UNLABELLED: We report the outcomes of scapholunate intercarpal ligamentoplasty in 26 wrists. For 15 wrists with static instability and 11 with dynamic instability, we used a free palmaris longus graft to reconstruct the dorsal part of the scapholunate interosseous ligament and the dorsal intercarpal ligament. These patients were evaluated for pain and active wrist range of motion, grip strength and radiological appearance after a mean follow-up of 36 months (range 12-54) after surgery. Pain score was improved from 4.5 to 1.4 at rest and 6.7 to 1.9 during hand use. The average wrist flexion was

PURPOSE: To examine the contact site of the radiocarpal joint and lengths of carpal ligaments at different forearm rotations in vivo. Rotation of the forearm could exert noteworthy influence on mechanics of the wrist, and understanding how forearm rotation influences wrist mechanics may help treat carpal disorders because wrist position closely relates to forearm rotation.
METHODS: We obtained computed tomography scans of the wrists of 8 volunteers at the following 7 positions of forearm rotation: neutral; 30°, 60°, and 90° of pronation; and 30°, 60°, and 90° of supination. Three-dimensional

Fractures of the distal radius occur commonly, and volar plating has emerged as a current, successful, and popular treatment for this injury. When this technology fails, salvage procedures must be selected that aim to maximize wrist motion and minimize pain. We present a case of volar radioscapholunate wrist arthrodesis in the setting of unfeasible intraoperative volar osteotomy to treat a malunited distal radius fracture.

The purpose of this study was to review and report the results of using a new procedure, the dorsal intercarpal ligament capsulodesis (DILC), to treat a group of patients with chronic flexible scapholunate dissociation. This was a prospective study of 21 patients (22 wrists), ranging in age from 16 to 62 years followed up for 1 to 4 years. For this study all patients returned to complete a questionnaire and have a physical examination performed by physicians and therapists independent of the treating surgeons and to obtain standardized radiographs of the wrists. Patient demographics, mechanism

This retrospective case-control study compares arthroscopic surgery (AS) with open reduction and internal fixation (ORIF) in early and late follow-up outcomes for perilunate injuries. Of 74 patients treated between 2012 and 2023, 28 were matched into two groups: 14 AS and 14 ORIF. Baseline characteristics were similar. Arthroscopic surgery had a significantly shorter operative time (mean difference (MD) 71 min, 95% confidence interval (CI) 10 to 132 min, p = 0.021). At early follow-up (mean 4.1, SD 1.3 months), AS patients had significantly higher Mayo Wrist Scores (MWS) with better pain relie

We describe a hand therapy protocol aimed at unloading the wrist and increasing blood supply to the wrist, specifically to the lunate. The protocol was used in a series of patients with clinical radial wrist pain, dysfunction and changes on wrist imaging studies. The patients were not candidates for surgical treatment. Application of the therapy protocol improved objective and subjective parameters such as pain and motion, and may provide a viable treatment option for patients with lunate overload or early Kienbock's disease that are not candidates for surgery.

We report a case of bifocal dislocation of the forearm due to posterior dislocation of the elbow and transcaphocapital retrolunate dislocation of the carpus, Fenton's syndrome, associated with fracture of both bones of the forearm. The emergency procedure achieved reduction of the elbow, plate fixation of the radius and ulna, used a posterior and an anterior access to achieve reduction and osteosynthesis of the radial styloid, the scaphoid, and the capitatum and temporary schapholunate, triquetrolunate, scaphocapitate, and radiolunate pinning. At ten months, the patient only complained of mode

BACKGROUND: Lunotriquetral dissociation with rupture of the lunotriquetral ligament and the radiolunotriquetral and the radiodorsal ligament is usually recognized late. Cinematography, arthrography, magnetic resonance imaging and arthroscopy are the diagnostic procedures. Treatment includes nonoperative methods in the case of incomplete lesions but also closed reposition with temporary LT arthrodesis and open ligament repair. In many cases, definitive LT arthrodesis may become necessary. It is the aim to develop a diagnostic algorithm on the base of retrospective analysed data.

