BACKGROUND: Ileus is common after colorectal surgery and is associated with an increased risk of postoperative complications. Identifying features of normal bowel recovery and the appropriateness for hospital discharge is challenging. This study explored the safety of hospital discharge before the return of bowel function.
METHODS: A prospective, multicentre cohort study was undertaken across an international collaborative network. Adult patients undergoing elective colorectal resection between January and April 2018 were included. The main outcome of interest was readmission to hospital with

INTRODUCTION: Low anterior resection (LAR) is a standard surgical procedure for distal rectal carcinoma that allows creation of a colorectal anastomosis, with anal sphincter preservation and permanent colostomy avoidance. Anastomotic leakage (AL) is a potential complication of LAR present in 3% to 20% of cases leading to a significant increase in postoperative morbidity. AL represents a communication between the intra luminal and extra luminal compartments caused by a violation of the integrity of the intestinal wall. The risk factors of this procedure have been discussed and still remain a co

OBJECTIVE: To provide normative data for the Low Anterior Resection Syndrome (LARS) score.
BACKGROUND: The LARS score is a validated and frequently used tool measuring bowel dysfunction after sphincter sparing surgery for rectal cancer. The interpretation of LARS score results has previously been limited by the lack of normative data.
METHODS: An age and sex-stratified random sample of 3440 citizens from the general population was drawn from the Danish civil registration system (age range 20-89 years, 50% females). A brief questionnaire including the LARS score and health-related items were

AIM: Human papillomavirus (HPV)-related anal squamous cell dysplasia has been well-reported in high-risk (HR) patients [human immunodeficiency virus (HIV)-positive, men having sex with men (MSM) or immune-suppressed transplant recipients]. However, data are extremely limited for all other patients. This study reports treatment outcomes for HPV-related dysplasia in a population of non-HR patients.
METHOD: A retrospective study was performed to review treatment efficacy in non-HR patients diagnosed with anal dysplasia or superficially invasive squamous cell carcinoma of the anus (SISCCA) with a

INTRODUCTION: Crohn's disease (CD) is a chronic inflammatory disorder of the gastrointestinal tract distinguished by progressive bowel damage with a risk of structuring and penetrating complications. It is characterised by focal or segmental transmural inflammation that disrupts intestinal mucosal integrity and favours the development of abscesses and fistulas. Perianal fistula develops in 13%-39% of patients with CD. Their care is difficult but improves with medical and surgical treatment to preserve anal continence and avoid a maximum proctectomy. Combined treatment with seton placement and

BACKGROUND: The international consensus delineates 8 symptom complexes and 8 consequences for defining low anterior resection syndrome (LARS). This exploratory cross-sectional study aimed to evaluate the differential associations between symptom complexes and consequence severity after intersphincteric resection (ISR) for ultralow rectal cancer.
METHODS: Patients were enrolled ≥12 months after diverting ileostomy reversal after ISR. The severity of the consensus-defined LARS variables was assessed using a 5-point Likert scale. Participants completed questionnaires on bowel function and condit

BACKGROUND: Reporting of pelvic sepsis rates following robotic total mesorectal excision (R-TME) for rectal cancer is inconsistent. This IDEAL stage 2b international multicentre study analysed the prevalence of pelvic sepsis rates and associated risk factors following R-TME and generated a risk prediction model for anastomotic leak (AL).
METHODS: Patients were identified through the EUREKA (Expert DUtch, FREnch and UK robotic rectal cAncer centres) collaborative. Adult patients undergoing R-TME with primary anastomosis for biopsy-proven rectal cancer were considered for inclusion. The primary

BACKGROUND: The vertical rectus abdominis flap is considered the gold standard in perineal reconstruction after oncological abdominoperineal resection; however, it has a nonnegligible donor site morbidity. The anterolateral thigh flap offers reliable soft tissue coverage.
OBJECTIVE: The aim was to analyze long-term outcomes of composite anterolateral thigh-vastus lateralis flaps in oncological abdominoperineal resections.
DESIGN: We conducted a retrospective cohort analysis of a prospectively maintained database.
SETTINGS: This study was conducted in the Lausanne University Hospital. Annual

BACKGROUND: Left hemicolectomy and complicated sigmoid colectomy require an anastomosis between the transverse colon and rectum. Generous mobilization will typically allow the colon to reach to the rectum. However, despite full mobilization of the splenic flexure and extensive work on the mesentery, there are cases in which reach to the pelvis is still an issue. Retroileal routing of the colon is one technique for overcoming such a reach problem and achieving a tension-free anastomosis. Performing retroileal routing using laparoscopic techniques has been reported rarely, and to date, there are

