From Caries to Periodontal Breakdown: A Biological and Clinical Continuum Linking Cariology, Operative Dentistry, Endodontics, and Periodontology.

Dental diseases have long been taught and treated as separate entities: cariology, operative dentistry, endodontics, and periodontology, each working within its own boundaries. However, increasing biological and clinical evidence suggests that this classified view does not fully reflect how disease progresses in the mouth. Instead, dental disease should be understood as a continuum within the interconnected tooth-pulp-periodontium complex. This review provides current evidence showing how dental caries can serve as the starting point of a process that can progress through pulpitis and apical p

Pulp liner materials in selective caries removal: study protocol for a randomised controlled trial.

INTRODUCTION: The literature shows that selective carious tissue removal (SCTR) decreases the number and diversity of bacteria, stops the caries process and reduces the risk of pulp exposure. However, no consensus exists on which pulp liner would be suitable for teeth undergoing SCTR. So, this study will verify the in vivo response of dentine-pulp complex after SCTR in primary teeth with or without pulp liner material. METHODS AND ANALYSIS: A randomised clinical trial, double-blinded, parallel-group and allocation concealment will be conducted with the enrolment of 384 patients from 5 to 9 ye

PGE2 Regulates Periodontal Ligament Repair after Tooth Replantation.

Tooth replantation is a clinical treatment commonly used for refractory periapical periodontitis, tooth avulsion, and autotransplantation. The condition of the periodontal ligament (PDL) is a prognostic factor that affects replantation outcomes. Various studies have been devoted to improving the repair of the PDL after replantation to improve the prognosis. Prostaglandin E2 (PGE2), an inflammatory mediator with critical effects involved in stem cell regulation as well as in tissue repair and regeneration, has been found to be increased after replantation. Thus, we hypothesized that the increas

Fibrodysplasia Ossificans Progressiva.

Fibrodysplasia ossificans progressiva is a rare genetic disorder characterized by progressive heterotopic ossification, which may lead to severe functional impairment. Orofacial involvement can result in marked restriction of jaw movement up to extra-articular temporomandibular ankylosis, severely complicating oral hygiene, diagnostics and dental treatment. This case report describes a 25-year-old male patient with advanced fibrodysplasia ossificans progressiva and complete jaw immobility with a maximum interincisal opening of 0 mm. He presented himself with daily dental pain, poor oral hygien

Innervation Changes in Apical Periodontitis and its Correlation with Preoperative Symptoms.

INTRODUCTION: Previous anatomical studies have demonstrated the cellular composition of apical periodontitis (AP) lesions, primarily focusing on immune cells. Only recently has the innervation of an apical lesion been reported, but evidence regarding the quantity and quality of neural innervation in AP lesions remains largely unknown. Importantly, periapical innervation has been shown to modulate cellular response in AP, and the voltage-gated sodium channels Nav1.8, a marker of nociceptors, and Nav1.9 have been associated with various pain-related conditions. This study aimed to quantify senso
