Development of a penem antibiotic against Mycobacteroides abscessus.

β-lactams are the most widely used antibiotic class to treat bacterial infections in humans. Mycobacteroides abscessus is an emerging pulmonary pathogen resistant to most antibiotics, including penicillins and cephalosporins. With no current FDA-approved treatment and cure rates <50%, there is a pressing need for effective therapies. Here we report T405, a new β-lactam of the penem subclass that exhibits potent activity against M. abscessus and a panel of drug-resistant strains isolated from cystic fibrosis patients. Additionally, in combination with the β-lactamase inhibitor avibactam, the ra

Analytical and clinical performance evaluation of the STANDARD™ M10 MTB/NTM assay for the simultaneous detection of Mycobacterium tuberculosis and nontuberculous mycobacteria.

The global surge in nontuberculous mycobacteria (NTM) infections, which clinically mimic Mycobacterium tuberculosis (MTB) infections, underscores the need for rapid differential diagnosis. This study evaluated the analytical and clinical performance of the STANDARD™ M10 MTB/NTM assay, a fully automated, cartridge-based, sample-to-result real-time PCR system. Analytical performance was assessed using contrived artificial sputum samples to evaluate inclusivity (3 MTB-complex, 12 NTM species), cross-reactivity (65 respiratory pathogens), and interference (33 substances). The limit of detection (L

Clinical Characteristics and Risk Factors Analysis of Nontuberculous Mycobacterial Pulmonary Disease Complicated with Bronchiectasis.

BACKGROUND: Nontuberculous mycobacteria pulmonary disease (NTM-PD) frequently coexists with bronchiectasis. This study aimed to compare clinical profiles and identify risk factors between NTM-PD patients with/without bronchiectasis and explore differences between rapid- vs slow-growing non-tuberculous mycobacteria (NTM) species. METHODS: A retrospective analysis was conducted on patients diagnosed with NTM-PD and admitted to Beijing Tsinghua Changgung Hospital between April 2021 and April 2025. Among 496 inpatients with pulmonary diseases who underwent metagenomic next-generation sequencing (

Genomic insights into the plasmidome of non-tuberculous mycobacteria.

BACKGROUND: Non-tuberculous mycobacteria (NTM) are a diverse group of environmental bacteria that are increasingly associated with human infections and difficult to treat. Plasmids, which might carry resistance and virulence factors, remain largely unexplored in NTM. METHODS: We used publicly available complete genome sequence data of 328 NTM isolates belonging to 125 species to study gene content, genomic diversity, and clusters of 196 annotated NTM plasmids. Furthermore, we analyzed 3755 draft genome assemblies from over 200 NTM species and 5415 short-read sequence datasets from six clinica

Nontuberculous mycobacteria remodel lung microbiota in cystic fibrosis-associated respiratory infections.

Nontuberculous mycobacterial (NTM) infections in people with cystic fibrosis (pwCF) can have detrimental effects on prognosis and pose significant challenges to treatment. However, there are still questions regarding the contribution and influence of NTMs on the respiratory microbiome and the mechanisms by which NTMs cause infections. Here, we investigate the impact of NTM infection on microbiome composition and lung function (percent predicted forced expiratory volume in 1 second). Primary comparisons were between culture-positive cohorts for Mycobacterium avium complex and Mycobacterium absc

Vertebral osteomyelitis caused by Mycobacteroides abscessus subsp. abscessus resulting in spinal cord injury due to vertebral body fractures.

Nontuberculous mycobacteria (NTM) rarely cause vertebral osteomyelitis; however, the clinical characteristics of vertebral osteomyelitis caused by NTM are poorly understood due to its rarity. A 74-year-old man with lung cancer was treated with prednisolone for immune checkpoint inhibitor-associated immune-related adverse events. He had been experiencing mild back pain without febrile episodes for five months, and was admitted to the hospital for worsening back pain and progressive paraplegia. Magnetic resonance imaging showed spinal cord compression at T4-5 due to fractures of the T5 and T7 ve

Microbiological diagnostic performance of metagenomic next-generation sequencing compared with conventional culture for patients with community-acquired pneumonia.

BACKGROUND: Community-acquired pneumonia (CAP) is an extraordinarily heterogeneous illness, both in the range of responsible pathogens and the host response. Metagenomic next-generation sequencing (mNGS) is a promising technology for pathogen detection. However, the clinical application of mNGS for pathogen detection remains challenging. METHODS: A total of 205 patients with CAP admitted to the intensive care unit were recruited, and broncho alveolar lavage fluids (BALFs) from 83 patients, sputum samples from 33 cases, and blood from 89 cases were collected for pathogen detection by mNGS. Species diversity of non-tuberculous mycobacteria isolated from humans, livestock and wildlife in the Serengeti ecosystem, Tanzania.

BACKGROUND: Non-tuberculous mycobacteria (NTM), which are ubiquitous micro-organisms occurring in humans, animals and the environment, sometimes receive public health and veterinary attention as opportunistic disease-causing agents. In Tanzania, there is limited information regarding the diversity of NTM species, particularly at the human-livestock-wildlife interface such as the Serengeti ecosystem, where potential for cross species infection or transmission may exist. METHODS: Mycobacterial DNA was extracted from cultured isolates obtained from sputum samples of 472 suspect TB patients and 6

[How to Avoid Reoperation for Complications Associated with Durable Mechanical Circulatory Support Therapy].

Since April 2011, durable mechanical circulatory support (DMCS) has been used for bridge to transplant (BTT-DMCS) in Japan, with its success leading to insurance coverage for destination therapy (DT-DMCS) in 2021. Despite advancements, managing DMCS-related complications remains challenging, with high readmission rates impacting patients' quality of life and healthcare facilities' workloads. Key complications include de novo aortic insufficiency (dnAI), driveline infections (DLI), and external outflow graft obstruction( EOGO). dnAI contributes significantly to chronic right heart failure, with

Molecular Epidemiology of Non-Tuberculous Mycobacteria Among Tuberculosis-Suspected Patients in Iran: Species Distribution and Drug Resistance.

In high-burden tuberculosis (TB) settings such as Iran, non-tuberculous mycobacteria (NTM) are increasingly identified among presumptive TB cases. However, their epidemiology and drug resistance patterns remain inadequately described. This study investigated the prevalence, species distribution, and antimicrobial susceptibility of NTM isolates from 3,000 clinical specimens collected from patients with presumptive TB at the Pasteur Institute of Iran between March 2022 and March 2023. Identification was performed through culture and sequencing of the 16S rDNA, rpoB, and hsp65 genes. Drug suscept

Pulmonary Disease Due to Nontuberculous Mycobacteria: Current State and New Insights.

Since pulmonary nontuberculous mycobacteria (PNTM) lung disease was last reviewed in CHEST in 2008, new information has emerged spanning multiple domains, including epidemiology, transmission and pathogenesis, clinical presentation, diagnosis, and treatment. The overall prevalence of PNTM is increasing, and in the United States, areas of highest prevalence are clustered in distinct geographic locations with common environmental and socioeconomic factors. Although the accepted paradigm for transmission continues to be inhalation from the environment, provocative reports suggest that person-to-p
