Distinguishing features of focal cemento-osseous dysplasias and cemento-ossifying fibromas: I. A pathologic spectrum of 316 cases.

Focally expressed cemento-osseous dysplasia (periapical cemento-osseous dysplasia and focal cemento-osseous dysplasia) and cemento-ossifying fibroma (ossifying fibroma and cementifying fibroma) are two clinically recognized entities that are not easily differentiated histopathologically because of the lack of recognition of specific microscopic features. We have assessed 20 pathologic parameters for their ability to distinguish reliably between the two. Included in this study were 241 cases of focally expressed cemento-osseous dysplasia and 75 cases of cemento-osseous fibroma diagnosed from a

A comparative analysis of periapical radiography and cone-beam computerized tomography for the evaluation of endodontic obturation length.

OBJECTIVE: The aim of this study was to determine the consistency and inconsistency between the periapical radiography (PR) and cone-beam computerized tomography (CBCT) in evaluating the length of root canal obturations (RCOs) in vivo. STUDY DESIGN: Thirty-six maxillary and mandibular first and second molars yielding 109 obturated root canals with available PR and CBCT images were analyzed. The inclusion criterion was that the RCO extended 0-2 mm short of the radiographic apex on PR images. Teeth having root canal calcification, apical resorption, or poor quality PR/CBCT images were excluded.

Ciliated epithelium-lined radicular cysts.

OBJECTIVE: This report describes 3 cases of ciliated epithelium-lined radicular cysts among 256 apical periodontitis lesions and also illustrates the occurrence of an Actinomyces-infected periapical cyst. STUDY DESIGN: Serial and step serial sections of 256 plastic-embedded root apices with attached apical periodontitis lesions that were prepared for a previous investigation were reviewed for the presence of ciliated epithelium-lined radicular cysts. The lesions that were found to have such epithelial lining were examined in a transmission electron microscope to elaborate the fine structure o

Detection of external root resorption in periapical radiographs using YOLO-based deep learning model.

OBJECTIVES: External root resorption is a destructive process that usually develops without any symptoms and, when diagnosed, can lead to tooth extraction because it causes serious tooth tissue loss. Therefore, it is aimed to develop artificial intelligence algorithms that can assist in the diagnosis of external root resorption. METHODS: Totally, 110 extracted teeth were demineralized by applying 40% nitric acid solution for 8 hours, 8% sodium hypochlorite for 10 minutes, and then a distilled water washing procedure. The prepared teeth were placed on a radioconjugate phantom model and imaged.

[Expert consensus on perioperative management of tooth extraction in patients receiving denosumab therapy].

Denosumab is a humanized monoclonal antibody targeting receptor activator of nuclear factor-κB ligand (RANKL) and is commonly used in the treatment of osteoporosis and cancer-related bone metastases. However, the persistent use of denosumab has been associated with an increasing incidence of denosumab-related osteonecrosis of the jaw (DRONJ), particularly following tooth extraction. This expert consensus aims to develop clinical management guidelines for the perioperative period of tooth extraction in patients who are currently receiving or have previously received denosumab therapy. The conse

Digital workflow with intraoral scanning and CAD/CAM PMMA crowns in a non-cooperative pediatric patient with early childhood caries: a case report.

Early childhood caries can cause extensive coronal destruction in preschool children, often requiring complex rehabilitation at an age when cooperation is limited. This case report describes a behavior-adapted digital workflow used to complete full-mouth rehabilitation in a four-year-old boy with severe early childhood caries and very limited cooperation. Clinical and radiographic assessment showed advanced multi-tooth carious involvement, with marked destruction of the maxillary anterior segment. Differential diagnostic reasoning supported severe early childhood caries as the most consistent

Comparative Study of Crestal Bone Loss Around Implants Placed Using Flapless Versus Flapped Technique.

BACKGROUND: Crestal bone loss is a critical determinant of long-term implant success. The choice of surgical technique-flapless versus flapped-may influence peri-implant bone preservation. OBJECTIVE: To compare the crestal bone loss around dental implants placed using the flapless technique versus the conventional flapped technique over a 6-month follow-up period. METHODS: A prospective, randomized clinical trial was conducted involving 40 systemically healthy patients requiring single-tooth implants in the posterior mandible. Participants were randomly assigned to two groups: Group A (flapl

Sex-specific nociceptor modulation of the apical periodontitis transcriptome.

Introduction: Apical periodontitis (AP) is a painful disease that develops quickly following dental infections and is primarily characterized by robust inflammation surrounding the tissues of the affected tooth, resulting in disruption of bone homeostasis and periradicular bone loss. Moreover, there are distinct clinical presentations, symptoms, and responses to AP treatment between male and female subjects, creating a desperate need to further understand the sex-specific mechanisms of AP. Methods: With the growing evidence that nociceptors modulate AP development, we utilized RNA sequencing i

Analysis of clinical and radiographic outcomes at least 5 years after coronectomy for deeply impacted mandibular third molars.

