Perioperative neurocognitive disorders (PND) remain difficult to prevent because conventional cognitive assessments may fail to capture brain vulnerability in patients who appear cognitively normal before anesthesia and surgery. This narrative review examines the potential role of objective perioperative neuromonitoring markers as complements to cognitive scales, focusing on electroencephalography (EEG), near-infrared spectroscopy (NIRS)/cerebral oximetry, and processed EEG/bispectral index (BIS). Evidence from perioperative studies and related neurophysiological literature suggests that reduc

Background: Congenital thoracic kyphoscoliosis caused by vertebral malformations is a challenging condition that may progress during skeletal growth, leading to spinal imbalance, pain, cosmetic deformity, and neurological compromise. Conventional correction often requires extensive anterior, posterior, or combined approaches associated with substantial surgical morbidity. We describe a single-position surgical technique combining a lateral retropleural asymmetric vertebral resection with percutaneous posterior instrumentation performed entirely in the lateral decubitus position. Methods: A 15-

Jugular foramen dural arteriovenous fistulas are exceedingly rare vascular abnormalities, with only 10 cases documented so far. Their complex angioarchitecture and the risk of postoperative complications necessitate carefully planned treatment strategies. We present the case of an 82-year-old man who presented with transient dizziness. Digital subtraction angiography revealed a Cognard type V dural arteriovenous fistula at the right jugular foramen, fed by the neuromeningeal branch of the right ascending pharyngeal artery and draining into the perimedullary veins of the brainstem and spinal co

OBJECTIVE: Extradural spinal nerve sheath tumors with foraminal involvement are rare, and the gold standard for surgical resection is not established yet. Traditionally, lumbar intra-extraforaminal schwannomas have been resected through a posterior midline or paraspinal open approach. The lateral transpsoas approach represents a suitable, minimally invasive approach which can provide direct access to the extraforaminal and lumbar plexus region avoiding bone removal, hence the necessity of lumbar fusion. The aim of this study is to share our experience of extraforaminal lumbar schwannomas appro

OBJECTIVE: To evaluate the predictive value of intraoperative neuromonitoring (IONM) in brainstem cavernous malformation (BSCM) surgery.
METHODS: Surgically treated patients with BSCM were included. All patients received IONM consisting of motor-evoked potentials (MEPs) and somatosensory-evoked potentials (SSEPs). Neurologic examination was conducted preoperatively and at discharge and follow-up >3 months after BSCM removal. Demographic, radiographic, and clinical features were assessed. Study end points were new motor or somatosensory deficits and functional disability.
RESULTS: A total of

The purpose of this study was to evaluate the application of multimodal intraoperative monitoring (MIOM) system in patients with congenital scoliosis (CS) and adolescent idiopathic scoliosis (AIS).Twelve patients who underwent posterior surgical correction of scoliosis for CS and AIS from June 2014 to July 2018 were enrolled in this study. During the operation, we monitored the functional status of the spinal cord by MIOM. An abnormal somatosensory evoked potential was defined as a prolonged latency of more than 10% or a peak-to-peak amplitude decline of more than 50% when compared to baseline

BACKGROUND: This study investigated whether visual localization of the external branch of the superior laryngeal nerve (EBSLN) coincides with its localization via intraoperative neuro-monitoring (IONM) during thyroidectomy and whether its use influences the frequency of injuries.
METHODS: A prospective, comparative, cross-sectional, observational study was performed in 240 superior thyroid poles. The metrics were visual identification of the EBSLN and its corroboration with IONM. The frequency of EBSLN injuries was also determined. Statistical analysis was achieved via kappa and chi-square te

OBJECTIVE: Postoperative CSF leakage is the most common unwanted sequela of transnasal pituitary surgery. The individual anatomy, the extent of the sellar opening, and the occurrence of an intraoperative CSF leak add to the risk of postoperative rhinorrhea. Despite the current sophistication and recent developments in pituitary surgery, watertight closure of the sellar floor remains a matter of concern. Improvements and additions to the technical armamentarium of sellar reconstruction are therefore still desirable.
METHODS: The authors present a closure technique of the sellar floor using a b

