Endolymphatic Hydrops in Fluctuating Hearing Loss and Recurrent Vertigo.

Background: Endolymphatic hydrops (EH) is the histopathological hallmark of Ménière's disease (MD) and has been found by in vivo magnetic resonance imaging (MRI) in patients with several inner ear syndromes without definite MD criteria. The incidence and relevance of this finding is under debate. Purpose: The purpose of the study is to evaluate the prevalence and characteristics of EH and audiovestibular test results in groups of patients with fluctuating audiovestibular symptoms not fulfilling the actual criteria for definite MD and compare them with a similar group of patients with definite

Dizziness in childhood.

Dizziness in childhood is not an infrequent symptom. Accurate history taking and close co-operation between otologist, paediatrician and neurologist are necessary in the approach to the dizzy child. Most cases of childhood dizziness settle in time and investigations should be carefully selected; those with severe and persistent dizziness or ataxia should be thoroughly investigated including: EEG, ENG, calorics and CT scan. The conditions causing dizziness in children are discussed and are illustrated with case histories from our series of 27 children. Dizziness of unknown aetiology, serous oti

Normative Values for the Head Shake Sensory Organization Test in an Active Duty Military Cohort.

INTRODUCTION: The Head Shake Sensory Organization Test (HS-SOT) assesses postural stability while the head is moving and may also identify deficits in attention associated with the dual task conditions of moving the head at a specified speed while maintaining balance. Normative values for the HS-SOT have not been established in a healthy military population or other highly trained populations such as athletes. Establishing normative values in a military population will enable clinicians to compare the scores of patients with medical conditions that affect postural stability and sensory integra

Assessment of the Progression of Vertical Semicircular Canal Dysfunction and Increased Vestibular Endolymphatic Hydrops in Patients With Early-Stage Ménière Disease.

IMPORTANCE: Vertical semicircular canals and endolymphatic hydrops play important roles in the pathophysiological mechanisms of Ménière disease. However, their characteristics and associations with disease progression during medical treatment have not been determined. OBJECTIVE: To examine the function of both the horizontal and vertical semicircular canals in patients with Ménière disease and to evaluate the change in endolymphatic hydrops volume during medical treatment, including treatment with diuretic therapy, over a 2-year period. DESIGN, SETTING, AND PARTICIPANTS: This prospective lon

Subclinical deviation of the subjective visual vertical in patients affected by a primary headache.

CONCLUSION: Our results suggest that patients with migraine or tension-type headache have subclinical deviations of the subjective visual vertical, which may be associated with their subjective imbalance. OBJECTIVES: Patients affected by migraine or tension-type headache often complain of unsteadiness. However, they rarely show a clinical significance in the objective examinations of their equilibrium. We investigated the equilibrium functions in patients affected by migraine or tension-type headache. SUBJECTS AND METHODS: We investigated the neurotological findings of 17 patients with migra

Comorbidities and impact on quality of life in patients with Ménière's disease: a single-center cross-sectional study.

BACKGROUND: Ménière's disease (MD) is a chronic vestibular disorder that significantly impacts patients' quality of life (QoL). However, the influence of comorbid conditions on QoL in MD patients remains inadequately explored. OBJECTIVE: To investigate the prevalence of comorbidities in patients with MD and analyze their impact on patients' QoL. METHODS: A cross-sectional analysis was conducted on 473 MD patients who received treatment in our hospital. Demographic data, comorbidity information, psychological symptoms (assessed by Hospital Anxiety and Depression Scale, HAD) and QoL (evaluated

Episodic Ataxias: Primary and Secondary Etiologies, Treatment, and Classification Approaches.

BACKGROUND: Episodic ataxia (EA), characterized by recurrent attacks of cerebellar dysfunction, is the manifestation of a group of rare autosomal dominant inherited disorders. EA1 and EA2 are most frequently encountered, caused by mutations in KCNA1 and CACNA1A. EA3-8 are reported in rare families. Advances in genetic testing have broadened the KCNA1 and CACNA1A phenotypes, and detected EA as an unusual presentation of several other genetic disorders. Additionally, there are various secondary causes of EA and mimicking disorders. Together, these can pose diagnostic challenges for neurologists.

A modern conceptual framework for study and treatment of Meniere's disease.

Prosper Meniere made his immortal contribution to the field of otology in 1861. At that time, all manner of "fits" were lumped together under the diagnosis of "apoplectiform cerebral congestion"-too much blood in the brain. His genius was to identify a specific subset of this heterogeneous pool whose cardinal symptoms, tinnitus, fluctuating progressive deafness, and episodic vertigo, were due to dysfunction of the inner ear. Seventy-seven years later, in 1938, Hallpike and Cairns in England and Yamakawa in Japan identified cochleosaccular endolymphatic hydrops (EH) as the histopathologic corre

Acrophobia and visual height intolerance: advances in epidemiology and mechanisms.

Historical descriptions of fear at heights date back to Chinese and Roman antiquity. Current definitions distinguish between three different states of responses to height exposure: a physiological height imbalance that results from an impaired visual control of balance, a more or less distressing visual height intolerance, and acrophobia at the severest end of the spectrum. Epidemiological studies revealed a lifetime prevalence of visual height intolerance including acrophobia in 28% of adults (32% in women; 25% in men) and 34% among prepubertal children aged 8-10 years without gender preponde

Abnormal intracellular ca(2+)homeostasis and disease.

A whole range of cell functions are regulated by the free cytosolic Ca(2+)concentration. Activator Ca(2+)from the extracellular space enters the cell through various types of Ca(2+)channels and sometimes the Na(+)/Ca(2+)-exchanger, and is actively extruded from the cell by Ca(2+)pumps and Na(+)/Ca(2+)-exchangers. Activator Ca(2+)can also be released from internal Ca(2+)stores through inositol trisphosphate or ryanodine receptors and is taken up into these organelles by means of Ca(2+)pumps. The resulting Ca(2+)signal is highly organized in space, frequency and amplitude because the localizatio

Relationship of vitamin D levels with clinical presentation and recurrence of BPPV in a Southeastern United States institution.

