Cardiopulmonary bypass time, intraoperative hematocrit and transfusion predict neurocognitive decline in patients undergoing cardiac surgery.

BACKGROUND: Neurocognitive decline (NCD) after cardiopulmonary bypass is a well-established phenomenon. While numerous preoperative risk factors have been identified, the influence of intraoperative factors on NCD still needs further exploration. METHODS: We conducted a prospective cohort study to evaluate NCD and associated pre-operative and intra-operative factors. Patients undergoing CABG or valvular surgeries under cardiopulmonary bypass from 2021 to 2023 were recruited. Preoperative characteristics and postop outcomes were extracted from chart review. Intraoperative variables were extrac

Prevalence of Abnormal Thyroid-Stimulating Hormone (TSH) Levels and Their Association With Iron Status in Pregnancy: A Retrospective Cross-Sectional Study.

Background Thyroid hormones play a vital role in maternal metabolism and fetal neurodevelopment. Pregnancy causes physiological changes in thyroid function, particularly during the early trimesters, when rising human chorionic gonadotropin levels temporarily lower thyroid-stimulating hormone (TSH) levels before increasing later in pregnancy. Iron deficiency anemia is the most common hematologic disorder during pregnancy, mainly due to hemodilution, poor dietary intake, and short intervals between pregnancies. Because iron is required for thyroid peroxidase activity, its deficiency may lead to

Excessive postoperative bleeding and outcomes in neonates undergoing cardiopulmonary bypass.

BACKGROUND: Neonates undergoing cardiac surgery are especially prone to the hemostatic alterations of cardiopulmonary bypass (CPB) and are at high risk for post-CPB bleeding. An immature coagulation system, significant hemodilution from the CPB prime, long CPB times at low temperatures, and extensive suture lines increase neonates' susceptibility to bleeding after CPB. In this study, we examined the relationship between excessive bleeding in neonates after CPB and major postoperative adverse events. METHODS: We retrospectively reviewed the medical records of 169 neonates who underwent complex

Acid-base and bio-energetics during balanced versus unbalanced normovolaemic haemodilution.

Fluids balanced to avoid acid-base disturbances may be preferable to saline, which causes metabolic acidosis in high volume. We evaluated acid-base and bio-energetic effects of haemodilution with a crystalloid balanced on physical chemical principles, versus crystalloids causing metabolic acidosis or metabolic alkalosis. Anaesthetised, mechanically ventilated Sprague-Dawley rats (n=32, allocated to four groups) underwent six exchanges of 9 ml crystalloid for 3 ml blood. Exchange was with one of three crystalloids with strong ion difference (SID) values of 0, 24 (balanced) and 40 mEq/l. Control

Artificial colloids impair haemostasis. An in vitro study using thromboelastometry coagulation analysis.

BACKGROUND: Hydroxyethyl starch (HES) solutions impair haemostatic mechanisms. The impact of the degree of substitution (DS) of a HES solution on thromboelastometry tracings is unclear. Therefore we tested the hypothesis of whether the DS has an effect on the haemostatic defect caused by HES, and assessed whole blood coagulation by thromboelastometry coagulation analysis (ROTEM, Pentapharm Co., Munich, Germany) in serial in vitro haemodilutions of colloids. METHODS: Whole blood was withdrawn from 12 volunteers in a crossover study. Six per cent low-molecular weight HES with a high (HES MW 120

Effects of fluids on sublingual microcirculation: a point of view review.

BACKGROUND: Fluids are a key component of shock resuscitation. Nevertheless, their microvascular effects are complex. In fluid responsive patients, fluids may increase tissue perfusion because of the increase in cardiac output. Since shock states are characterized by a partial loss of the coherence between systemic hemodynamics and microcirculation, the increase in cardiac output does not guarantee improvements in tissue perfusion. Furthermore, the administration of fluids carries risks of hemodilution and tissue edema that can dampen microcirculation. Regarding this, colloid solutions have so

Pharmacokinetics and Pharmacodynamics of Analgesic and Anesthetic Drugs in Patients During Cardiac Surgery With Cardiopulmonary Bypass: A Narrative Review.

Cardiopulmonary bypass (CPB) impacts pharmacokinetics and -dynamics of drugs used during cardiac surgery. These alterations can lead to changes in drug efficacy resulting in under- or overdosing. This review summarizes current knowledge on the effects of CPB on commonly used intraoperative and continuously administered anesthetics and analgesics. Out of 197 articles initially identified, 22 were included in the final review. The breakdown of studies by main topic was as follows: propofol (9 articles), sevoflurane (4), remifentanil (3), isoflurane (2), fentanyl (2), and sufentanil (2), and alfe

Factors Influencing Neurodevelopment after Cardiac Surgery during Infancy.

