Trends in Methadone Distribution for Pain Treatment, Methadone Diversion, and Overdose Deaths - United States, 2002-2014.

Use of the prescription opioid methadone for treatment of pain, as opposed to treatment of opioid use disorder (e.g., addiction), has been identified as a contributor to the U.S. opioid overdose epidemic. Although methadone accounted for only 2% of opioid prescriptions in 2009 (1), it was involved in approximately 30% of overdose deaths. Beginning with 2006 warnings from the Food and Drug Administration (FDA), efforts to reduce methadone use for pain have accelerated (2,3). Validation of the naloxone cardiac arrest decision instrument for identifying opioid-associated cardiac arrests.

BACKGROUND: Up to fifteen percent of out-of-hospital cardiac arrests (OHCAs) are precipitated by occult drug overdose - cases without history or evidence of drug use that are often attributed to a non-overdose cause. The NAloxone Cardiac ARrest Decision Instrument (NACARDI) was derived to help emergency medical service (EMS) providers rapidly identify patients at higher risk of occult opioid-associated (OA)-OHCAs during resuscitation. In this analysis we externally validate NACARDI in an independent cohort of OHCA patients. METHODS: We conducted a retrospective validation using data from EMS-

An investigation of methadone-related mortalities: A report from southeast of Iran.

BACKGROUND: Methadone is increasingly being used as a pharmacological agent in treating opioid use disorder (OUD). However, reports of illicit overuse of methadone have resulted in fatal consequences, mainly in those under methadone maintenance treatment (MMT). Considering the high prevalence of opioid addiction in Kerman, the present study was conducted to investigate methadone-related deaths in this province. METHODS: A descriptive cross-sectional study was performed on mortality cases caused by methadone use referred to the Forensic Medicine Organization of the Kerman Province in 2020 and

COVID-19, mental health, and opioid use disorder: Old and new public health crises intertwine.

The United States is facing both the coronavirus disease 2019 (COVID-19) pandemic and an ongoing epidemic of opioid overdose. Opioid use disorder is associated with other mental health problems, trauma, and social and health disparities. While the United States has acted to improve access to treatment for mental health and opioid use, research will be needed to understand the effectiveness of new policies in the context of COVID-19. Criminal Justice Barriers to Treatment of Opioid Use Disorders in the United States: The Need for Public Health Advocacy.

Expanding access to treatment of opioid use disorder (OUD) is central to addressing the US overdose mortality crisis. Numerous barriers to OUD treatment are encountered in criminal justice institutions and processes, with which people with OUD are disproportionately involved. OUD treatment access is severely limited in US corrections facilities, with few exceptions. Drug treatment courts, which in principle provide court-supervised treatment as an alternative to prison, have also unduly limited treatment options, particularly medication-assisted treatment. The voice and expertise of health pro

Increased overall and cause-specific mortality associated with disability among workers' compensation claimants with low back injuries.

BACKGROUND: Mortality tends to be higher among people who do not work than among workers, but the impact of work-related disability on mortality has not been well studied. METHODS: The vital status through 2015 was ascertained for 14 219 workers with an accepted workers' compensation claim in West Virginia for a low back injury in 1998 or 1999. Mortality among the cohort compared with the West Virginia general population was assessed using standard life table techniques. Associations of mortality and disability-related factors within the cohort were evaluated using Cox proportional hazards re

Coprescription of Opioids With Other Medications and Risk of Opioid Overdose.

Polypharmacy is common among patients taking prescription opioids long-term, and the codispensing of interacting medications may further increase opioid overdose risk. To identify nonopioid medications that may increase opioid overdose risk in this population, we conducted a case-crossover-based screening of electronic claims data from IBM MarketScan and Optum Clinformatics Data Mart spanning 2003 through 2019. Eligible patients were 18 years of age or older and had at least 180 days of continuous enrollment and 90 days of prescription opioid use immediately before an opioid overdose resulting

Overdose Risk and Client Characteristics Associated With the Injection of Buprenorphine at a Medically Supervised Injecting Center in Sydney, Australia.

Background: Buprenorphine and buprenorphine/naloxone (BNX) were developed to improve the safety profile of opioid substitution treatment (OST) and reduce diversion and injection, yet continue to be injected, despite the risk of harm. Previous studies examining injection of these substances have relied on self-reported injection and overdose. Using data from the Uniting Medically Supervised Injecting Center (MSIC) in Sydney, this study aimed to assess the overdose risk associated with the use of buprenorphine and BNX and identify factors associated with injecting. Methods: Client data routinely

Risk factors for experiencing substance use-related employment stigma among emergency department patients at high risk of opioid overdose.

BACKGROUND: People who use drugs report hesitance to seek employment because of stigma around drug use, which other forms of stigma may compound. We evaluated risk factors for substance use-related employment stigma among emergency department (ED) patients who use drugs. METHODS: This was a cross-sectional study among ED patients at high risk of opioid overdose in Rhode Island. The outcomes were three self-reported measures of substance use-related employment stigma. Multivariable log-binomial regression was used to estimate the association between participant characteristics and each outcome

Comparing actual and forecasted numbers of unique patients dispensed select medications for opioid use disorder, opioid overdose reversal, and mental health, during the COVID-19 pandemic, United States, January 2019 to May 2020.

BACKGROUND: COVID-19 community mitigation measures (e.g., stay-at-home orders) may worsen mental health and substance use-related harms such as opioid use disorder and overdose and limit access to medications for these conditions. We used nationally-representative data to assess dispensing of select substance use and mental health medications during the pandemic in the U.S. METHODS: IQVIA Total Patient Tracker data were used to calculate U.S. monthly numbers of unique patients dispensed buprenorphine, extended-release (ER) intramuscular naltrexone, naloxone, selective serotonin or serotonin-n
