Surgical treatment of brainstem cavernomas using diffusion tensor imaging and diffusion tensor tractography.

INTRODUCTION: The aims of this study were to assess the prognosis of patients after a single haemorrhage from the cavernoma, and also in the case of rehaemorrhage, and to determine the indications for surgical treatment of brainstem cavernomas. MATERIAL AND METHODS: The study included a group of 35 patients with brainstem cavernomas, 23 women and 12 men aged 27 to 57 years (mean age 38.4). Up to 2005, MRI perfusion-weighted imaging/diffusion-weighted imaging had been carried out in 13 surgically treated patients. From 2005 onwards, the other 22 patients also underwent MRI diffusion tensor ima

Microsurgical endoportal MRI/US-navigated approach for the resection of large intraventricular tumours: a 20-consecutive patients case series.

OBJECTIVES: Deep lesions located in lateral and third ventricles can be accessed thorough interhemispheric transcallosal or transcortical trans-ventricular approaches. Traditional brain retractors are made by 'non-cerebral engineered' spatulas, which do not equally distribute pressure on surrounding structures damaging brain. In this paper, we present a series of 20 intraventricular tumours resected through a MRI/US-navigated microscopic transcortical endoportal approach. PATIENTS AND METHODS: Between January 2014 and December 2017, 20 patients underwent US-MRI neuronavigated (Esaote®, Genova

Outcome of transsphenoidal surgery for Cushing's disease: a high remission rate in ACTH-secreting macroadenomas.

OBJECTIVE: Although numerous studies have shown that transsphenoidal surgery is the best initial treatment for Cushing disease offering 59-95% of success, fewer information is available on the long-term outcome in the subgroup of patients harboring ACTH-secreting macroadenomas. The aims of this study were to analyze our 10-year experience with transsphenoidal surgery in Cushing's disease and to examine whether remission rates were different between micro- and macroadenomas. PATIENTS AND METHODS: Forty consecutive patients with proven Cushing's disease (28 microadenomas, 12 macroadenomas [diam

Complex technical methodologies and their applications in the surgery of intracranial meningiomas.

As neurosurgery moves into the twenty-first century, improved visualization/localization techniques, neuromonitoring, and advanced instrumentation will become standards of care for all intracranial procedures. This article has focused on current available technologies that can be used to facilitate operations on intracranial meningiomas. Preoperative anatomic localization with MR imaging, CT, MR angiography, and angiography are standard techniques. Preoperative functional assessments with MR imaging, magnetic source imaging, PET, and functional MR imaging are crucial to recognize and preserve

Pituitary Surgery.

Surgery, more specifically transsphenoidal approach, remains the primary treatment chosen for the majority of pituitary tumors (except for prolactinomas where medical treatment is indicated) allowing for pathologic analysis and complete or partial tumor removal. Transcranial approaches, employing craniotomies, are still needed in some patients with giant pituitary adenomas and in many of those harboring craniopharyngiomas. However, the surgical treatment of pituitary tumors is challenging because of their complex anatomical location and their heterogenous histology. Due to the proximity of cri

The variability of motor evoked potential latencies in neurosurgical motor mapping by preoperative navigated transcranial magnetic stimulation.

BACKGROUND: Recording of motor evoked potentials (MEPs) is used during navigated transcranial magnetic stimulation (nTMS) motor mapping to locate motor function in the human brain. However, factors potentially underlying MEP latency variability in neurosurgical motor mapping are vastly unknown. In the context of this study, one hundred brain tumor patients underwent preoperative nTMS-based motor mapping of the tumor hemisphere between 2010 and 2013. Fourteen predefined predictor variables were recorded, and MEP latencies of abductor pollicis brevis muscle (APB), abductor digiti minimi muscle (

Multilevel Spondylolysis Repair Using the "Smiley Face" Technique with 3-Dimensional Intraoperative Spinal Navigation.