Short- and long-term neurodevelopmental (ND) disabilities with negative impact on psychosocial and academic performance, quality of life, and independence in adulthood are known to be the most common sequelae for surviving children after surgery for congenital heart disease (CHD). This article reviews influences and risk factors for ND impairment. For a long time, the search for independent risk factors was focused on the perioperative period and modalities of cardiopulmonary bypass (CPB). CPB operations to ensure intraoperative vital organ perfusion and oxygen supply with or without circulato

Anaerobic metabolism associated with traumatic hemorrhagic shock monitored by microdialysis of muscle tissue is dependent on the levels of hemoglobin and central venous oxygen saturation: a prospective, observational study.

BACKGROUND: Traumatic hemorrhagic shock resulting in tissue hypoxia is a significant cause of morbidity and mortality in polytraumatized patients. Early identification of tissue hypoxia is possible with microdialysis. The aim of this study was to determine the correlation between a marker of tissue hypoxia (L/P; lactate to pyruvate ratio) and selected parameters of systemic oxygen delivery (Hb; hemoglobin) and oxygen extraction (ScvO2; central venous oxygen saturation). We also investigated the severity of tissue hypoxia over the course of care. METHODS: Adult patients with traumatic hemorrha

Analysis of immunohistochemical markers in bone marrow sections to evaluate for myelodysplastic syndromes and acute myeloid leukemias.

BACKGROUND: Accurate bone marrow (BM) blast counts (BCs) are essential for diagnosis (dx) of myelodysplasia (MDS), MDS/myeloproliferative (MDS/MPD) disease, or acute myeloid leukemia (AML), and may be difficult in hemodiluted bone marrow aspirates (BMAs). Erythroid precursors (EPs) may be indistinguishable from myeloblasts in BM sections (aspirate clots/cores). We compare the usefulness of immunohistochemistry (IHC) [ie, CD34, CD117, myeloperoxidase (MPO), Hemoglobin A1 (HbA1), and terminal deoxynucleotidyl transferase (TdT)] of BM sections (IHC-BM) with BMA, bone marrow touch preparation (BMT

Extracorporeal circulation as an additional method for cerebral protection in simultaneous carotid endarterectomy and coronary artery surgical revascularization.

Surgical management of patients with concomitant carotid and coronary artery stenosis remains controversial. Our policy was always to perform at the same time carotid endarterectomy (CE) and coronary artery bypass grafting (CABG), but it was also considered that extracorporeal circulation (ECC), because of full heparinization, hemodilution, pulsatile flow, and hypothermia could provide better cerebral protection during CE. Retrospective data of 124 patients undergoing simultaneous CE and CABGs between January 1994 and December 2001 were reviewed. CE was performed prior to ECC in 65 patients (G

Impact of polynitroxylated albumin (PNA) and tempol on ischemia/reperfusion injury: intravital microscopic study in the dorsal skinfold chamber of the Syrian golden hamster.

Nitric oxide-releasing drugs have been shown to reduce ischemia/reperfusion (I/R) injury by acting as radical scavengers. However, their therapeutic application is hampered by specific side effects and rapid bioreduction in vivo. The half-life and antioxidant activity of nitroxides may be enhanced by their covalent binding to human serum albumin, resulting in polynitroxyl albumin (PNA). Thus, PNA may represent a novel antioxidative drug. The objectives of this study were to elucidate 1) whether PNA is able to diminish I/R injury; 2) the most effective dose of PNA in vivo; and 3) whether the ad

Indications for and complications of transfusion and the management of gynecologic malignancies.

Anemia, which is highly prevalent in oncology patients, is one of the most established negative prognostic factors for several gynecologic malignancies. Multiple factors can cause or contribute to the development of anemia in patients with gynecologic cancers; these factors include blood loss (during surgery or directly from the tumor), renal impairment (caused by platinum-based chemotherapy), and marrow dysfunction (from metastases, chemotherapy, and/or radiation therapy). Several peri- and intra-operative strategies can be used to optimize patient management and minimize blood loss related t

An observational study on the spectrum of heat-related illness, with a proposal on classification.

During operations in subtropical areas over the summer months of 2001 and 2003 the authors audited 80 patients with heat-related illness, with the intention of defining the nature and distribution of the underlying pathophysiology. Haematological, biochemical and clinical data were gathered prospectively and patients allocated to diagnostic categories on the basis of the combination of clinical findings and investigations. Four basic types of heat-related illness could be distinguished: (1) excessive salt loss with hyponatraemic dehydration, (2) hypokalaemic alkalosis with low serum bicarbonat

[Haemodynamic effects following preoperative hypervolemic haemodilution with hypertonic hyperoncotic colloid solutions in coronary artery bypass graft surgery].

OBJECTIVE: Using hyperoncotic colloids as volume replacement to provide haemodynamic stability appears to be a suitable approach to diminish fluid overload and subsequent interstitial edema during cardiac surgery. The aim of the present study was to investigate for the first time the haemodynamic effects following preoperative haemodilution with different hypertonic hyperoncotic colloid solutions in patients undergoing coronary artery bypass grafting. METHODS: A total of 43 patients with normal left ventricular ejection fraction, undergoing elective coronary artery bypass grafting received pr
