Pyrocarbon interposition shoulder arthroplasty in advanced collapse of the humeral head.

BACKGROUND: Pyrocarbon (PyC) interposition shoulder arthroplasty can be used to treat advanced collapse of the humeral head after avascular necrosis. OBJECTIVES: We examined outcomes for this bone-preserving implant which has a PyC coating and a novel biomechanical concept. MATERIALS AND METHODS: For a minimum of 2 years, we followed 10 patients (4 men, 6 women, 55.6 ± 12.9 years) treated with a free interposition PyC arthroplasty ("snookerball") due to advanced humeral head collapse but with an intact glenoid and rotator cuff. Anteroposterior radiographs, the Constant score (CS), adjusted C

Cemented versus uncemented fixation of humeral components in total shoulder arthroplasty for osteoarthritis of the shoulder: a prospective, randomized, double-blind clinical trial-A JOINTs Canada Project.

BACKGROUND: Although cemented humeral fixation is recognized as the standard of care in total shoulder arthroplasty (TSA), uncemented fixation has the potential to provide stable fixation, decrease operative time, and simplify potential revision procedures. This prospective, randomized, double-blind clinical trial compared cemented and uncemented humeral fixation in TSA for primary shoulder osteoarthritis. METHODS: Patients with primary shoulder osteoarthritis requiring replacement were screened for eligibility. After providing informed consent, subjects received baseline clinical and radiolo

Endovascular treatment of subclavian pseudoaneurysm causing delayed brachial plexus paralysis.

Pseudoaneurysms of the subclavian artery are rarities. Due to their close relationship to the brachial plexus in the shoulder girdle, delayed lesions of the brachial plexus are possible. We report a case of a subclavian pseudoaneurysm following fracture of the clavicle, and the endovascular treatment using a balloon-expandable stent-graft.

Axillary artery injury: report of two cases and review of the literature.

These are the cases of two young men, 21- and 28-year old, who sustained penetrating stab injuries to their axillary arteries. Both patients presented initially with profuse bleeding and were in a state of shock. The first patient underwent initial emergency surgery to control his bleeding whereby deep stitches were inserted around the axillary vessels and nerves and ended up with permanent iatrogenic Median nerve palsy. The bleeding in the second patient was controlled initially by compression dressing and he underwent successful vascular repair of the axillary artery with full recovery of hi

[4-fragment fractures of the proximal humerus. Alternative strategies for surgical treatment].

The operative treatment of dislocated fractures of the proximal humerus has been evolving in recent years. Replacement using endoprostheses often results in only moderate functional outcome, and with the high risk of aseptic necrosis with ORIF, new methods of minimally invasive stabilisation have been developed. These methods reduce the opening of the fracture site to a minimum and thereby limit the risk of iatrogenic damage to local vascularity and the rotator cuff. This study reviewed 18 patients operated on with minimal osteosynthesis for dislocated four-part fractures of the proximal humer

Antibiotic cement spacer retention for chronic shoulder infection after minimum 2-year follow-up.

HYPOTHESIS: The treatment of periprosthetic shoulder infections and proximal humerus osteomyelitis is challenging. The outcomes of antibiotic cement spacer retention are poorly defined in the literature. The purpose of this study was to review long-term functional and patient-reported outcomes data of patients with retained antibiotic cement spacers. We predict reasonable functional outcomes and minimal pain. METHODS: We identified 22 patients of the senior author who have been treated with definitive antibiotic spacer placement. All patients were originally offered a 2-stage revision and dec

Observations on retrieved glenoid components from total shoulder arthroplasty.

HYPOTHESIS: Polyethylene components retrieved at revision of total knee and hip replacements have been analyzed to study the effect of design, patient, and surgical factors on initial implant performance, but few studies have reported similar types of findings in retrieved glenoids. MATERIALS AND METHODS: From 1979 to 2006, 78 glenoid components were retrieved from revision surgery in 73 patients at a single institution. Each glenoid component was analyzed for 9 modes of damage in each of 4 quadrants into which the bearing surface was divided. For each glenoid, the most recent radiographs bef

Clinical and radiographical outcomes of scapular spine fractures after reverse shoulder arthroplasty treated with double plating and autologous bone graft.

