CSF Aquaporin-4 levels in controls and in idiopathic normal pressure hydrocephalus before and after shunt surgery.

BACKGROUND: Aquaporin-4 (AQP4) is crucial for brain fluid regulation and glymphatic system function. Idiopathic normal pressure hydrocephalus (INPH) is characterized by impaired CSF flow and is treated with shunt surgery. This study investigated AQP4 levels in INPH patients to explore its role in pathophysiology and as a potential biomarker for shunt response. METHODS: CSF samples from 233 INPH patients and 29 controls were analysed. AQP4 levels were compared between preoperative patients and controls, before and after shunt surgery (110 patients), and between shunt responders and non-respond

The TyrRS cascade: circadian gating of neuronal DNA repair and its collapse in aging.

Age-related neurodegenerative diseases are characterized by progressive DNA damage in post-mitotic neurons against a backdrop of deteriorating circadian rhythms, yet the molecular link between these conjoined features of brain aging remains unclear. We propose the TyrRS cascade as that link: a signaling architecture in which the noncanonical nuclear functions of tyrosyl-tRNA synthetase (TyrRS/YARS1) schedule neuronal genome maintenance across the day through three coregulated streams, PARP1-mediated damage sensing, TRIM28/NuRD heterochromatin maintenance, and LIN9/DREAM control of a 67-gene re

Deep Cervical Lymphovenous Anastomoses Improved Brain Function and Internal Neck Edema in Central Lymphatic Anomaly Associated with 13q Deletion Syndrome.

We report a case of a 21-year-old woman with 13q deletion syndrome, a rare chromosomal disorder associated with developmental delays, craniofacial dysmorphism, and multiorgan abnormalities associated with central conducting lymphatic anomaly. Her symptoms included bilateral chylothorax, pulmonary lymphangiectasis, generalized edema, and recurrent respiratory distress. Despite conservative management, her condition progressively worsened, leading to significant edema affecting the tongue and upper airway. Magnetic resonance lymphangiography revealed a severe central lymphatic flow disorder with

Large molecules from the cerebrospinal fluid enter the optic nerve but not the retina of mice.

It has been proposed that cerebrospinal fluid (CSF) can enter and leave the retina and optic nerve along perivascular spaces surrounding the central retinal vessels as part of an aquaporin-4 (AQP4) dependent ocular 'glymphatic' system. Here, we injected fluorescent dextrans and antibodies into the CSF of mice at the cisterna magna and measured their distribution in the optic nerve and retina. We found that uptake of dextrans in the perivascular spaces and parenchyma of the optic nerve is highly sensitive to the cisternal injection rate, where high injection rates, in which dextran disperses fu

A cross-comparative analysis of in vivo versus ex vivo MRI indices in a mouse model of concussion.

BACKGROUND: We present a cross-sectional, case-matched, and pair-wise comparison of structural magnetic resonance imaging (MRI), diffusion tensor imaging (DTI), and neurite orientation dispersion and density imaging (NODDI) measures in vivo and ex vivo in a mouse model of concussion, thus aiming to establish the concordance of structural and diffusion imaging findings in living brain and after fixation. METHODS: We allocated 28 male mice aged 3-4 months to sham injury and concussion (CON) groups. CON mice had received a single concussive impact on day 0 and underwent MRI at day 2 (n = 9) or 7

Management of Cerebral Edema, Brain Compression, and Intracranial Pressure.

PURPOSE OF REVIEW: This article reviews the pathophysiology and management of cerebral edema, brain compression, and elevated intracranial pressure (ICP). It also provides a brief introduction to the concept of the glymphatic system and select cellular contributors to cerebral edema. RECENT FINDINGS: Cerebral edema and brain compression should be treated in a tiered approach after the patient demonstrates a symptomatic indication to start treatment. All patients with acute brain injury should be treated with standard measures to optimize intracranial compliance and minimize risk of ICP elevat

Sleep and β-Amyloid Deposition in Alzheimer Disease: Insights on Mechanisms and Possible Innovative Treatments.

