A proposed roadmap for inpatient neurology quality indicators.

BACKGROUND/PURPOSE: In recent years, there has been increasing pressure to measure and report quality in health care. However, there has been little focus on quality measurement in the field of neurology for conditions other than stroke and transient ischemic attack. As the number of evidence-based treatments for neurological conditions grows, so will the demand to measure the quality of care delivered. The purpose of this study was to review essential components of hospital performance measures for neurological disease and propose potential quality indicators for commonly encountered inpatien

Management of the patient with chronic dizziness.

In this review we present a pragmatic approach to the patient with chronic vestibular symptoms. Even in the chronic patient a retrospective diagnosis should be attempted, in order to establish how the patient reached the current situation. Simple questions are likely to establish if the chronic dizzy symptoms started as benign paroxysmal positional vertigo (BPPV), vestibular neuritis, vestibular migraine, Meniere's disease or as a brainstem stroke. Then it is important to establish if the original symptoms are still present, in which case they need to be treated (e.g. repositioning maenouvres

A mathematical model for top-shelf vertigo: the role of sedimenting otoconia in BPPV.

Benign paroxysmal positional vertigo (BPPV) is a mechanical disorder of the vestibular system in which calcite particles called otoconia interfere with the mechanical functioning of the fluid-filled semicircular canals normally used to sense rotation. Using hydrodynamic models, we examine the two mechanisms proposed by the medical community for BPPV: cupulolithiasis, in which otoconia attach directly to the cupula (a sensory membrane), and canalithiasis, in which otoconia settle through the canals and exert a fluid pressure across the cupula. We utilize known hydrodynamic calculations and make

Intersession Variations in Frequency Tuning of Ocular Vestibular Evoked Myogenic Potentials in Healthy Individuals.

Objective Frequency tuning of ocular vestibular evoked myogenic potential (oVEMP), which is the stimulus frequency corresponding to the largest oVEMP amplitude, has been used to diagnose cases with Ménière's disease and differentiate them from cases with benign paroxysmal positional vertigo with a high degree of sensitivity and specificity. However, this measure of oVEMP is carved out of the peak-to-peak amplitude of oVEMP, and studies on test-retest reliability of amplitude of oVEMP have shown moderate-to-excellent reliability of amplitude. This would theoretically render the frequency tuning

Psychiatric morbidity and comorbidity in different vestibular vertigo syndromes. Results of a prospective longitudinal study over one year.

OBJECTIVES: High rates of coexisting vestibular deficits and psychiatric disorders have been reported in patients with vertigo. Hence, a causal linkage between the vestibular system and emotion processing systems has been postulated. The aim of this study was to evaluate the impact of vestibular function and vestibular deficits as well as preexisting psychiatric pathologies on the course of vestibular vertigo syndromes over 1 year. METHODS: This interdisciplinary prospective longitudinal study included a total of 68 patients with vestibular vertigo syndromes. Four subgroups were compared: ben

Benign paroxysmal vertigo in childhood. Diagnostic significance of vestibular examination and headache provocation tests.

Sixteen children with benign paroxysmal vertigo (BPV) are presented. The great majority had a family history of migraine, neurological and autonomic signs associated with vertiginous attacks, and headache or other sign of the periodic syndrome (motion sickness, cyclic vomiting, abdominal pain) unrelated to the attacks. Vestibular examination, including bithermal caloric and rotational testing with ENG recording, showed normal or transiently decreased vestibular function. Headache provocation tests with nitroglycerin, histamine and fenfluramine were positive in 9 of the 13 patients examined, an

Comprehensive Review of Nystagmus and Vertigo Diagnostics: From Pathological Foundations to AI-Driven Telemedicine.

Nystagmus, the involuntary rhythmic oscillation of the eyes, is a critical diagnostic marker in vestibular medicine, distinguishing life-threatening central disorders such as stroke from benign peripheral conditions including Benign Paroxysmal Positional Vertigo (BPPV). Despite its clinical importance, accurate nystagmus assessment has long been constrained by expensive infrared video-oculography equipment such as videonystagmography, specialist dependency, and the episodic nature of vestibular symptoms that are often resolved before a clinical encounter. This review synthesizes approximately

The vestibular system: Unsteadiness, dizziness, and vertigo.

This chapter provides a detailed overview of the vestibular system's anatomy, physiology, and clinical relevance, emphasizing how inner-ear sensory structures and brainstem pathways interact to maintain balance, gaze stability, and spatial orientation. It begins with the peripheral vestibular apparatus, describing the semicircular canals and otolith organs in detecting angular and linear accelerations. These signals are transmitted via the vestibulocochlear nerve to the vestibular nuclei at the pontomedullary junction, where they integrate with visual and somatosensory inputs to drive coordina
