Management of irreparable massive rotator cuff tears: a systematic review and meta-analysis of patient-reported outcomes, reoperation rates, and treatment response.

BACKGROUND: There is no consensus on the treatment of irreparable massive rotator cuff tears. The goal of this systematic review and meta-analysis was to (1) compare patient-reported outcome scores, (2) define failure and reoperation rates, and (3) quantify the magnitude of patient response across treatment strategies. METHODS: The MEDLINE, Embase, CENTRAL (Cochrane Central Register of Controlled Trials), and Scopus databases were searched for studies including physical therapy and operative treatment of massive rotator cuff tears. The criteria of the Methodological Index for Non-randomized S

Incidence of early development of radiolucent lines in keeled polyethylene glenoid components after total shoulder arthroplasty.

Glenoid loosening after total shoulder arthroplasty (TSA) remains a major concern. The purpose of this study was to determine the incidence of radiolucent lines (RLLs) after TSA performed with modern glenoid bone preparation and cement techniques for keeled-back glenoid components. One hundred ten consecutive patients with osteoarthritis were included in this study. Patients had undergone primary TSA with a keeled-back glenoid component. Mean age was 64.0 years (SD, 10.6 years; range, 27-91 years). Two independent, blinded observers assessed the initial postoperative radiographs for RLLs using

[Corrective interventions subsequent to humeral head fractures].

The complications of old fractures of the proximal humeral head and fracture dislocations are nonunion, malunion, osteonecrosis, early traumatic arthritis, chronic dislocation and instability with subluxation. We reviewed a consecutive series of 34 shoulders that were treated between 1986 and 1990. We performed 7 hemiarthroplasties and 11 total shoulder arthroplasties (Neer type) in 17 patients (group I). Group II consisted of 12 osteotomies, 3 internal fixations for nonunion (AO technique), and 1 shoulder arthrodesis. The average follow-up was 27 months (group I) and 26 months (group II), res

Outcomes of primary reverse shoulder arthroplasty in patients with morbid obesity.

BACKGROUND: Obesity is a risk factor for worse outcomes in anatomic shoulder arthroplasty. The purpose of this investigation was to determine outcomes of primary reverse shoulder arthroplasty (RSA) in patients with morbid obesity. METHODS: We reviewed all primary RSAs performed on morbidly obese (body mass index [BMI] ≥40 kg/m(2)) patients from 2005 to 2012 at our institution with at least 2 years of follow-up. The study included 41 patients (57% female), the mean BMI was 44 kg/m(2), and the mean age was 67.7 years. RESULTS: The only intraoperative complication was an inferior glenoid fractu

The Effect of Motor Imagery Ability on Function and Proprioception in Myoelectric Prosthesis Users: Protocol for a Cross-Sectional Study.

BACKGROUND: Motor imagery ability (MIA) is a critical cognitive process that enables the mental simulation of movements, facilitating skill acquisition in the use of myoelectric prostheses and their effective control in individuals who have undergone amputation. Although MIA is essential, its impact on upper extremity function and proprioception in myoelectric prosthesis users remains insufficiently studied. OBJECTIVE: This study aims to evaluate MIA in individuals who have undergone transradial amputation and use myoelectric prostheses, compare it with a healthy control group, and examine it

A Meta-Analysis of Polymerase Chain Reaction for Prosthetic Joint Infection Diagnosis.

BACKGROUND: One of the most dangerous side effects of joint replacement for the hip, knee, shoulder, and elbow is prosthesis joint infection (PJI). Polymerase chain reaction (PCR) has been considered a promising method for PJI diagnosis due to its short diagnostic time and high sensitivity. Although several PCR methods such as multiplex PCR and broad-range PCR are useful diagnostic methods for detecting microorganisms causing PJI, values of different PCR methods for the diagnosis of PJI remain unclear. Thus, the objective of this study was to perform a meta-analysis of different PCR methods in

Functional Outcomes and Health-Related Quality of Life of Adults With Congenital Below-Elbow Amputation in North America.

PURPOSE: Congenital below-elbow amputation (BEA) is a common upper-extremity anomaly and generally encompasses 2 diagnoses, symbrachydactyly and transverse deficiency. Little is known about the physical, mental, and social well-being of adults with congenital BEA. A deeper understanding of longitudinal outcomes within this population may help guide family conversations and counseling for patients with congenital BEA. METHODS: The Shriners Hospitals for Children Health Outcomes Network was queried to identify all patients currently >18 years of age who had been seen as a child between 1975 and

Midterm results of anatomic total shoulder arthroplasty with a third-generation implant.

BACKGROUND: Anatomic total shoulder arthroplasty (TSA) provides reliable, reproducible, and durable results; however, outcomes of many modern TSA systems are lacking. The present study reports early to midterm results of a third-generation TSA system using a traditional-length press-fit humeral stem and cemented glenoid. METHODS: A retrospective review was conducted of TSA patients with minimum 2-year clinical follow-up. Patient-reported outcome measures, including Simple Shoulder Test, American Shoulder and Elbow Surgeons Total, visual analog scale for pain, 12-Item Short Form Health Survey,

Mildly decreased preoperative bilirubin levels are associated with infections after shoulder and knee arthroplasty.

