Space Flight-Associated Neuroocular Syndrome, Idiopathic Intracranial Hypertension, and Pseudotumor Cerebri: Phenotypic Descriptions, Pathogenesis, and Hydrodynamics.

Recent data from astronauts who have returned to Earth from a long-duration space flight have unequivocally distinguished spaceflight-associated neuro-ocular syndrome (SANS) from idiopathic intracranial hypertension (IIH) and pseudotumor cerebri (PTC). We review the semiology and pathogenesis of these three entities, noting that optic disc edema is what unites them, and this where the similarities between SANS and IIH/PTC end. We distinguish between PTC and IIH and between SANS and IIH/PTC and review the medical and surgical therapy of IIH/PTC. The key to understanding the phenomenon of optic

Parkinsonism and cerebrovascular disease.

The relationship between cerebrovascular disease and parkinsonism is commonly seen in everyday clinical practice but remains ill-defined and under-recognised with little guidance for the practising neurologist. We attempt to define this association and to illustrate key clinical, radiological and pathological features of the syndrome of Vascular Parkinsonism (VaP). VaP is a major cause of morbidity in the elderly associated with falls, hip fractures and cognitive impairment. Although acute parkinsonism is reported in the context of an acute cerebrovascular event, the vast majority of VaP prese

Zinc ions regulate opening of tight junction favouring efflux of macromolecules via the GSK3β/snail-mediated pathway.

Zinc is an essential trace element presenting in particularly high concentration in the brain. In some regions, e.g. lateral amygdala, subiculum and hippocampus, rapidly-exchangeable zinc may transiently reach even up to 600 μM. To explore the possible roles of high-concentration Zn2+ in regulating the blood-brain barrier (BBB), we investigated the effects of Zn2+ on the functions and structures of the tight junction (TJ) with an in vitro model of a Madin-Darby canine kidney (MDCK) cell monolayer. The experimental results indicated that high concentrations (>200 μM) of Zn2+ can affect the TJ i

A diffusion MRI-derived perivascular metric related to glymphatic-associated processes in bipolar disorder vulnerability: Multimodal correlates across emotion dysregulation patients and offspring.

Bipolar disorder (BD) is characterized by marked emotion dysregulation and high familial risk. Identifying early biological markers of vulnerability in BD, including unaffected offspring, is critical to improve risk stratification and intervention, and glymphatic-associated processes may contribute to this vulnerability. We examined 237 participants, including 97 patients with emotion dysregulation disorders (EDD; 34 BD, 33 borderline personality disorder [BPD], 30 attention-deficit/hyperactivity disorder [ADHD]), 67 offspring of EDD patients (EDDoff; including 23 BD offspring [BDoff]), and 73

Glymphatic Dysfunction in Non-Manifesting Carriers of LRRK2 and GBA Pathogenic Variants.

BACKGROUND: Non-manifesting carriers of LRRK2 and GBA pathogenic variants represent a unique cohort for investigating neuroprotective interventions at the prodromal stage of Parkinson's disease (PD). A critical challenge is identifying effective markers to predict non-manifesting carriers at high risk of developing PD. OBJECTIVES: Our goal was to investigate whether glymphatic function is impaired in non-manifesting carriers and to evaluate the potential of glymphatic dysfunction as a marker for identifying individuals at high risk of PD. METHODS: We used diffusion-tensor imaging analysis al

Non-invasive MR imaging of human brain lymphatic networks with connections to cervical lymph nodes.

Meningeal lymphatic vessels have been described in animal studies, but limited comparable data is available in human studies. Here we show dural lymphatic structures along the dural venous sinuses in dorsal regions and along cranial nerves in the ventral regions in the human brain. 3D T2-Fluid Attenuated Inversion Recovery magnetic resonance imaging relies on internal signals of protein rich lymphatic fluid rather than contrast media and is used in the present study to visualize the major human dural lymphatic structures. Moreover we detect direct connections between lymphatic fluid channels a

Challenges and Opportunities of Deferoxamine Delivery for Treatment of Alzheimer's Disease, Parkinson's Disease, and Intracerebral Hemorrhage.