PURPOSE: Perilunate injuries cause severe carpal malalignment. Open reduction and internal fixation of these injuries has become the treatment of choice. This study evaluated clinical outcome and the patients' perception of disability in activities of daily living after open reduction, ligament reconstruction, and/or internal fixation of the scaphoid. In addition, potential prognostic factors for functional outcome and individual perceptions of disability were analyzed and compared with radiologic findings.
METHODS: This study consisted of a retrospective analysis of patients with perilunate

Thirty-nine patients were retrospectively reviewed after a wrist arthrodesis using a Synthes wrist fusion plate and iliac crest bone graft. Information was obtained from review of patient files, a questionnaire to assess pain, function and work status, and clinical assessment of grip strength, forearm rotation and fingers motion. All wrist fusions united except that the index carpometacarpal joint failed to unite in one patient. Thirty-seven patients were satisfied with the procedure, noting a reduction in wrist pain after fusion, but all reported some limitation of function. The wrist fusion

A combined volar-dorsal approach was used to treat 11 perilunate dislocations and fracture dislocations between 1989 and 1994. The mean average age of the patients was 38 years, and the mean average time between injury and surgery was 13 hours. Outcome was assessed after an average of 30 months. Results were based on measurements of grip strength, range of motion, radiographs, and patient satisfaction. Patient satisfaction was high in 9 of 11 patients. Seven had satisfactory pain relief, and 5 had returned to their previous occupation without limitation. The wrist flexion-extension arc and gri

PURPOSE OF THE STUDY: We looked for the anatomic origin and mechanism of constitution of the so-called "ganglions" of the wrist.
MATERIAL AND METHODS: Fifty-nine formations considered to be synovial ganglions were dissected and removed according to the same protocol by the same surgeon. Eleven were re-examined by a pathologist. All ganglions were extra-articular but had intra- and extra-capsular components. The extra-capsular part was the clinically palpable main cyst. The intra-capsular part was composed of the cystic stalk and its base of implantation. An intra-capsular stalk was present in

AUDIENCE: The target audience for this small group session is emergency medicine residents, primarily for use in didactic conference. This session can also be utilized with medical students, or faculty looking to review relevant hand anatomy and common injuries.
INTRODUCTION: Three-dimensional (3D) printing is an emerging technology that has the ability to produce highly accurate anatomic, cellular and medical device models. Limited research has shown promise in teaching anatomy,1 congenital heart disease2 and surgical pre-operative planning.3 Despite this potential, there is sparse evidence

PURPOSE: Intra-articular distal radius fractures are common and can be associated with carpal instability. Failure to address articular fragments linked to maintaining carpal stability can lead to radiocarpal subluxation or dislocation. The purpose of this study was to evaluate the size of a dorsal osteotomy in the dorsal/volar plane of the lunate facet that leads to dorsal carpal subluxation.
METHODS: Dorsal lunate facet fractures were simulated twice in each of nine fresh-frozen cadavers. After completing a partial dorsal osteotomy in the radial/ulnar plane between the scaphoid and lunate f

PURPOSE: The lunate is classified into two types, one with a single distal facet and the other with two distal facets. The effect of lunate type on the incidence of wrist disease and trauma remains unclear. The purpose of this study is to evaluate a potential association between lunate morphology and wrist disorders.
METHODS: We retrospectively reviewed the cases of 637 patients who had undergone wrist arthroscopy for wrist disorders. Patient charts and arthroscopic video images were reviewed retrospectively. We defined lunate type based on the Viegas classifications, according to its distal

Four measurements, ulnar tilt, lunate subsidence, lunate fossa angle and palmar carpal displacement, on wrist radiographs of 26 patients with Madelung's deformity and 48 normal subjects were compared. The range of measurements on wrists with Madelung's deformity was wider than on normal wrists, with severe deformities having very abnormal values. Some Madelung's patients had values that were within the normal range for each of the four measurements. Measurement of the lunate fossa angle demonstrated the least overlap between normal wrists and wrists of patients with Madelung's deformity with o

Background: Perilunate fracture-dislocations are frequently associated with a high risk of developing post-traumatic arthritis. Current studies indicate that during mid-term follow-ups, radiological signs of arthritis do not appear to correspond with functional score. The aim of this study was to assess the occurrence of posttraumatic arthritis and the wrist function after perilunate dislocations (PLD) and fracture dislocations at a mid-term follow-up of 7 years. Methods: We report the clinical and radiological outcomes of 17 wrists treated for PLD or fracture-dislocation by open reduction and