Monitoring of the neurocritical in the perioperatory is in constant evolution. There are essentially two ultrasonographic application of neuromonitoring: the diameter of the sheath of the optic nerve and transcranial Doppler. Ultrasound-guided neuromonitoring can detect stenosis or occlusion of intracranial arteries, monitor the evolution of patients with vasospasm after subarachnoid hemorrhage, detect cerebral embolism, evaluate the cerebral collateral system, determine brain death, calculate indirectly Intracranial pressure and cerebral perfusion and helps in clinical decisions and early the

Transcranial electrical stimulation motor-evoked potential (TES-MEP) has been widely used to monitor major motor pathways in cranial and spinal surgeries. However, the results of TES-MEP might be strongly influenced by anesthetic agents and muscle relaxants. To compensate for this effect, a technique using compound muscle action potentials of the abductor pollicis brevis (APB-CMAP) evoked by median nerve stimulation has recently been reported. In this article, we adopted the transcranial electrical stimulation motor-evoked potential of facial muscles (TES-FMEP) instead of APB-CMAP as a referen

OBJECTIVES/HYPOTHESIS: The nonrecurrent laryngeal nerve (NRLN) is a common anatomic variation of the right recurrent laryngeal nerve (RLN), which has been associated with an increased risk of injury during thyroid and parathyroid surgery. We suggest that early successful identification using intraoperative nerve monitoring (IONM) and preservation of this variant will help the surgeon to avoid injury to this nerve. Our objective was to examine the electrophysiological parameters of the NRLN and the efficacy of IONM for successful identification of the NRLN.

Paraplegia remains the most devastating complication of open and endovascular thoracic/thoracoabdominal aortic aneurysm (TAA/A) repair. However, the assessment of currently available neuromonitoring modalities remains challenging and difficult to interpret. Near-infrared spectroscopy (NIRS) has been introduced as a strategy for noninvasive, real-time monitoring of the paraspinous collateral network (CN) to detect potential spinal cord ischemia at our institution. Prior to TAA/A repair, a cerebrospinal fluid catheter is placed and four NIRS optodes are bilaterally positioned on the patient's ba

BACKGROUND: Physiological responses to nociception are complex and involve intricate associations between the central, peripheral, and autonomic nervous systems. To optimize intraoperative analgesic titration, several monitoring devices have been developed, each targeting specific physiologic variables. However, existing devices primarily focus on isolated components of the nociceptive response, such as autonomic or cortical activity, without integrating these perspectives comprehensively. Our aim was to compare the performance of different nociception monitors in response to standardized teta

This study aimed to determine whether absolute baseline transcranial motor evoked potential (Tc-MEP) amplitude is associated with false-positive (FP) alerts during intraoperative neurophysiological monitoring and to estimate pathology-specific cutoff values. We retrospectively reviewed 307 high-risk spine surgery cases monitored with Tc-MEPs from 2018 to 2020. For waveform-level analysis, 1,748 muscle waveforms from 287 FP/true-negative cases were analyzed. Alerts were defined as final amplitude ≤ 30% of baseline, and FP alerts were defined as alerts without new postoperative motor deficit. Re

Penetrating neck impalement injuries in children are rare but potentially life-threatening. We present two cases to illustrate the clinical spectrum of this injury pattern and reinforce key management principles. Case 1 was a 13-year-old female patient who sustained a wooden stick impalement to the right neck during a sledding accident. She was hemodynamically stable with a Glasgow Coma Scale (GCS) score of 15 and no neurological deficits. Computed tomography (CT) angiography showed a 2.5 cm soft tissue laceration with no vascular or aerodigestive injury. Case 2 was a 17-year-old female patien

BACKGROUND: Anterior petrous meningiomas pose substantial surgical challenges because of their close association with multiple cranial nerves (CNs) and venous structures. In this case, we demonstrate the retrosigmoid transpetrosal fissure approach for resection of an anterior petrous meningioma.
CASE DESCRIPTION: A 68-year-old man presented with left-sided facial pain and diplopia due to trigeminal and abducens nerve palsy. Preoperative imaging demonstrated a tumor arising from the anterior petrous surface, with slight extension into the Meckel cave and dorsocaudal displacement of the facial