OBJECTIVE: Coronectomy is an alternative to traditional mandibular third molar extraction when the inferior alveolar nerve (IAN) is closely associated with the roots of these molars. Given the paucity of research on the long-term outcomes after coronectomy, we conducted this study to evaluate clinical and radiographic outcomes at least 5 years post-coronectomy. METHODS: This retrospective study enrolled all patients with impacted mandibular third molars treated with coronectomy at our institution who were willing to return for follow-up evaluation 5 years or more after their original procedur

Biologic and pathologic aspects of osteocytes in the setting of medication-related osteonecrosis of the jaw (MRONJ).

Medication-related osteonecrosis of the jaw (MRONJ) is a potentially severe, debilitating condition affecting patients with cancer and patients with osteoporosis who have been treated with powerful antiresorptives (pARs) or angiogenesis inhibitors (AgIs). Oral risk factors associated with the development of MRONJ include tooth extraction and inflammatory dental disease (e.g., periodontitis, periapical infection). In bone tissues, osteocytes play a bidirectional role in which they not only act as the "receiver" of systemic signals from blood vessels, such as hormones and drugs, or local signals

Mini-invasive Implant Placement in Combination with Maxillary Sinus Membrane Perforation During Transcrestal Sinus Floor Elevation: A Retrospective Study.

The aim of this retrospective study was to report preliminary outcomes of a modified technique for transcrestal sinus floor elevation with simultaneous implant placement. A total of 165 implants were placed in 110 patients using a modified Summers technique. During implant site preparation, after fracturing the sinus floor, a small perforation of the membrane was made using the first osteotome. After grafting with anorganic bovine bone mixed with venous blood, standard-length implants were inserted. The prosthetic phase occurred after 4 to 5 months. Patients were followed for at least 2 years

Inflammatory suppression by endodontic sealers after aging 12 weeks In vitro.

Dental endodontic sealers are in intimate contact with tissues around the root apex (periapical area) for extended periods. New endodontic sealers have been developed in the past decade, but the biological responses to many new products are not well documented. In this study, we assessed in vitro monocytic cytotoxic and inflammatory responses to several contemporary endodontic sealers. AH-Plus (AH), Pulp Canal Sealer (PC), Epiphany (EPH), Endo-Rez (ER), and an experimental Endo-Rez (ERx) were initially placed in buffered-saline for 12 weeks to simulate in vivo use. After "aging," specimens wer

On the local applications of antibiotics and antibiotic-based agents in endodontics and dental traumatology.

Antibiotics are a valuable adjunctive to the armamentarium available to health professionals for the management of bacterial infections. During endodontic treatment and when managing trauma to the teeth, antibiotics may be applied systemically (orally and/or parenterally) or locally (i.e. intra-dentally via irrigants and medicaments). Due to the potential risk of adverse effects following systemic application, and the ineffectiveness of systemic antibiotics in necrotic pulpless teeth and the periradicular tissues, the local application of antibiotics may be a more effective mode for delivery i

Reactive pocket epithelium in untreated chronic periodontal disease: possible derivation from developmental remnants of the enamel organ and root sheath.

The pathological lining epithelium of destructive periodontitis was studied by analysis of the expression of intermediate filament proteins in biopsies of untreated advanced periodontitis. The cytokeratin (CK) pair 8/18 characteristic of simple epithelia was expressed consistently in a distribution pattern confined to the reactive pocket epithelium. The pattern of CK8/18 expression was complex with two broad presentations evident. In two-thirds of the advanced disease biopsies, the entire pathological lining epithelium was strongly reactive for both CK8 and CK18. In the remainder, the more sup

From Caries to Periodontal Breakdown: A Biological and Clinical Continuum Linking Cariology, Operative Dentistry, Endodontics, and Periodontology.

Dental diseases have long been taught and treated as separate entities: cariology, operative dentistry, endodontics, and periodontology, each working within its own boundaries. However, increasing biological and clinical evidence suggests that this classified view does not fully reflect how disease progresses in the mouth. Instead, dental disease should be understood as a continuum within the interconnected tooth-pulp-periodontium complex. This review provides current evidence showing how dental caries can serve as the starting point of a process that can progress through pulpitis and apical p

Nonvital Pulp Therapy in Primary Teeth Using Mineral Trioxide Aggregate Obturation: A Retrospective Case Series.

Nonvital pulp therapy is a treatment option that can preserve pathologically affected primary teeth and contribute to space maintenance. This retrospective case series examined the healing effects in infected primary teeth after pulpectomy (PE) using mineral trioxide aggregate (MTA) obturation and eruption characteristics of the succedaneous teeth. Twenty-one teeth in 17 patients (mean age: 7 years 1 month) were selected for PE using MTA and diagnosed with pulp necrosis, periapical/furcation radiolucencies (67%, n = 14), resorptive defects (62%, n = 13), failing previous PEs (33%, n = 7), pulp

Management of an Open Apex Using a Platelet-Rich Fibrin Membrane as an Internal Matrix and Mineral Trioxide Aggregate as an Apical Barrier: A Case Report.

Apexification is a crucial procedure for achieving apical healing in non-vital teeth with open apices. Traditionally, calcium hydroxide has been used for this purpose, but it has significant drawbacks, including prolonged treatment duration, increased risk of root fracture, and the potential for porous barrier formation. Mineral trioxide aggregate (MTA) has emerged as a superior alternative due to its biocompatibility, faster setting time, and better sealing properties. However, MTA extrusion into periradicular tissues can cause persistent discomfort. The use of an internal matrix such as plat