OBJECTIVE: To examine the relationship of 25hydroxyvitamin D serum levels with BPPV incidence and recurrence rates. METHODS: A retrospective cross-sectional, case-controlled study with follow-up phone survey was performed on patients diagnosed with BPPV between 05/2017-05/2020, who had available 25hydroxyvitamin D serology. Patients were seen at a multidisciplinary, vestibular-focused, neurotology clinic at a tertiary referral center. Controls consisted of subjects from the National Health and Nutrition Examination Survey (NHANES), and a locoregional age, sex, and race-matched group of patien

Test-retest reliability of the Vestibular Ocular Motor Screening (VOMS) tool and modified Balance Error Scoring System (mBESS) in US military personnel.

UNLABELLED: The Military Acute Concussion Evaluation 2 (MACE 2), which includes the Vestibular-Ocular Motor Screening (VOMS) tool and the single-leg stance component of the modified Balance Error Scoring System (mBESS), was introduced in 2018 as an assessment of acute mTBI in US military personnel. However, the reliability of the VOMS and mBESS in this population has not been established. OBJECTIVES: The primary purpose of this study was to examine the reliability of the VOMS across a 6-month period in healthy, uninjured US Army Special Operations Command (USASOC) personnel. DESIGN: Active d

Central positional nystagmus: an update.

Clinically, central positional nystagmus (CPN) is often suspected when atypical forms of its peripheral counterpart, i.e., benign paroxysmal positional vertigo (BPPV), are observed, namely a linear horizontal nystagmus as in horizontal canal BPPV or a downwardly and torsionally beating nystagmus as in anterior canal BPPV. Pathophysiologically, CPN is caused by cerebellar and/or brainstem dysfunction. Recent work has provided further insights into the different clinical phenotypes and the underlying pathomechanisms. We performed a PubMed review focused on the findings on CPN using the key words

Prevalence, clinical profile, and diagnosis of pediatric dizziness in a tertiary care hospital.

INTRODUCTION: Pediatric dizziness is an uncommon complaint presenting to the Otolaryngology clinic. While the term dizziness may be used to describe any altered sensation of orientation to the environment which includes presyncope, light-headedness and ataxia, vertigo refers to a false sensation of motion of self or surroundings. Although a variety of etiologies are known to cause dizziness and vertigo, evaluation of this symptom becomes challenging in children who are unable to clearly explain what they experience, the provoking factors, associated symptoms and the duration of attacks. Caloric test and video-head-impulse: a study of vertigo/dizziness patients in a community hospital.

The head-impulse test (HIT) is an important test for examining unilateral vestibular hypofunction. The new video-head-impulse test (vHIT) is more sensitive and specific than the clinical bedside-head-impulse test (bHIT). Alternatively, one can test for vestibular hypofunction with the caloric irrigation test. Various studies showed that both tests may not always identify vestibular hypofunction; instead, the results of the tests might be contradictory. To evaluate the diagnostic value of these tests, we routinely measured patients exhibiting vertigo or dizziness at our community hospital with

Risk factors for benign paroxysmal positional vertigo recurrence: a systematic review and meta-analysis.

BACKGROUND AND PURPOSE: Benign paroxysmal positional vertigo is one of the most common vestibular diseases, especially in the elderly. Although the treatment of BPPV is relatively successful, many patients suffer recurrence after treatment. Therefore, identifying potential risk factors for BPPV recurrence may help improve treatment outcome and patient prognosis. However, some related risk factors for BPPV recurrence are relatively controversial and poorly studied. This meta-analysis aims to identify potential risk factors associated with BPPV recurrence, thereby reducing the recurrence rate of

Endoscopic-assisted surgical repair of ear cerebrospinal fluid leakage through a subtemporal craniotomy.

BACKGROUND: CSF fistula involves abnormal leakage of cerebrospinal fluid (CSF) from its normal pathways, typically from high- to low-pressure areas at the base of the skull. It requires an osteomeningeal breach, causing discomforts such as headaches and abnormal drainage. Temporal bone CSF leakage often results from trauma or chronic otitis media, and its surgical treatment is often delayed. The application of endoscopes in skull base procedures has become increasingly common, offering new ways to improve visualization during performance. Endoscopic tools provide a broad view, sharp image qual

Comprehensive Clinical Profile of Mal De Debarquement Syndrome.

BACKGROUND: There has been increasing awareness that post-motion triggered rocking self-vertigo can last for months or years, a disorder known as Mal de Debarquement Syndrome (MdDS). A similar feeling of oscillating self-motion can occur without a motion trigger in some individuals, leading to controversy about whether motion triggered (MT) and non-motion triggered (non-MT) symptoms ultimately represent the same disorder. Recognizing the similarities and differences between MT and non-MT MdDS can prevent unnecessary diagnostic testing and lead to earlier and more effective treatments. METHODS

Targeted multidomain intervention for complex mTBI: protocol for a multisite randomized controlled trial in military-age civilians.

BACKGROUND: Mild traumatic brain injury (mTBI) affects ~18,000 military personnel each year, and although most will recover in 3-4 weeks, many experience persisting symptoms and impairment lasting months or longer. Current standard of care for U.S. military personnel with complex mTBI involves initial (<48 h) prescribed rest, followed by behavioral (e.g., physical activity, sleep regulation, stress reduction, hydration, nutrition), and symptom-guided management. There is growing agreement that mTBI involves different clinical profiles or subtypes that require a comprehensive multidomain evalua
