INTRODUCTION: Biliopancreatic limb obstruction is a surgical emergency in patients who have undergone gastric bypass surgery. This case report presents an unexpected cause of biliopancreatic limb obstruction and the importance of prompt intervention.

A case controlled study comprising 51 patients with homozygous sickle cell (HbSS) disease who complained of dyspepsia and 41 age and sex matched non-HbSS control dyspeptic patients was carried out, to look at upper gastrointestinal mucosal lesions associated with dyspepsia. Upper gastrointestinal tract (UGIT) endoscopy was performed with gastric control biopsy taken for histology. We studied reflux of duodenal contents into the stomach in patients with gastric ulcers, patients with duodenal ulcers, and normal subjects. Duodenogastric reflux was assessed in the fasting state and after cholecystokinin octapeptide administration (0.02 micrograms/kg intravenously). A simple radiological technique is described for detecting the presence, or absence, of duodeno-gastric reflux (DGR) at barium meal examination. The incidence of DGR was found to be approximately the same in both normal volunteers and dyspeptic patients. Although there was a slight trend towards an increased incidence in gastric ulcer, this did not reach statistical significance. Tubes of canine gastric corpus were constructed so that the mucosa was exposed chronically to jejunal contents, pancreatic juice, or bile. Biopsies of these mucosae obtained 4 to 24 months later were compared with each other and with the normal stomach for parietal cell numbers, mucus cell numbers, inflammation, glandular disorder, and surface-cell hyperplasia. All three preparations showed marked gastrititis, but whole jejunal contents caused more severe changes than did either pancreatic juice or bile, which were approximately equal.

PURPOSE: We investigated the relationship between the resected stomach measurements, the incidence of delayed gastric emptying (DGE), and food residue 1 year after surgery in patients who underwent laparoscopic pylorus-preserving gastrectomy (PPG).
MATERIALS AND METHODS: The DGE group included 10 patients fasting due to nausea, vomiting, abdominal distension, or remnant stomach distension on radiographs; the control group included 36 patients without these symptoms.
