Chronic Posttraumatic Nail Reconstruction with an Eponychium Flap: Technique Description and Case Series.

PURPOSE: Fingertip trauma and amputations, when not treated correctly, can evolve unfavorably, leading to pain, loss of strength, and tactile difficulties. Post-traumatic nail changes are not just a cosmetic complaint as the nail, along with its protective function, also helps to stabilize the fingertip, adding to pinching motion. There are techniques that provide the nail complex improvement for the treatment of post-traumatic sequelae, thus enhancing fingertip function and aspect. METHODS: We have used a rectangular, eponychium flap with an indentation in the eponychial fold that increases

Effect of activity-based training versus strengthening exercises on upper extremity functions in Parkinson's patients; A randomized controlled, single blind, superiority trial.

BACKGROUND: Idiopathic Parkinson's disease is a neurodegenerative disease which is appeared by symptoms such as resting tremor, bradykinesia, rigidity, and postural instability. Such symptoms as tremor, bradykinesia, and rigidity significantly affect patients' upper extremity function. Rehabilitation programs should be implemented to improve the upper extremity functions of patients. OBJECTIVE: Activity-based training would improve dexterity and functional activities to a larger extent than strengthening exercises. This study aimed at evaluating the effect of activity-based training and stren

An alternative technique for stabilisation of the carpometacarpal joint of the thumb after dislocation or subluxation.

We report a new surgical technique for the treatment of traumatic dislocation of the carpometacarpal (CMC) joint of the thumb. This is a tenodesis which uses part of the flexor carpi radialis. Between January 2010 and August 2013, 13 patients with traumatic instability of the CMC joint of the thumb were treated using this technique. The mean time interval between injury and ligament reconstruction was 13 days (0 to 42). The mean age of the patients at surgery was 38 years: all were male. At a mean final follow-up of 26 months (24 to 29), no patient experienced any residual instability. The mea

Outcomes of percutaneous pinning in treatment of distal radius fractures.

Distal radius fractures are among the most common bone fractures all over the world. Close reduction and fixation by percutaneous pinning is a less invasive method comparing with other open surgeries. This study aims at evaluating the functional outcome of this treatment in distal radius fractures. In this prospective study, 50 patients with distal radius fractures underwent percutaneous pinning during a 16 month period in Tabriz Shohada Hospital. Follow-up for 3 months was possible in 48 (96%) patients. Possible complications (such as infection, nonunion, pin loosening and pain), functional o

Grasp pattern variations seen in the scleroderma hand.

OBJECTIVES: Therapists generally describe a person's grasp in terms of normal prehension patterns that have been discussed in the literature. However, the grasp pattern used by a person with a hand impairment may not fit into these patterns. The purpose of this study was to describe, with qualitative classification systems, static grasp patterns used by hands impaired by scleroderma and to determine the relationship between hand impairment and variations in grasp. METHODS: Seventeen subjects with scleroderma were observed grasping a key, a coin, a glass, and a saucepan. Their grasp patterns w

Revision Strategies for Complications and Failure of Trapeziometacarpal Joint Arthroplasty Based on Residual Trapezial Bone Stock.

PURPOSE: The main reasons for revision surgery after primary trapeziometacarpal (TM) joint prosthetic implantation are trapezium loosening and instability. Although secondary trapeziectomy is the gold standard, here we describe alternative surgical procedures that can be integrated in a revision strategy based on residual trapezial bone stock after implant removal. METHODS: This single-center retrospective study analyzed the clinical and radiologic results for 41 patients who underwent surgical TM joint prothesis revision from 2006 to 2021 at our institution. We applied a classification of tr

Therapy for Dupuytren's Disease (II): Collagenase Therapy vs. Limited Fasciectomy-A Long-Term Comparative Study.

BACKGROUND: Dupuytren's disease (DD) is a systemic connective tissue disorder of the palm, predominantly affecting men of Northern European or Caucasian origin over 55. In addition to conventional surgery, Dupuytren's contracture can be treated in a minimally invasive way by injecting bacterial collagenase into the cord. However, studies on the long-term success rate when compared to the gold standard, surgical limited fasciectomy, are limited. METHODS: This monocentric retrospective study examined 35 patients who had been treated with bacterial collagenase for Dupuytren's contracture, conduc

Spontaneous rupture of flexor pollicis longus tendon by tendolipomatosis in proximal phalanx: A case report.

RATIONALE: Spontaneous flexor pollicis longus (FPL) tendon rupture is rarely reported. Although there are several studies investigating spontaneous FPL tendon rupture, the exact etiology of spontaneous rupture is unclear. Here, we present a case of unusual spontaneous FPL tendon rupture due to tendolipomatosis. PATIENT CONCERNS: A 64-year-old right-handed retired male teacher was referred to our clinic with an inability to flex the interphalangeal joint of his left thumb. DIAGNOSIS: Magnetic resonance imaging (MRI) revealed complete FPL tendon rupture at the level of the distal one-third of

Functional disability associated with disease and quality-of-life parameters in Chinese patients with rheumatoid arthritis.

BACKGROUND: As an important outcome measure among rheumatoid arthritis (RA) patients, functional disability may contribute to unemployment, loss of work productivity, and impaired quality of life. However, little is known about the risk factors of functional disability in Chinese RA patients. This study aimed (1) to examine the prevalence of functional disability in Chinese RA patients; (2) to explore factors associated with the health assessment questionnaire-disability index (HAQ-DI). METHODS: A total of 101 RA patients in this cross-sectional study underwent standardized laboratory examina

Donor site morbidity in cross-finger flaps.

As relevant literature is scarce, this study was undertaken to assess the donor site morbidity of cross-finger flaps. It included 23 patients who had undergone reconstruction of a finger defect with a cross-finger flap. Any additional trauma to the donor finger was an exclusion criterion. Split thickness skin grafts were employed for donor site closure in 13 cases, full thickness skin grafts were used in 10 cases. Follow-up time averaged 83 months. Active and passive total range of motion of the donor finger and maximal pinch grip strength in kilopascals were measured. Both parameters were com