Deferoxamine mesylate (DFO) is an FDA-approved, hexadentate iron chelator routinely used to alleviate systemic iron burden in thalassemia major and sickle cell patients. Iron accumulation in these disease states results from the repeated blood transfusions required to manage these conditions. Iron accumulation has also been implicated in the pathogenesis of Alzheimer's disease (AD), Parkinson's disease (PD), and secondary injury following intracerebral hemorrhage (ICH). Chelation of brain iron is thus a promising therapeutic strategy for improving behavioral outcomes and slowing neurodegenerat

Astrocyte-centered temporal resilience at the brain-behavior-immunity interface: sleep-circadian disruption and perioperative cognitive vulnerability in older adults.

Perioperative cognitive vulnerability in older adults reflects interactions among inflammatory stress, altered brain state, sleep-circadian disruption, and recovery capacity. This narrative review examines astrocyte-centered temporal resilience as a hypothesis-generating framework for integrating these brain-immune-behavior processes. Astrocytes participate in sleep-pressure signaling, circadian regulation, neurometabolic support, inflammatory responses, extracellular homeostasis, and clearance-related physiology; however, these functions operate within broader neuronal, microglial, vascular,

Operationalizing perioperative clearance reserve: a reserve-load interaction model for neurocognitive outcomes in older surgical patients.

Older patients who undergo similar operations and receive comparable anesthetic care do not always recover cognitively in the same way. Inflammation explains an important part of perioperative brain injury, yet it does not fully account for why the response settles quickly in one patient and persists in another. We suggest that this difference may reflect an interaction between the patient's preoperative capacity for brain fluid and solute clearance and the load imposed by surgery and anesthesia. In the proposed model, baseline clearance reserve is a continuous latent phenotype informed primar

Astrocyte-blood-brain barrier crosstalk in cerebral small vessel disease: linking barrier dysfunction to neurovascular failure.

Cerebral small vessel disease (CSVD) is a heterogeneous group of disorders involving the cerebral microvasculature and is a major cause of stroke and vascular cognitive impairment. The role of blood-brain barrier (BBB) dysfunction in CSVD is now recognized as a key pathological process; however, conventional vascular risk factors do not fully explain its initiation, progression, or clinical heterogeneity. Astrocyte-BBB crosstalk has been implicated in endothelial dysfunction, pericyte injury, and vascular remodeling, and the mechanisms by which early barrier instability progresses to neurovasc

Unraveling the complex role of microglia in Alzheimer's disease: amyloid β metabolism and plaque formation.

BACKGROUND: Alzheimer's disease (AD) is characterized by amyloid β (Aβ) accumulation in the brain. Recent genome-wide association studies have identified numerous AD risk genes highly expressed in microglia, highlighting their potential role in AD pathogenesis. Although microglia possess phagocytic capacity and have been implicated in Aβ clearance, accumulating evidence suggests their contribution to AD pathogenesis is more complex than initially anticipated. MAIN BODY: This review synthesizes current knowledge on microglial Aβ metabolism in AD, reconciling conflicting data from various studi

The microcirculation, the blood-brain barrier, and the neurovascular unit in health and Alzheimer disease: The aberrant pericyte is a central player.

High fidelity neuronal signaling is enabled by a stable local microenvironment. A high degree of homeostatic regulation of the brain microenvironment, and its separation from the variable and potentially neurotoxic contents of the blood, is brought about by the central nervous system barriers. Evidence from clinical and preclinical studies implicates brain microcirculation, cerebral hypoperfusion, blood-brain barrier dysfunction, and reduced amyloid clearance in Alzheimer pathophysiology. Studying this dysregulation is key to understanding Alzheimer disease (AD), identifying drug targets, deve

Quantitative evaluation of normal cerebrospinal fluid flow in Sylvian aqueduct and perivascular spaces of middle cerebral artery and circle of Willis using 2D phase-contrast MRI imaging.

