Laparoendoscopic single site surgery in pediatric urology: does it require specialized tools?

PURPOSE: To describe our experience utilizing Laparoendoscopic single site (LESS) surgery in pediatric urology. MATERIALS AND METHODS: Retrospective chart review was performed on LESS urologic procedures from November 2009 through March 2013. A total of 44 patients underwent 54 procedures including: nephrectomy (23), orchiopexy (14), varicocelectomy (9), orchiectomy (2), urachal cyst excision (3), and antegrade continence enema (3) (ACE). RESULTS: Median patient age was 6.9 years old. Estimated blood loss (EBL), ranged from less than 5cc to 47cc for a bilateral nephrectomy. Operative time va

Laparoscopic inguinal hernia repair in children.

BACKGROUND: This study aimed to document the authors' experience with laparoscopic inguinal hernia repair in children. METHODS: Ninety-three hernia repairs were performed in 64 children. The neck was closed with a purse string suture by using 4-0 absorbable suture. RESULTS: Ninety-three indirect inguinal hernial sacs were closed in 64 children. Nine percent of children had an ectopic testis. The mean operating time for laparoscopic ring closure was 25 minutes (range, unilateral 21 to 35; bilateral, 28 to 50). The contralateral processus vaginalis was patent in 20% of children. In 24% of chil

An international multicenter cohort study of management and outcomes of undescended testis in German-speaking countries.

BACKGROUND: Undescended testis (UDT) is the most common congenital genitourinary anomaly in boys. Guidelines recommend orchidopexy by 12-18 months to optimize fertility and reduce malignancy risk, but adherence is often poor. The objectives of this study were to asses institutional management for UDT and evaluate surgical practices and long-term patients outcomes following orchidopexy. METHODS: We conducted a multicenter observational study in German-speaking countries using a standardized questionnaire on UDT management and collected retrospective data on all patients who underwent orchidope

Is torsion of the undescended testis intrinsically associated with worse salvage? A matched cohort analysis.

BACKGROUND: Testicular torsion in undescended testes (UDT) has poor salvage rates, but it remains unclear if the cryptorchid state itself confers an intrinsic disadvantage. We aimed to compare testicular salvage between UDT and descended torsion while controlling for presentation variables. METHODS: In a single-institutional retrospective cohort (2010-2024), using propensity score matching, 25 UDT torsion cases were matched 1:2 to 129 descended torsion controls on age, ischemia time, and testicular volume. The primary outcome was salvage on post-operative imaging (≥50% volume of contralateral

European Paediatric Surgeons' Association Survey on the Adherence to EAU/ESPU Guidelines in the Management of Undescended Testes.

INTRODUCTION: The aim of this study was to assess the adherence to the European Association of Urology (EAU)/European Society for Pediatric Urology (ESPU) 2016 guidelines in the management of undescended testes (UDT). MATERIALS AND METHODS: An online questionnaire was sent in 2023 to members of the European Paediatric Surgeons' Association (EUPSA). RESULTS: Among 157 members, 46 and 44% perform orchidopexy before 12 and 18 months, respectively. In total, 92% recommend conservative management of retractile testes and 58% offer close follow-up. In case of nonpalpable testes, 78% favor laparosc

Timing of orchidopexy and its relationship to postoperative testicular atrophy: results from the ORCHESTRA study.

BACKGROUND: In 2011 a consensus statement from the British Association of Paediatric Urologists recommended lowering the age at orchidopexy to under 1 year. There are concerns that a younger age at operation may increase postoperative testicular atrophy. The ORCHESTRA study aimed to establish the current age at orchidopexy in a multicentre, international audit and to see whether testicular atrophy was affected by age at operation. METHODS: The study was undertaken over a 3-month period in 28 centres in boys undergoing orchidopexy for unilateral, palpable undescended testes. Data collection wa

The outcome of laparoscopic assisted orchidopexy in very young children: A single hospital experience.

INTRODUCTION: Early orchidopexy has been linked to improved long term outcomes of fertility and reduced malignancy rates. However, the optimal age of intervention has been subject to change over the years. OBJECTIVE: This study aims to study males aged 6 months or less who undergo laparoscopic assisted orchidopexy for intraabdominal testes to establish the safety, efficacy, and benefit in the defined age group. STUDY DESIGN: We prospectively assessed 19 boys at or below 6 months of age, who had laparoscopic assisted orchidopexy for intraabdominal undescended testes at Salmaniya Medical Compl

Bilateral polyorchidism with ipsilateral two undescended testes: a rare congenital anomaly.

Polyorchidism is defined as the presence of more than two testes. This rare congenital anomaly has been reported with only 140 pathologically proven cases in the published literature to date. While triorchidism is the most common variation and generally affects the left side, bilateral polyorchidism is even rare and only seven cases of patients with four testes have been reported in the literature. There is no consensus in the literature regarding the management of supernumerary testis due to its rareness. We report such a rare case of a 20-year-old male patient, who was presented with left-si

[Cryptorchidism: guidelines for surgical management].

