Plasma collected from 6 experimentally induced and 29 clinical cases of gastric dilatation-volvulus was assayed for cardioactive substances utilizing isolated canine papillary muscles. The results were correlated with in vivo isovolumetric indices of myocardial contractility observed during experimental gastric dilatation-volvulus in the dog. Cardioactive substances were not detected during experimental gastric dilatation-volvulus. Two experimental dogs showed a cardiostimulatory response and 4 experimental dogs showed a cardiodepressant response following gastric decompression. Cardioactive s

Between 1960 and 1980, 53 patients with massive incarcerated hiatal hernia were treated surgically. In 24 of the 53 patients, there was an associated organoaxial volvulus. The following symptoms and signs, which are almost peculiar to massive, incarcerated hernias, were observed: postprandial precordial distress in 43 patients, upper gastrointestinal bleeding (manifest or occult) in 24 patients, severe dyspnea in 13 patients, and complete obstruction associated with organoaxial volvulus in 4. In only 1 of the 53 patients was the hernia of the true paraesophageal type with the esophagogastric j

BACKGROUND: Surgical repair of large type III/IV paraesophageal hernia (PEH) is challenging especially in the presence of acute gastric volvulus (AGV). This study compared postoperative complications (POCs) and 30-day mortality between urgent and elective PEH repair.
METHODS: Between 2007 and 2024, a matched case-control study was conducted in a tertiary care hospital in Barcelona, Spain. Patients undergoing PEH repair on an emergency basis or electively were matched (1:1) by age and sex (male/female). Cases were patients with AGV requiring urgent or semielective repair, whereas controls were

BACKGROUND: Operative repair of obstructive gastric volvulus is challenging. In high-operative risk patients with obstructive gastric volvulus, we perform laparoscopic reduction of gastric volvulus and anterior abdominal wall sutured gastropexy. This case series reports our experience with this operation.
METHODS: We reviewed the charts of all patients who presented with obstructive gastric volvulus and underwent laparoscopic gastropexy between 2007 and 2013.
RESULTS: Eleven patients underwent laparoscopic gastropexy. Median age was 83 years (50 to 92). Six patients presented with chronic ob

AIM: To investigate clinical outcomes of patients with chronic gastric volvulus (GV) who were managed conservatively over a 5-year period.
METHODS: A total of 44 consecutive patients with chronic GV, as diagnosed by barium study between October 2002 and July 2008 were investigated. All of these patients received conservative management initially without anatomical correction. Their clinical manifestations, diagnostic work-ups, and clinical outcomes were analyzed. We sought to identify independent risk factors for poor outcome by using the Cox proportional hazards model.
RESULTS: The enrolled

Using dry dog food label information, the hypothesis was tested that the risk of gastric dilatation-volvulus (GDV) increases with an increasing number of soy and cereal ingredients and a decreasing number of animal-protein ingredients among the first four ingredients. A nested case-control study was conducted with 85 GDV cases and 194 controls consuming a single brand and variety of dry food. Neither an increasing number of animal-protein ingredients (P=0.79) nor an increasing number of soy and cereal ingredients (P=0.83) among the first four ingredients significantly influenced GDV risk. The Veterinary Medical Data Base was usd to conduct an epidemiologic study of gastric dilatation and dilatation-volvulus (GDV) to describe changes over time in frequency of canine hospital admissions, to identify risk factors, and to estimate their relative importance. Cases in this case-control study included 1,934 dogs with GDV that were admitted to 12 participating veterinary hospitals from 1980 to 1989. The controls were 3,868 dogs with other diagnoses that were randomly selected from the same hospitals. Frequency of GDV per 1,000 canine hospital admissions ranged from 2.9 to 6.8. The case