The growing interest in the preclinical stage of Alzheimer's disease (AD) led investigators to identify modifiable risk and predictive factors useful to design early intervention strategies. The preclinical stage of AD is characterized by β-amyloid (Aβ) aggregation into amyloid plaques and tau phosphorylation and aggregation into neurofibrillary tangles. There is a consensus on the importance of sleep within this context: the bidirectional relationship between sleep and AD pathology is supported by growing evidence that proved that the occurrence of sleep changes starting from the preclinical

Dynamical Regulation of Blood Pressure and Cognitive Function.

BACKGROUND: Blood pressure (BP) is not steady. It varies over intervals from months to consecutive cardiac cycles, and this variation contains meaningful information beyond mean BP. Variability over multiple clinic visits (VVV-BP) and during 24-h ambulatory monitoring (ABPV) is positively related to risk of stroke and coronary artery disease and negatively associated with cognitive performance. Beat-to-beat BP variation, often quantified as low frequency variability (0.04-0.15 Hz, LF-BPV), is less well-studied. Here, we examine the relationship between LF-BPV and cognitive outcomes in 1953 par

Circadian Rhythm Disruption in Alzheimer's Disease: Mechanistic Insights, Diagnostic Implications, and Emerging Chronotherapeutic Strategies: A Narrative Review.

Alzheimer's disease (AD) is a progressive neurodegenerative disease with a complex etiology that involves environmental, genetic, and metabolic components. Among these, circadian rhythm disruption has been seen critical but underexplored component in the pathogenesis of Alzheimer's disease. The suprachiasmatic nucleus (SCN), the central pacemaker of circadian regulation, coordinates all the physiological processes, such as sleep-wake cycles, metabolism, and neuroendocrine signalling. Tau-pathology, neuroinflammation, amyloid-β build-up, and cognitive decline in Alzheimer's disease are all beco

What is idiopathic in normal pressure hydrocephalus?

INTRODUCTION: Normal pressure hydrocephalus (NPH) can be caused by acquired events - e.g. subarachnoid hemorrhage, meningitis, or trauma - or can be "idiopathic" (iNPH) when no clear cause is identifiable. The entity and nosology of iNPH has received renewed attention and has recently gone through scrutiny and academic debate. EVIDENCE ACQUISITION: Authors searched PubMed using the following keywords: "adult hydrocephalus," "alfa synuclein," "Alzheimer's disease," "beta-amyloid," "cerebrospinal fluid," "cilia," "CSF," "genes," "hydrocephalus," "idiopathic," "Lewy Body Dementia," "phosphorylat

Impact of Sleep Disorders and Disturbed Sleep on Brain Health: A Scientific Statement From the American Heart Association.

Accumulating evidence supports a link between sleep disorders, disturbed sleep, and adverse brain health, ranging from stroke to subclinical cerebrovascular disease to cognitive outcomes, including the development of Alzheimer disease and Alzheimer disease-related dementias. Sleep disorders such as sleep-disordered breathing (eg, obstructive sleep apnea), and other sleep disturbances, as well, some of which are also considered sleep disorders (eg, insomnia, sleep fragmentation, circadian rhythm disorders, and extreme sleep duration), have been associated with adverse brain health. Understandin

Association of choroid plexus volume with motor symptoms and dopaminergic degeneration in Parkinson's disease.

BACKGROUND: The choroid plexus (CP) is involved in the clearance of harmful metabolites from the brain, as a part of the glymphatic system. This study aimed to investigate the association between CP volume (CPV), nigrostriatal dopaminergic degeneration and motor outcomes in Parkinson's disease (PD). METHODS: We retrospectively searched drug-naïve patients with early-stage PD who underwent dopamine transporter (DAT) scanning and MRI. Automatic CP segmentation was performed, and the CPV was calculated. The relationship between CPV, DAT availability and Unified PD Rating Scale Part III (UPDRS-II

A phase I/II study on intracerebroventricular tralesinidase alfa in patients with Sanfilippo syndrome type B.