BACKGROUND: Scapular spine fractures (SSFs) are a challenging complication after reverse total shoulder arthroplasty (rTSA) leading to persisting pain, loss of function, and instability. So far there is no established uniform surgical procedure for successful treatment. METHODS: Single-center consecutive cohort study of 25 surgically treated SSFs after rTSA between 2016 and 2021. Retrospective analysis regarding demographic data, implant specification, radiographic scans, and clinical analysis of all SSFs treated with a standardized fixation technique using a double plate augmented with an il

Progressive bone loss and bleeding on probing: A cohort study.

AIM: To investigate whether a progressive marginal bone loss (PMBL) occurring beyond the initial bone remodeling (IBR) is linked with bleeding on probing. MATERIALS AND METHODS: A total of 70 partially edentulous patients exhibiting 112 two-piece bone-level implants were included in this retrospective study. Panoramic radiographs were obtained after implant insertion (T0), after delivery of a final prosthetic restoration (T1) and subsequently during the 1-(T2), 5-(T3), 10-(T4), and 15-years (T5) follow-up visits. At each time point, radiographic marginal bone levels were assessed from the imp

Shoulder arthroplasty for advanced glenohumeral arthritis after anterior instability repair.

We retrospectively reviewed 19 patients (19 shoulders) in whom advanced glenohumeral arthritis developed after anterior instability repair. The anterior instability repairs included 4 Bristow, 4 Putti-Platt (2 in combination with other procedures), 4 Magnuson-Stack, 2 Bankart, and 5 other anterior capsulorrhaphies. Seventeen of the shoulders were treated with arthroplasty: 15 with total shoulder replacement and 2 with humeral head replacement, at a mean age of 45 years (range, 32-69 years). Two patients did not have surgical treatment. The mean internal rotation contracture was 58 degrees (15

The role of serum D-Dimer for the diagnosis of periprosthetic shoulder infection.

INTRODUCTION: D-Dimer was recently identified as an additional biomarker in the diagnosis of hip and knee periprosthetic joint infection (PJI). Currently, there is only one study in literature dealing with the role of D-Dimer in the diagnosis of shoulder PJI. The purpose of this study was, therefore, to validate the sensitivity and specificity of D-Dimer in detecting shoulder PJI. MATERIALS AND METHODS: All patients, who underwent septic or aseptic revision shoulder arthroplasty in our institution between November 2018 und March 2021, were analyzed. Our cohort consisted of 30 patients, of tha

The results of ORIF of displaced unstable proximal humeral fractures using a locking plate.

BACKGROUND/HYPOTHESIS: Surgical management of displaced unstable proximal humerus fracture remains a challenge due to poor proximal bone quality and significant deforming forces. We hypothesized that the technique of application and mechanical properties of the proximal humeral locking plate would allow successful treatment of unstable and displaced proximal humeral fractures even in the face of osteoporotic bone. METHOD: We evaluated prospectively the results of open reduction internal fixation of 22 displaced unstable proximal humerus fractures in 22 patients utilizing a proximal humeral lo

In vivo glenohumeral contact forces--measurements in the first patient 7 months postoperatively.

Knowledge of forces in the glenohumeral joint is essential for understanding normal and pathologic shoulder function. It forms the basis for performing fracture treatment or joint replacement surgery, for optimizing implant design and fixation and for improving and verifying analytical biomechanical models of the shoulder. An instrumented shoulder implant with telemetric data transmission was developed to measure six components of joint contact forces and moments. A patient with humeral head arthrosis achieved good joint function after its implantation. During the first 7 postoperative months,