Increasing numbers of arthroplasties are also accompanied by postoperative infections. The main purpose was to evaluate preoperative serum bilirubin levels between patients with and without infections after shoulder and knee arthroplasties. For this retrospective case-control single-center study, a total of 108 patients were extracted from a prospectively collected database. Eighteen patients with infections after shoulder (n = 8) and knee (n = 10) arthroplasty were matched by age, gender, and implant type in a 1:5-scenario to 90 patients (40 shoulders and 50 knees) without postoperative infec

Accuracy of the Subchondral Smile and Surface Referencing Techniques in Reverse Shoulder Arthroplasty.

Inferior glenoid baseplate tilt relative to the coronal axis of the scapular body has been associated with improved results and fewer postoperative complications in reverse shoulder arthroplasty. However, the native glenoid surface is not always a reliable reference for the true scapular axis. Digital preoperative planning software and advanced imaging now allow surgeons to more precisely determine optimal glenoid placement. The purpose of this study was to evaluate the accuracy of the subchondral smile and cannulated surface guide techniques in achieving inferior glenoid baseplate tilt by usi

Vacuum assist glenoid fixation: does this technique lead to a more durable glenoid component?

INTRODUCTION: This was a prospective random study to determine if suction applied to the glenoid vault through the coracoid process would facilitate deeper penetration of cement into the vault, and if deeper penetration would lead to a more durable glenoid component. MATERIALS AND METHODS: A group of 96 patients were randomly assigned to 1 of 2 pathways for glenoid fixation. One was cemented using modern "third generation" cementing techniques, while the second group utilizes these same steps as well as a vacuum assist (weep-hole) technique. We measured the "amount" of cement that penetrated

Medial support by fibula bone graft in angular stable plate fixation of proximal humeral fractures: an in vitro study with synthetic bone.

BACKGROUND: Failure to achieve stable fixation with medial support in proximal humeral fractures can result in varus malalignment and cut-through of the proximal screws. The purpose of this study was to investigate the influence of an intramedullary fibula bone graft on the biomechanical properties of proximal humeral fractures stabilized by angular stable plate fixation in a bone model under cyclic loading. METHODS: Two fixation techniques were tested in 20 composite analog humeri models. In group F- (n = 10), fractures were fixed by an anatomically formed locking plate system. In group F+ (

Results of the percutaneous pinning of proximal humerus fractures with a modified palm tree technique.

PURPOSE: Many techniques are available for closed reduction and percutaneous fixation of proximal humeral fractures. The palm tree technique was described by Kapandji in 1989. In that technique three curved wires are inserted through one hole at the V-shaped insertion of the deltoid muscle. It is a good technique but it has some disadvantages. In this study we introduced some modifications for the technique, and we evaluated the clinical results of the modified technique in treatment of 18 cases. METHODS: Eighteen patients with displaced proximal humeral fractures were treated by closed reduc

Radiographic comparison of flat-back and convex-back glenoid components in total shoulder arthroplasty.

The purpose of this study was to compare the radiographic results of 2 different glenoid component designs. This series consisted of 66 shoulder arthroplasties with primary osteoarthritis divided into 2 groups based on glenoid component type. One group comprised shoulders receiving cemented flat-back polyethylene glenoid implants. The other group comprised shoulders receiving cemented convex-back polyethylene glenoid implants. Immediate postoperative and 2-year postoperative radiographs were evaluated for the presence and progression of periglenoid radiolucencies, and the 2 groups were compare

Shoulder joint replacement for osteoarthrosis in association with thalidomide-induced phocomelia.

The natural history of thalidomide-induced phocomelia has not yet been established since only 35 years have elapsed since the problems associated with this drug became evident. This paper presents what is considered to be the first case of osteoarthrosis in such an individual treated by shoulder joint replacement. Joint arthoplasty is almost certainly going to become an integral part of the life-long rehabilitation process in such affected individuals.

Shoulder function after prosthetic replacement of proximal humerus.

Between 1973 and 1985 eighteen patients with bone tumours and two patients with comminuted fractures had their proximal humerus replaced with a custom made isoelastic hemiendoprosthesis. In 1987 a clinical and radiographical review was performed of eleven patients with a mean follow up time of seven years, range 3-10 years. Four patients had pain at rest and six patients experienced severe pain on exertion. Active range of motion was poor and constituted only half of the corresponding range of passive motion. Shoulder girdle muscles were generally weak, only one patient could keep the arm stra

Towards standardised definitions of shoulder arthroplasty complications: a systematic review of terms and definitions.

INTRODUCTION: A transparent, reliable and accurate reporting of complications is essential for an evidence-based evaluation of shoulder arthroplasty (SA). We systematically reviewed the literature for terms and definitions related to negative events associated with SA. MATERIALS AND METHODS: Various biomedical databases were searched for reviews, clinical studies and case reports of complications associated with SA. Any general definition of a complication, classification system, all reported terms related to complications and negative events with their definitions were extracted. Terms were

Nontobacco Nicotine Dependence and Rates of Periprosthetic Joint Infection and Other Postoperative Complications in Shoulder Arthroplasty: A Retrospective Analysis.

INTRODUCTION: Nontobacco nicotine products, including e-cigarettes and vaping, are marketed as healthier alternatives to tobacco. However, the literature on nontobacco nicotine dependence (NTND) is scarce. It is important to analyze the influence of these products as they pertain to medical and surgical postoperative complications. This study hypothesizes that patients with NTND will experience more postoperative complications. METHODS: Using the TriNetX database, which aggregates deidentified medical records from 89 healthcare organizations in the Research Network, Current Procedural Termino
