Congenital malformations of the lung.

Congenital malformations of the lung are rare and vary widely in their presentation and severity. The most common manifestation of the congenital cystic disease of the lung at newborn and early infancy is respiratory distress. Later on in life, cysts usually lose this compressive character and may remain asymptomatic until infection occurs, while producing cough, dyspnea and thoracic pain. The purpose of this study is to review authors institutional experience of congenital cystic lung disease, with specific reference to diagnosis, treatment, as well as outcome, furthermore, to present some ca

Diffusion-weighted MRI for distinguishing non-neoplastic cysts from solid masses in the mediastinum: problem-solving in mediastinal masses of indeterminate internal characteristics on CT.

OBJECTIVES: To evaluate the usefulness of diffusion-weighted (DW) magnetic resonance images for distinguishing non-neoplastic cysts from solid masses of indeterminate internal characteristics on computed tomography (CT) in the mediastinum. METHODS: We enrolled 25 patients with pathologically proved mediastinal masses who underwent both thoracic CT and magnetic resonance imaging (MRI) including diffusion-weighted imaging (DWI). MRI was performed in patients with mediastinal masses of indeterminate internal characteristics on CT. Two thoracic radiologists evaluated the morphological features an

Video-Assisted Thoracoscopic Treatment of Coelomic Cysts.

Background: Coelomic cysts are uncommon benign cyctic lesions of the mediastinum. Complete resection of the cyst with surgical treatment is indicated depending on whether the patient is symptomatic or whether the cyst grows during follow-up, or whether the diagnosis is uncertain. Surgical treatment prevents complications, relieves symptoms, and establishes diagnosis. Video-assisted thoracoscopic surgery (VATS) is the best method for excision of these cysts. This approach is safe and effective, has low morbidity, and shortens the length of hospital stay Objectives: We aimed to analyze our exper

Prevalence of Pulmonary Bullae and Blebs in Postmortem CT Imaging With Potential Implications for Diving Medicine.

BACKGROUND: Pulmonary bullae and blebs can result in a pneumothorax. Their prevalence in the normal population is currently unknown. Postmortem CT (PMCT) images from a forensic database were used to determine the prevalence of pulmonary bullae/blebs in the normal Dutch adult population and its consequence for diving medicine, as bullae and blebs are often considered a contraindication for diving. METHODS: Forensic PMCT scans of 130 adults were analyzed for the presence of bullae and/or blebs in a cross-section of the Dutch population without pulmonary disorders. The lungs had to be unharmed,

Hearing Loss and Cognitive Function: An Analysis in Older Adults at the Beginning of the Auditory Rehabilitation Process.

Congenital lung malformations, primary and secondary, contribute to an important portion of pediatric thoracic surgery. One purpose of this report is to outline the close relation in terms of embryology and clinical presentation of congenital parenchymatous pulmonary malformations. In a retrospective study we also aim to evaluate our experience with the diagnosis and surgical management of congenital parenchymatous bronchopulmonary malformations and to compare our data with the literature. From January 1979 to December 1996 a series of 48 patients, 30 males (62.5%) and 18 females (37.5%), were

Congenital lung lesions: prenatal MRI and postnatal findings.

BACKGROUND: Congenital lung lesions refer to a spectrum of malformations and developmental abnormalities of the foregut, pulmonary airways and vasculature. These lesions range from small, asymptomatic to large space-occupying masses that can increase risk of fetal death and respiratory compromise after birth. Prenatal sonography has been used for routine screening in pregnancy. The advent of prenatal magnetic resonance imaging leads to complementary use in the diagnosis of fetal anomalies, including in fetuses with congenital lung lesions. OBJECTIVE: To determine whether fetal MRI can differe

Congenital Lung Malformations and Quantitative Assessment of Infant Tracheal-Bronchial Dimensions.

BACKGROUND: Congenital lung malformations (CLMs) are heterogeneous developmental anomalies caused by disrupted airway branching morphogenesis, but whether the central airways are also involved remains unclear. We quantified infant tracheobronchial dimensions in CLMs and compared them with published infant reference values. METHODS: This retrospective single-center study included 81 of 94 infants with CLMs assessed for eligibility. Exclusions were secretions preventing measurement (n = 3), motion artifacts (n = 7), unavailable preoperative computed tomography (CT) (n = 2), and missing diagnosi

Birt-Hogg-Dubé syndrome. State-of-the-art review with emphasis on pulmonary involvement.

BACKGROUND: Birt-Hogg-Dubé syndrome (BHDS) is a rare, inherited autosomal-dominant disorder characterized by the development of cutaneous lesions, renal tumors, pulmonary cysts, and spontaneous pneumothorax. The gene responsible for BHDS is located on the short arm of chromosome 17 (17p11.2) and codes for the protein folliculin, which is believed to be an oncogene suppressor protein. METHODS: We reviewed currently published literature on the main characteristics of BHDS. RESULTS: Pulmonary cysts and spontaneous pneumothorax are often the presenting manifestations that lead to a final diagnos

[A screening test in looking for adrenal tumors (author's transl)].

2600 abdominal ultrasound examinations done over a period of one year were reviewed. Six cases of adrenal tumors were amongst them and proved to be two carcinomas, two simple cysts and two metastases of bronchial carcinoma. The ultrasonic signs of adrenal tumors are discussed. Ultrasonography is recommended as a fairly good screening test in looking for adrenal tumors.

A 50-year-old man with incidental cardiac mass.

CLINICAL INTRODUCTION: A 50-year-old white male with a history of paroxysmal atrial fibrillation presented for transoesophageal echocardiogram prior to atrial fibrillation ablation. However, an echo lucent mass was noted (figure 1A). Colour Doppler and contrast administration showed no flow across the mass or the interatrial septum (see online supplementary videos 1 and 2). CT of the chest demonstrated a thin-walled, well-demarcated mass in the inferior border of the fossa ovalis protruding into the left atrium (figure 1B). QUESTION: Which of the following is the most likely diagnosis? Atrial

Thoracoscopic resection of the pulmonary aspergilloma: report of two cases.

Pulmonary aspergillomas causing hemoptysis were successfully resected through videothoracoscopy in two cases. We removed an aspergilloma located in the right lower lobe of the lung in a 48-year-old woman with a bronchial cyst and rheumatoid arthritis. We also removed an aspergilloma located in the left upper lobe of the lung in a 59-year-old man with emphysematous bullae and diabetes mellitus. In both cases, the aspergillomas were removed by pulmonary wedge resection through scheduled thoracoscopy.

Congenital thoracic masses: imaging features in the adult.

Congenital anomalies in the adult thorax are frequently asymptomatic and may remain undetected for many years. Whether they then become symptomatic or are discovered as an incidental finding on an imaging study obtained for unrelated reasons, thoracic anomalies may appear as masses or contour abnormalities that mimic other pathology, particularly neoplastic disease. The aim of this review is to discuss and illustrate a wide variety of congenital thoracic anomalies that can potentially present as a mass in the thorax. For ease of discussion, lesions are classified under numerous headings, inclu