BACKGROUND/OBJECTIVE: Multilevel spondylolysis is a rare cause of progressive lower back pain, and patients who fail conservative management are treated surgically. Direct repair methods can maintain mobility and lead to decreased morbidity compared with spinal fusion in single-level spondylolysis. In this paper, we present a patient with nonadjacent multilevel spondylolysis who underwent the "smiley face" technique of direct multilevel repair without fusion using 3-dimensional intraoperative spinal navigation. METHODS: Bilateral spondylolysis at L3 and L5 with associated spondylolisthesis in

Adenoid cystic carcinoma of the skull base mimicking temporomandibular disorder.

BACKGROUND: Temporomandibular disorder (TMD) involves problems of the temporomandibular joint and its adjacent muscular system. Because TMD prevalence is high among Western populations, complaints in these regions are generally attributed to TMD. However, in rare cases, TMD symptoms are mimicked by malignant tumors of the head. CASE REPORT: Upon first presentation, an 18-year-old female complained about typical symptoms of TMD. After an initial splint therapy and physiotherapy, painful symptoms increased significantly. Twelve weeks after initial diagnosis, further diagnostic imaging revealed

Monostotic fibrous dysplasia of the clivus: image-guided endoscopic endonasal transsphenoidal surgery.

INTRODUCTION: Fibrous dysplasia is a non-heritable, congenital disease involving the bones. Its diagnosis relies on image examination and pathology. Treatment is usually conservative, but in cases of cranial nerve impingement or extension to the condyles, surgery should be considered. In this paper, we describe a singular case of monostotic fibrous dysplasia of the clivus in a child. CASE REPORT: A 14-year-old boy presented with chronic headache and diplopia. A neurological examination revealed palsy of the right VIth cranial nerve. The radiological findings were consistent with fibrous dyspl

Differential mechanisms of continuous theta burst stimulation and low-frequency repetitive transcranial magnetic stimulation for chronic tinnitus: preliminary efficacy dissociation by local focalization vs. network dispersion.

OBJECTIVE: This study aimed to preliminarily compare clinical efficacy and neural mechanisms of continuous theta-burst stimulation (cTBS) and low-frequency repetitive transcranial magnetic stimulation (rTMS) targeting the auditory cortex in chronic subjective tinnitus. Differential effects on tinnitus loudness and distress, as well as the moderating role of disease duration, were explored. METHODS: Twenty-nine patients were randomly assigned to cTBS (n = 16) or rTMS (n = 13) groups. Neuronavigation-guided stimulation was applied to the individualized auditory cortex peak activation site. Outc

Craniometric Anatomy of the Middle Meningeal Artery Bifurcation With Relevance for Simultaneous Surgical Occlusion in Chronic Subdural Hematoma.

BACKGROUND AND OBJECTIVES: Despite effective surgical evacuation, chronic subdural hematoma (cSDH) recurrence rates remain high. Middle meningeal artery (MMA) embolization has emerged as a promising adjunct significantly reducing recurrence rates but adding procedural risk, logistical complexity, and cost. Thus, single-stage cSDH evacuation with direct surgical MMA occlusion through a single pterional/frontotemporal burr hole could offer a practical alternative. This study assesses the applicability of this approach on routine preoperative imaging and evaluates both neuronavigation-guided and

Pediatric intracranial high-grade solitary fibrous tumor/hemangiopericytoma with drop metastasis to cervicodorsal spine: a rare case report.

BACKGROUND: Intracranial solitary fibrous tumor/hemangiopericytoma (SFT/HPC) is extremely rare in children, accounting for fewer than 10% of all SFT/HPCs and significantly fewer primary central nervous system tumors. Pediatric SFT/HPCs of high-grade are particularly aggressive, having a high frequency of recurrence and late metastasis, though spinal "drop" metastases remain, nevertheless, very rare. CASE PRESENTATION: We discuss the case of an 11-year-old girl patient with a 2-year history of recurrent generalized tonic-clonic seizures, worsening headache, vomiting, and acute-onset bilateral