BackgroundSanfilippo type B is a mucopolysaccharidosis (MPS) with a major neuronopathic component characterized by heparan sulfate (HS) accumulation due to mutations in the NAGLU gene encoding alfa-N-acetyl-glucosaminidase. Enzyme replacement therapy for neuronopathic MPS requires efficient enzyme delivery throughout the brain in order to normalize HS levels, prevent brain atrophy, and potentially delay cognitive decline.MethodsIn this phase I/II open-label study, patients with MPS type IIIB (n = 22) were treated with tralesinidase alfa administered i.c.v. The patients were monitored for drug

Cerebrospinal fluid volumetric net flow rate and direction in idiopathic normal pressure hydrocephalus.

The aim of the present study was to examine cerebrospinal fluid (CSF) volumetric net flow rate and direction at the cranio-cervical junction (CCJ) and cerebral aqueduct in individuals with idiopathic normal pressure hydrocephalus (iNPH) using cardiac-gated phase-contrast magnetic resonance imaging (PC-MRI). An in-depth, pixel-by-pixel analysis of regions of interest from the CCJ and cerebral aqueduct, respectively, was done in 26 iNPH individuals, and in 4 healthy subjects for validation purposes. Results from patients were compared with over-night measurements of static and pulsatile intracra

Cerebral Waste Accumulation and Glymphatic Clearance as Mechanisms of Human Neurological Diseases.

The brain is a complex system that requires continual regulation of parenchymal pressure, osmolarity, and waste removal for optimal function; despite this, human brain lacks any obvious extension of lymphatic circulation for moderating fluid and waste regulation. We recapitulate herein a recent analysis of proteinaceous waste deposition in the human brain, its observed route of clearance, and the implications of abnormal accumulation along this clearance pathway as a potential mechanism of neurological diseases. This study uncovered an analogous staining pattern of cerebral phosphorylated tau

CSF Flow in the Brain in the Context of Normal Pressure Hydrocephalus.

CSF normally flows back and forth through the aqueduct during the cardiac cycle. During systole, the brain and intracranial vasculature expand and compress the lateral and third ventricles, forcing CSF craniocaudad. During diastole, they contract and flow through the aqueduct reverses. Hyperdynamic CSF flow through the aqueduct is seen when there is ventricular enlargement without cerebral atrophy. Therefore, patients presenting with clinical normal pressure hydrocephalus who have hyperdynamic CSF flow have been found to respond better to ventriculoperitoneal shunting than those with normal or

Associations of DTI Metrics with Structural and Cerebrovascular Disease Imaging Metrics in Idiopathic Normal Pressure Hydrocephalus.

BACKGROUND AND PURPOSE: Idiopathic normal pressure hydrocephalus (iNPH) symptoms can be alleviated through shunt placement. However, the identification of suitable patients is often challenging despite several existing biomarkers. DTI is commonly used to assess white matter integrity, with DTI analysis along the perivascular space being a recently described though less extensively evaluated metric. It has been proposed to monitor glial-lymphatic (glymphatic) activity in the brain, the impairment of which may play a role in several neurologic diseases. The goal of this study is to evaluate the

Perivascular Spaces Are Associated With CSF Aβ in Cerebral Amyloid Angiopathy but Not in Deep Perforator Arteriopathy.

BACKGROUND: Centrum semiovale perivascular spaces (CSO PVS) are common to different cerebral small vessel disease (CSVD) subtypes, yet their enlargement to the point of visibility on magnetic resonance imaging is thought to occur through distinct mechanisms. In cerebral amyloid angiopathy (CAA), CSO PVS are thought to reflect impaired perivascular Aβ (amyloid-β) drainage, whereas in deep perforator arteriopathy (DPA) they may result from microangiopathy-related arterial stiffening and altered fluid dynamics. The extent to which this can be confirmed in vivo remains unclear. METHODS: We retros