INTRODUCTION: Recently, it was proposed that CSF flow comprises a critical part of the glymphatic system, playing a role in various brain abnormalities from Alzheimer's disease to hydrocephalus. Thus, CSF flow measurements have been increasingly used for diagnostic and clinical monitoring purposes. However, CSF flow in the periarterial spaces of the circle of Willis and the middle cerebral artery remain unexplored. METHODS: We employed phase-contrast MRI to establish baseline parameters of CSF flow along the perivascular spaces of the circle of Willis and the middle cerebral artery and compar

The Cerebrovascular Side of Plasticity: Microvascular Architecture across Health and Neurodegenerative and Vascular Diseases.

The delivery of nutrients to the brain is provided by a 600 km network of capillaries and microvessels. Indeed, the brain is highly energy demanding and, among a total amount of 100 billion neurons, each neuron is located just 10-20 μm from a capillary. This vascular network also forms part of the blood-brain barrier (BBB), which maintains the brain's stable environment by regulating chemical balance, immune cell transport, and blocking toxins. Typically, brain microvascular endothelial cells (BMECs) have low turnover, indicating a stable cerebrovascular structure. However, this structure can

Estimation of Various Physiological Parameters Affecting Amyloid Plaque Distribution of a Novel MRI Contrast Agent in the Brain of Alzheimer's Disease Transgenic Mice.

Alzheimer's disease (AD) is characterized by the accumulation of amyloid-β (Aβ) peptides as plaques in the brain parenchyma and as deposits in the cerebral vasculature. Early detection of amyloid plaques and deposits is imperative for diagnosing AD before the onset of cognitive decline. Magnetic resonance (MR) imaging using Gd (III)-based agents for contrast enhancement and plaque targeting provides a promising avenue. However, there remains a challenge due to the limited permeability of these contrast agents across the blood-brain barrier (BBB), which restricts its delivery. Furthermore, clea

An Alzheimer's Disease Mechanism Based on Early Pathology, Anatomy, Vascular-Induced Flow, and Migration of Maximum Flow Stress Energy Location with Increasing Vascular Disease.

This paper suggests a chemical mechanism for the earliest stages of Alzheimer's disease (AD). Cerebrospinal fluid (CSF) flow stresses provide the energy needed to induce molecular conformation changes leading to AD by initiating amyloid-β (Aβ) and tau aggregation. Shear and extensional flow stresses initiate aggregation in the laboratory and in natural biophysical processes. Energy-rich CSF flow regions are mainly found in lower brain regions. MRI studies reveal flow stress "hot spots" in basal cisterns and brain ventricles that have chaotic flow properties that can distort molecules such as A

Multimodal Neuroimaging-Based Machine Learning Models Leveraging Cerebral Morphometry and Glymphatic Parameters Predict Short-Term Post-Programming STN-DBS Motor Response in Parkinson's Disease.

BACKGROUND AND OBJECTIVES: Deep brain stimulation of the subthalamic nucleus (STN-DBS) is effective for medication-refractory Parkinson's disease (PD) motor symptoms, but clinical response varies across symptom domains, particularly tremor and gait. Accurate preoperative stratification is clinically important, especially for early post-programming outcomes. METHODS: We retrospectively enrolled 155 patients with PD undergoing bilateral STN-DBS and 43 healthy controls. Preoperative structural magnetic resonance imaging and diffusion-weighted imaging were used to quantify brain morphometry and g

Vascular permeability mediates contralateral glymphatic dysfunction across glioma grades and IDH status.

PURPOSE: Bilateral hemisphere-specific glymphatic dysfunction (GS) is a prevalent feature in glioma patients and may be affected by vascular permeability. However, the mechanisms by which tumor-related vascular permeability influences tumor-ipsilateral or contralateral glymphatic function remain unclear. This study aimed to elucidate how tumor-related vascular permeability modulates ipsilateral or contralateral glymphatic function in glioma patients, stratified by WHO grades and IDH mutation statuses, employing a novel integration of noninvasive diffusion tensor imaging along the perivascular
