OBJECTIVE: Although hearing impairment after microvasuclar decompression (MVD) for hemifacial spasm (HFS) is not common, its occurrence could detrimentally affect the patient's surgical outcome. The object of this study is to address the optimal approaches for reducing postoperative hearing problems after MVD for HFS.
METHODS: We retrospectively analyzed the medical records of patients with HFS who underwent MVD with the same surgeon at our institute from March 2003 to October 2016, and reviewed the pertinent literature. Patients who were followed up for more than 6 months were selected, resu

BACKGROUND: Brain mapping during awake craniotomy is a well-known technique to preserve neurological functions, especially the language. It is still challenging to map the optic radiations due to the difficulty to test the visual field intraoperatively. To assess the visual field during awake craniotomy, we developed the Functions' Explorer based on a virtual reality headset (FEX-VRH).
METHODS: The impaired visual field of 10 patients was tested with automated perimetry (the gold standard examination) and the FEX-VRH. The proof-of-concept test was done during the surgery performed on a patien

It is well established that a mixed-agent general anesthetic regimen of volatile gas and intravenous anesthetic or total intravenous anesthetic (TIVA) is required to obtain adequate transcranial motor-evoked potentials (TcMEPs) to detect and hopefully prevent injury during brain, spinal cord, and peripheral nerve surgery. But even under ideal general anesthetic conditions, TcMEPs are not always detectable in every muscle monitored, and are prone to anesthetic fade, especially when neuropathic or injured tissue is monitored. TcMEP sensitivity to general anesthesia can be especially problematic

PURPOSE: Recurrent tethered cord syndrome (TCS) with split cord malformation (SCM) is a rare presentation with distinct surgical challenges to successful patient outcomes. SCMs are rare and can progress to cause significant neurological deficits including pain, weakness, sensory disturbance, bowel and/or bladder dysfunction. In these circumstances, surgical management of TSC with SCM is indicated. Reoperation for recurrent TCS, particularly in the context of retained or regrown bony septa and prior spinal instrumentation, is technically demanding and carries elevated risk.
ILLUSTRATIVE CASE:

BACKGROUND AND OBJECTIVES: Selective dorsal root rhizotomy (SDR) has been traditionally used to alleviate spasticity in patients with cerebral palsy. Its application to modulate bladder dysfunction in neurogenic spastic bladder, however, remains underexplored. We present single-level conus medullaris approach for sacral SDR as a technically feasible and potentially effective strategy for treating neurogenic bladder in a selected patient with neurogenic spastic bladder.
METHODS: This instrumentation report details the surgical technique of sacral SDR targeting the S2-S5 dorsal rootlets by a mi

Background and Study Aim Facial nerve (FN) weakness as a presenting feature in vestibular schwannoma (VS) is extremely rare. We are presenting two different cases of VS with significant facial weakness and reviewed the literature for similar cases. Methods and Results We are presenting two cases of VS with significant facial weakness. The first case was a 63-year-old male patient presented with 3 weeks' history of severe left-sided facial weakness (House-Brackmann [HB] grade V) and hearing loss. Magnetic resonance imaging (MRI) of the brain showed a tumor in the left internal auditory canal.

OBJECTIVE: Residual lateral spread response (LSR) after seemingly adequate decompression for hemifacial spasms (HFSs) can provide prognostic information about the outcome of microvascular decompression (MVD). In the present study, the main objective was to evaluate the clinical and prognostic impact of residual LSR after adequate MVD for HFS.
METHODS: An observational study was conducted in patients who underwent MVD for HFS at the University of Pittsburgh Medical Center between January 2000 and December 2007. Clinical and neurophysiological outcomes after pre- and post-MVD, including spasm r

The anesthetic management of complex skull base surgical procedures provides unique problems and concerns for the neuroanesthesiologist. Positioning to access the skull base could put the patient at risk for peripheral nerve injury and some of the positions may increase the risk for air emboli. In addition, tumor pathology and involvement with vital structures could increase the chances for substantive blood loss, destruction of associated nerves or vessels, and may require temporary occlusion of the carotid artery necessitating intraoperative neuroprotection. Neurophysiological monitors may a

Multi-modal neurophysiologic monitoring consisting of triggered and spontaneous electromyography and transcranial motor-evoked potentials may detect and prevent both acute and slow developing mechanical and vascular nerve injuries in lateral lumbar interbody fusion (LLIF) surgery. In case report 1, a marked reduction in the transcranial motor-evoked potentials on the operative side alerted to a 28% decrease in mean arterial blood pressure in a 54-year-old woman during an L3-4, L4-5 LLIF. After hemodynamic stability was regained, transcranial motor-evoked potentials returned to baseline and the

