Laparoscopic partial nephrectomy for renal tumor: single center experience comparing clamping and no clamping techniques of the renal vasculature.

PURPOSE: We performed a nonrandomized retrospective comparison of 2 techniques for laparoscopic partial nephrectomy, that is without and with clamping the renal vessels. MATERIALS AND METHODS: Between December 1997 and February 2002, 28 consecutive patients underwent transperitoneal laparoscopic partial nephrectomy for renal tumor. In group 1 (12 patients) partial nephrectomy was performed with ultrasonic shears and bipolar cautery without clamping the renal vessels, while in group 2 (16 patients) the renal pedicle was clamped before tumor excision. In group 2 patients intracorporeal kidney c

Predictors of endoscopic third ventriculostomy ostomy status in patients who experience failure of endoscopic third ventriculostomy with choroid plexus cauterization.

OBJECTIVE: At failure of endoscopic third ventriculostomy (ETV) with choroid plexus cauterization (CPC), the ETV ostomy may be found to be closed or open. Failure with a closed ostomy may indicate a population that could benefit from evolving techniques to keep the ostomy open and may be candidates for repeat ETV, whereas failure with an open ostomy may be due to persistently abnormal CSF dynamics. This study seeks to identify clinical and radiographic predictors of ostomy status at the time of ETV/CPC failure. METHODS: The authors conducted a multicenter, retrospective cohort study on all pe

Submucosal Tunneling and Endoscopic Resection of an Esophageal Leiomyoma in a 13-Year-Old Male.

INTRODUCTION: Esophageal leiomyoma, while rare in children, is the most common benign tumor of the esophagus.1 Many children with leiomyomas present with diffuse disease.2-4 Isolated tumors have been managed by primary resection or enucleation with thoracotomy or thoracoscopy.5,6 More recently, submucosal tunneling and endoscopic resection (STER) has been used for resection of intramural esophageal masses.7-14 This technique for endoscopic surgical resection takes advantage of the submucosal plane, as employed in the per-oral endoscopic myotomy procedure. We describe the first use of STER to t

Management of pilonidal sinus and recurrences in 2025.

Pilonidal sinus is a common pathology of the intergluteal cleft that can develop into abscess or suppuration. This lesion corresponds histologically to a granuloma that organizes around foreign bodies, most often hairs, and fistulizes to the skin through partially epithelialized orifices. If suppuration and abscess develop, treatment is based either on medical treatment combining analgesics, local antiseptics and sometimes antibiotics, or on emergency incision and drainage in the operating room. This is performed in more than 10,000 patients per year in France. Outside of emergencies, elective

Penile amputation after neonatal circumcision: a case report.

UNLABELLED: In children, one of the most common surgical procedures worldwide is circumcision, which has strong religious implications and is frequently performed for nontherapeutic reasons. Circumcision is typically associated with old customs. Complete penile amputation is extremely uncommon, and the prognosis is little understood. CASE PRESENTATION: A 7-day-old male term baby was circumcised with a cauter by an unexperienced practitioner, and the patient was admitted to the department of pediatric surgery. Both the glans and the body had become discolored and necrotic. The patient was take

Impact of endoscopic ultrasound-guided gallbladder drainage on reducing costs of reintervention and unplanned readmission: a budget impact analysis.

Background and study aims  Endoscopic ultrasound-guided gallbladder drainage (EUS-GBD) is the preferred treatment for patients with acute calculous cholecystitis who are unfit for surgery. The aim of this study was to perform a cost-effective analysis (CEA) comparing EUS-GBD with percutaneous gallbladder drainage (PT-GBD). Patients and methods  CEA was performed on patients recruited for our prior randomized controlled trial. A budget impact model was developed to compare the base-case and scenario of EUS-GBD applications. The costs including peri-procedure and intra-procedure, reinterventions

Minimally Invasive Techniques for Iliac Bolt Placement: 2-Dimensional Operative Video.

As the popularity of minimally invasive surgery (MIS) continues to grow, novel techniques are needed to meet the demands of multisegment fixation for advanced spinal diseases. In one such example, iliac bolts are often required to anchor large fusion constructs, but MIS technical notes are missing from the literature.  A 67-yr-old female presented with a symptomatic coronal deformity: preoperative pelvic incidence = 47°, pelvic tilt = 19°, and lumbar lordosis = 29°, sagittal vertical axis = +5.4 cm with 30° of scoliosis. Estimating the Incidence of Stray Energy Burns during Laparoscopic Surgery based on Two Statewide Databases and Retrospective Rates: An Opportunity to Improve Patient Safety.

BACKGROUND: The growth of laparoscopic surgery has increased the use of laparoscopic electrosurgical devices based on radiofrequency current. Despite an improvement in most post-operative outcomes, the use of these devices can be associated with inadvertent thermal or mechanical injuries, also called accidental punctures and lacerations (APLs). APLs can occur through either operator error or system error, including insulation failure or capacitive coupling resulting in stray energy burns. Our aim was to estimate the incidence and-as a result-the impact of laparoscopic APLs. METHODS: A retrosp

Technique to minimize paraspinal muscle atrophy after posterior cervical fusion.

OBJECTIVE: Paraspinal muscle atrophy (PMA) after posterior cervical fusion is a known complication that causes considerable morbidity. It has been shown in the lumbar spine that preservation of the posterior ramus of the spinal nerve is important in minimizing paraspinal muscle atrophy. During posterior cervical spine fusions, we modified the exposure of the dorsal cervical spine by exposing only the medial two-thirds of the lateral mass utilizing a low electrocautery setting. In a retrospective analysis, we compared the incidence of paraspinal muscle atrophy using this modified technique with

Plasminogen activators: regulators of tumor cell adherence to sites of lower urinary tract surgical trauma.

Tumor cell adherence (TCA) to sites of surgical injury is a requisite step in implantation-mediated tumor recurrence. This study examined the relationship between tumor-associated plasminogen activator (PA) and the ability of tumor cells to remain adherent to surgical injury sites. High (1E8) and low (3A9) PA producing clones from the murine transitional carcinoma cell line 4909 were selected using an in vitro 125I fibrinolysis assay. Net cellular PA activity of each clone was determined from cell lysates using a chromogenic substrate assay. In vitro TCA and fibrin substrate lysis as a functio