BACKGROUND: Sphincter preserving surgery for the treatment of rectal cancer is very often feasible, avoiding a permanent colostomy. It is well recognized that a large proportion of patients will experience altered bowel habit following low anterior resection (LAR). Faecal incontinence is a common symptom associated with LAR syndrome. The aim of this study is to evaluate the long-term improvement in continence and quality of life (QoL) in LAR patients treated with sacral nerve modulation.
METHODS: Patients with ongoing faecal incontinence for >1 year after reversal of diverting ileostomy post

UNLABELLED: Authors retrospectively reviewed results of surgical treatment of the patients with anorectal Leśniowski-Crohn's (LC) disease, operated in the Department of General and Gastroenterological Surgery in Warsaw.
MATERIAL AND METHODS: In years 1987-2007 we treated 110 patients with LC disease. The anorectal localization of lesions was noticed in 24 patients (13 women, 11 men, average age of fall--28 year), i.e. 21.8% of all patients. The operations performed in other hospitals were taken into account. Results were compared with literature data.
RESULTS: Anorectal lesions were the firs

BACKGROUND AND OBJECTIVES: Rectovaginal fistulas (RVF) in Crohn's disease continue to be a challenging problem. Several operations are often necessary to attain definitive healing of the disease process. There are no guidelines concerning optimal therapeutic approaches. Endoanal mobilization techniques such as the advancement flap technique were considered the therapy of choice for many years, but are now regarded ever more critically. We have implemented several less aggressive closure techniques that take account of the anatomy and morphology of the anorectum. The long-term results are prese

PURPOSE: Controlled trials have demonstrated the efficacy of sildenafil for "mixed etiology" erectile dysfunction, but this may not be the case if there is underlying pelvic parasympathetic nerve damage. We aimed to determine the efficacy of sildenafil after rectal excision for rectal cancer and inflammatory bowel disease.
METHODS: Patients with erectile dysfunction after rectal excision were randomly assigned in a double-blind manner to sildenafil or placebo groups. After unblinding, placebo patients crossed over to open sildenafil. Primary end points were improvement in erectile function on

OBJECTIVE: To evaluate continued experience with a one-stage stapled ileoanal pouch procedure without temporary ileostomy diversion.
SUMMARY BACKGROUND DATA: Most centers perform colectomy, proctectomy, and ileal pouch anal anastomoses (IPAA) with a protective ileostomy. Following a previous report, the authors performed 126 additional stapled IPAA procedures for ulcerative colitis and familial adenomatous polyposis, of which all but 2 were without an ileostomy. Outcomes in these patients question the need for temporary ileal diversion, with its complications and need for subsequent surgical

AIM: Standard management with chemoradiotherapy (CRT) or total neoadjuvant therapy (TNT) followed by total mesorectal excision (TME) reduces local recurrence but often is associated with significant long-term functional impairment. Organ preservation (OP) has become a key therapeutic goal in rectal cancer to reduce surgery-related morbidity without compromising oncological outcomes. Three main OP strategies have been developed: Watch and Wait (W&W) strategy, Local Excision (LE) and Contact X-ray Brachytherapy (CXB), both applicable in patients showing a favourable tumour response after neoadju

INTRODUCTION: We conducted a retrospective study of all consecutive patients operated for rectal cancer with LAR and coloanal anastomosis in our tertiary center to compare the occurrence of LAR syndrome (LARS) more than 1 year after surgery between patients treated with DCAA and those with ICAA. Secondary objectives were to compare post-operative complications at 90 days. METHODS: From 2010 to 2022, 139 patients underwent LAR (ICAA, n = 45; DCAA, n = 25) with coloanal anastomosis for rectal cancer in our department. Long-term bowel function after LAR and post-operative outcomes were compared.

AIM: Anastomotic leak (AL) is the anathema of colorectal surgery and its occurrence constitutes a serious risk to patients and places a substantial burden on the health system. The analysis of extravasated intraluminal substances in drain fluid has shown promise for the early detection of AL. The aim of this study is to assess the measurement of drain fluid iodine as a biomarker of AL.
METHOD: This prospective, observational, 2b exploration cohort study measured the iodine in drain fluid of patients undergoing a low colorectal anastomosis and without a diverting ileostomy (DI) when the rectal