BACKGROUND: Hyperhidrosis is caused by sympathetic dysfunction of the central or peripheral nervous system. However, intramedullary spinal cord tumors presenting with hyperhidrosis as an initial symptom have been rarely reported in the literature.
CASE DESCRIPTION: This case involves an 18-year-old man who presented with abnormal enhanced sweating and flushing on the bilateral side of his face and neck that had persisted for 6 years. Magnetic resonance (MR) images revealed that at the C7-T2 levels of the spinal cord, a large intramedullary tumor was involved in the cervicothoracic region. The

INTRODUCTION: Adult tethered cord syndrome (TCS) is uncommon and may be overlooked when chronic lumbosacral pain coexists with subtle occult dysraphism and urinary complaints.
RESEARCH QUESTION: How should an adult patient with back pain, recurrent urinary tract infections, and imaging evidence of a low-lying conus with a lipomatous filum terminale be evaluated and managed?
MATERIAL AND METHODS: We report a 27-year-old woman with progressive low back and left leg pain, recurrent urinary tract infections, and detrusor overactivity. Computed tomography showed L5-S1 spina bifida occulta, and ma

BACKGROUND: Surgical trapping or endovascular deconstruction commonly is used for the treatment of giant or complex intracranial aneurysms. Preoperative balloon test occlusion and cerebral blood flow studies and intraoperative neurophysiologic monitoring can indicate whether sufficient collateralization exists or whether revascularization is needed. Hemodynamic insufficiency can occur, however, despite passing these tests, necessitating posttreatment revascularization.
METHODS: We conducted a retrospective review of patients who underwent surgical or endovascular parent vessel occlusion for t

OBJECTIVE: Stroke remains a potentially devastating complication of aortic arch intervention. The value of neurophysiologic intraoperative monitoring (NIOM) in the early identification of stroke is unclear. We evaluated the utility of NIOM for early stroke detection in aortic arch surgery.
METHODS: Across 8 years at our institution, 365 patients underwent aortic arch surgery with hypothermic circulatory arrest, and 224 cases utilized NIOM. One patient was excluded for intraoperative death. In the remaining cohort, we reviewed the incidence, timing, and location of strokes, and the incidence a

INTRODUCTION: Spinal cord ischemia is a potentially devastating complication of thoracic aortic surgery. However, predictors of outcome have not been well characterized. The study objective was to generate a prognostic score that accurately stratifies patient outcomes, aiding in future management and planning.
METHODS: A retrospective review of 224 consecutive open thoracic aortic surgeries identified patients with spinal cord ischemia, defined as changes on intraoperative somatosensory evoked potentials (SSEP) and/or paraparesis/paraplegia postoperatively. The outcome of interest was poor ou

BACKGROUND AND OBJECTIVE: Moyamoya disease (MMD) is a progressive cerebrovascular disorder treated primarily with surgical revascularization to prevent stroke recurrence. Despite established surgical techniques, standardized perioperative care protocols remain limited, potentially leading to variable outcomes and perioperative complications. Enhanced Recovery After Surgery (ERAS) protocols have demonstrated improved outcomes across surgical disciplines but have not been systematically applied to cerebral bypass surgery for MMD. The objective was to develop evidence-based ERAS recommendations f

Background and Clinical Significance: Spinal arachnoid cysts are rare lesions that may become symptomatic through progressive spinal cord compression. We present a complex case of a thoracic extradural SAC in a 17-year-old male, managed through a stepwise, multidisciplinary approach. Case Presentation: The patient presented with progressive lower limb weakness, right knee paresthesia, and urinary hesitancy following physical exertion. MRI revealed a large posterior extradural SAC extending from T2-T3 to T8, with associated spinal cord compression. Initial management involved T8 laminectomy and

Objectives Total intravenous anesthesia (TIVA) is used during surgery with intraoperative neurophysiological monitoring. Addition of adjuvant may minimize suppression of potentials by reducing doses of propofol. We studied the effect of addition of ketamine or dexmedetomidine to propofol-fentanyl-based TIVA on corticobulbar motor evoked potential (CoMEP) in patients undergoing posterior fossa surgeries. Materials and Methods Forty-two patients were assigned to three groups ( n = 14 each), Group S-saline, Group D-dexmedetomidine (0.25 μg/kg/h), and Group K-ketamine (0.25 mg/kg/h). Patients r