Cryptorchidism is one of the commonest congenital anomalies in the male genitalia, affecting 3 to 5% of male full-term neonates. It is a known cause of infertility associated with a greater risk of development of germ cell tumor. The benefits of early orchidopexy include psychological affects, prevention of testicular degeneration and decrease in the risk of testicular cancer. Laparoscopy is the best way to diagnose and manage intra-abdominal testes.

The risk of failure after primary orchidopexy: an 18 year review.

OBJECTIVE: To review the primary orchidopexy failure rate and outcome of repeat orchidopexy in a tertiary paediatric surgical centre and identify risk factors. METHODS: A prospectively collected and validated audits system was used to identify all boys having a repeat orchidopexy from August 1990 to December 2008 (18 years). RESULTS: In total, 1538 boys underwent orchidopexy with 1886 testicles operated on. Of these 348 (22.6%) patients had bilateral cryptorchidism. A need for repeat orchidopexy was identified in 31 boys resulting in a primary failure rate of 1.6% over the 18 years. Unilater

[Time of operative treatment of maldescensus testis in childhood - wishes and reality].

BACKGROUND: Maldescensus testis constitutes the most common diagnosis in the daily routine of a paediatric surgeon and is proved to be one of the most established risk factors for future infertility and malignancy. MATERIAL AND METHODS: We examined therapy strategies of paediatricians in the catchment area of the University Clinic of Würzburg in the light of the late time of orchidopexy. This study consists of a short survey sent to 100 local paediatricians. After return, the given answers were put in comparison to the 504 cases of retentio testis treated in our clinic from 2005 to 2010. RES

Cryptorchidism and its long-term complications.

Cryptorchidism is the most frequent defect of the male urogenital tract at birth. It represents a risk factor for primitive testiculopathy associated with long-term complications (infertility, testicular neoplasia, and hormonal changes). An only consensus exists: "children with bilateral cryptorchidism who are not treated in early age are certainly set to become infertile". The majority of Authors agrees that the cryptorchid testicle will be in for structural and functional alterations and the rate of infertility is inversely proportional to the age at the time of orchidopexy. Cryptorchidism c

Age of orchidopexy: Where are we now? A 12-year multicenter cross-sectional study about China.

BACKGROUND: The current practices in undescended testis (UDT) management in China over the past 12 years are little known. METHODS: The demographics of orchidopexies performed at the four major hospitals in Southwest China between 2013 and 2024 were reviewed. The data were taken from the hospital case system and were mainly collected from children's age at surgery. A questionnaire was distributed to obstetric care specialists, pediatricians and obstetricians. RESULTS: A total of 13,528 cryptorchid patients from the four largest centers in Western China were included. The proportion of childr

Orchidopexy and Split-thickness Skin Graft for Scrotal Defects After Necrotizing Fasciitis.

OBJECTIVE: To demonstrate a technique for performing orchidopexy and split-thickness skin graft for patients with deficient scrotal skin after debridement for Fournier's gangrene. This is an alternative strategy to healing by secondary intention, flaps, or testicular thigh pouches. METHODS: Orchidopexy was performed after initial debridement using interrupted Vicryl sutures to bring the testicles and inferior penis together. During this procedure, the testicles and spermatic cord were mobilized, and redundant spermatic cord was coiled under the abdominal wall. Once patients were medically sta

Laparoscopy-Assisted Transscrotal Orchidopexy for Palpable Undescended Testis: Initial Results.

Background: Undescended testis (UDT) is one of the most common congenital genital malformations in boys. However, orchidopexy carries a risk of injuring the testicular vessels and vas deferens. We therefore developed a novel approach to manage palpable UDT. Materials and Methods: We analyzed the medical records of patients who underwent orchidopexy at our institute between January 2017 and April 2020. This study was approved by the Institutional Review Board of our hospital. The age, body weight, laterality, testicular position, length of operation, and complications were investigated. The pat

Postoperative pain in small-for-gestational age infants after hernia repair, orchidopexy and urethral reconstruction surgery: A pilot study.

BACKGROUND: Small-for-gestational-age (SGA) birth bears an enhanced risk of developing hypertension, obesity, insulin resistance and mental health disorders in later life as a consequence of adaptive processes in utero. Only a small number of studies on pain perception in SGA infants exist. These are indicative of a blunted stress response to pain in SGA newborns. AIM: We initiated a pilot study investigating differences in postoperative pain perception between SGA and appropriate-for-gestational-age (AGA) infants. METHODS: Pain and alertness levels of 10 formerly SGA and 14 AGA infants at t

The volume of unilaterally undescended testis after hCG therapy compared to orchidopexy and combined methods.

The aim of the study was to compare the effects of human chorionic gonadotropin (hCG) therapy with those of surgical or combined therapy on testicular volume (TV) in boys at different ages with unilateral canalicular undescended testis (UDT). In total, 155 boys aged 1 to 12 years were treated: either surgically (ST), or by 50 IU/kg body weight hCG administration every three days for five weeks (HT), or by a combination of the two. The patients underwent ultrasound examination of TV before the treatment, 9-12 (median 10) and 24-39 months (median 32) after therapy. The testicular atrophy index (
