressive dizziness and diplopia. Neurological examination showed horizontal nystagmus, dysmetria, and unbalance. Preoperative magnetic resonance imaging (MRI) suggested a cavernous malformation in the right middle cerebellar peduncle. A telovelar approach was

BACKGROUND: Fractures of the axis are commonly seen in spinal injuries. Upper cervical fractures are usually managed conservatively. However, the complications due to long-term external immobilization cannot be ignored. The traditional open surgery has the disadvantages of too much blood loss and soft tissue injury. The aim of our paper is to introduce a minimally invasive surgical treatment for multiple axis fractures.
CASE SUMMARY: We report a 40-year-old Chinese male who had severe neck pain and difficult neck movement after falling from 3 meters. X-ray and computed tomography (CT) scan re

PURPOSE: Although there are guidelines analyzing transcranial motor evoked potentials (MEP) waveform criteria, they vary widely and are not applied universally during intraoperative neurophysiologic monitoring (IONM). The objective is to generate hypotheses to identify early and reliable MEP waveform characteristics prior to complete loss of MEP to predict impending motor spinal cord injuries during spinal surgeries. The ultimate goal is to enhance real-time feedback to prevent injury or detect reversible spinal cord damage.
METHODS: Fifteen true positive cases of persistent intraoperative ME

STUDY DESIGN: A clinical study of the surgical procedure for cervical disc herniation was conducted.
OBJECTIVES: To describe microsurgical transdural discectomy with laminoplasty, and to assess the clinical outcome of this surgical technique.
SUMMARY OF BACKGROUND DATA: A posterior approach for cervical disc herniation has been considered risky, and few reports on a transdural approach to this disorder have appeared in the past decade. However, a transdural approach with recent innovations (a microsurgical technique, intraoperative spinal cord monitoring, and laminoplasty) has not been repor

We define specific neurophysiologic characteristics for focal cortical dysplasia, a neuronal migration disorder. We reviewed data from published reports and our patients with focal cortical dysplasia. Our patients underwent preoperative scalp video-electroencephalography (EEG), magnetic resonance imaging (MRI), magnetoencephalography, and intraoperative or extraoperative electrocorticography monitoring. Scalp EEG showed trains of rhythmic epileptiform spike or sharp waves. Positive spikes correlated with early seizure onset, MRI lesion around the rolandic fissure, hemiparesis, and a less favor

To determine if the use of intraoperative microelectrode recording (MER) influences the final location of lead implant in deep brain stimulation (DBS) of the ventral intermediate nucleus (VIM), and to evaluate the incidence of associated complications. The usefulness of intraoperative MER in DBS is debated, some centers suggesting it increases complications without additional benefit. We conducted a retrospective chart review of all patients who underwent VIM DBS with MER at the University of Texas Health Science Center in Houston from June 1, 2009 to October 1, 2013. Initial (MRI determined)

BACKGROUND AND OBJECTIVES: Anterior cervical discectomy and fusion (ACDF) is a widely regarded surgical technique for the management of cervical disc herniation (CDH) and myelopathy, offering significant therapeutic efficacy. However, the potential for neurological complications persists, underscoring the need for strategies to enhance procedural safety. Multimodal intraoperative neuromonitoring (IONM) has emerged as a promising adjunct, providing real-time assessment of neural integrity to identify and address intraoperative compromise. This study seeks to rigorously assess the diagnostic acc

PURPOSE: Facial nerve injury is a potential complication in intradural middle cranial fossa surgery, particularly because of dehiscence of the geniculate ganglion, exposing the nerve to thermal injury. Identifying the geniculate ganglion during surgical approaches is crucial for preserving facial nerve function. This study aimed to evaluate the feasibility and efficacy of transdural stimulation in identifying the geniculate ganglion during intradural approaches to improve the preservation of facial nerve function.
METHODS: We conducted a retrospective analysis on patients who underwent surger

OBJECTIVE: The current study examined the efficacy of the facial corticobulbar motor evoked potentials (FCoMEPs) and blink reflex (BR) on predicting postoperative facial nerve function during cerebellopontine angle (CPA) tumor surgery.
METHODS: Data from 110 patients who underwent CPA tumor resection with intraoperative FCoMEPs and BR monitoring were retrospectively reviewed. The association between the amplitude reduction ratios of FCoMEPs and BR at the end of surgery and postoperative facial nerve function was determined. Subsequently, the optimal threshold of FCoMEPs and BR for predicting