This review illustrates the changing paradigms in the understanding of the pathogenesis of pneumatosis intestinalis. Although many theories have been evoked, pragmatically there appear to be four major clinical and diagnostic imaging considerations. The most common and most emergent life-threatening cause of intramural bowel gas is the result of bowel necrosis due to bowel ischemia, infarction, necrotizing enterocolitis, neutropenic colitis, volvulus, and sepsis. In the stomach, intramural gas can be caused by emphysematous gastritis or ingestion of caustic agents. These situations represent s

BACKGROUND: The clinical and plain radiographic differentiation of congenital intrinsic duodenal anomalies (atresia, web, stenosis) from intestinal malrotation is not always clear. Although sonography has been documented as an important diagnostic tool in the differentiation of these two entities, its role is still not widely appreciated and it is still not universally utilized in this clinical setting.
OBJECTIVE: To assess the usefulness of sonographic features of the duodenal and gastric wall in the differentiation of congenital intrinsic duodenal anomalies from midgut malrotation in a larg

We report the case of a girl who had tetralogy of fallot (TOF) repaired at birth without any associated diaphragmatic hernia. At the age of 2½ years, she experienced an upper gastrointestinal occlusion. At laparoscopy an organoaxial gastric volvulus was observed related to a peritoneal adhesion secondary to pericardial drainage that had been performed at the time of the TOF repair. After reduction of the volvulus, a phrenofundopexy was done. Postoperatively, the child has remained asymptomatic with a follow-up of 24 months. There are few cases of pericardial drainage complications documented i

BACKGROUND/AIMS: Gastric volvulus is a rare, potentially life-threatening condition, which is difficult to diagnose. This study represents a series of patients with acute gastric volvulus.
METHODS: All patients presenting with acute gastric volvulus over a 10-year period were reviewed.
RESULTS: Twenty-one patients with a median age of 66 years were identified. Acute gastric volvulus was secondary to a paraesophageal hiatus hernia in 16 patients. The major symptoms were abdominal pain, vomiting and upper gastrointestinal bleeding/anemia. The most useful investigations were barium studies and

A prospective study evaluating preoperative prognostic indicators in 80 cattle with abomasal volvulus was done. Surgical correction of the abomasal volvulus was performed in all animals. After surgery, cattle were categorized into 3 groups: productive (acceptable milk production or appetite, n = 59), salvaged (poor milk production or appetite, n = 10), and nonsurvivors (n = 11). Mean values for study variables did not differ significantly between salvaged and nonsurvivor groups. Cattle in these 2 groups were combined to form a nonproductive group, which was compared with the productive group.

Gastric volvulus is an extremely rare disorder in the pediatric age group. It is a complex entity as to the etiology and management. The study was conducted to evaluate etiology, mode of presentation, management, and outcome in our patients with gastric volvulus. It was a retrospective study of 10 patients with gastric volvulus managed during May 2006 and October 2010. The medical record of these patients including history, clinical examination, investigations, operative notes, and the outcome was reviewed. Of the ten patients, seven were males and three females. The mean age of presentation w

BACKGROUND: Hepatic portal venous gas, which may occur in conjunction with severe intra-abdominal infections, is associated with a high mortality rate and is considered as an alarm signal.
HISTORY: CT diagnostics revealed hepatic portal venous gas in a 65-year-old patient with advanced perforated sigmoid diverticulitis. This phenomenon was also observed in a 75-year-old patient in association with an upside-down stomach incarceration. Both patients underwent emergency surgery and survived this life-threatening disease.
CONCLUSION: Since hepatic portal gas is usually caused by severe intra-ab

Background: Surgery is the mainstay of treatment for gastric volvulus. Despite its rarity, early experience from recent publications suggests that laparoscopy is a safe and effective approach for the treatment of acute gastric volvulus. Yet, more data focusing on patients' postoperative quality of life (QoL) is needed. The aim of this study is to report our institutional experience with the management of acute gastric volvulus, assessing surgical outcomes and postoperative QoL. Materials and Methods: We performed a retrospective review of a prospectively maintained database, looking for patien
