Rehabilitation protocol after suturing the medial meniscus of a stable knee, a retrospective series of the Francophone Arthroscopy Society.

INTRODUCTION: Surgical meniscal lesions are common and 80% of cases involve the medial meniscus of a stable knee. There is a lack of consensus regarding postoperative rehabilitation protocols and a wide variation between restrictive and accelerated rehabilitation protocols exists. The main objective of this study was to report the functional results and the failure rate of the various rehabilitation protocols of the retrospective series of the French Society of Arthroscopy (SFA) after suturing the medial meniscus of a stable knee, considering whether the tear is stable or unstable. HYPOTHESIS

Biplanar Proximal Tibial Opening-Wedge Osteotomy for a Chronic PCL Tear With Medial Compartment Arthritis.

BACKGROUND: Chronic posterior cruciate ligament (PCL) insufficiency leads to instability of the knee, resulting in increased loading of the medial and patellofemoral compartments. Chronic PCL insufficiency progressively damages the menisci and chondral surfaces and can accentuate and worsen varus malalignment. Biplanar proximal tibial opening-wedge osteotomy (BPTO) is a treatment option for chronic PCL tears with medial compartment arthritis which concurrently treats sagittal (flat slope) and varus coronal alignment. INDICATIONS: Opening-wedge BPTO is indicated in active patients who present

Combined Anterior Cruciate Ligament Reconstruction and Lateral Extra-Articular Tenodesis: The "Over-the-Top" Technique.

BACKGROUND: The anterior cruciate ligament (ACL) is a primary restraint to anteroposterior as well as rotatory knee laxity. In case of concomitant lesion of menisci or other ligamentous structures, further dynamic instability is encountered. A lateral extra-articular tenodesis (LET) augmentation has been proposed by the Authors to treat or prevent residual laxity. INDICATIONS: ACL reconstruction is recommended in young athletes involved in pivoting sports, non-contact pivoting injuries, high-grade pivot shift, deep notch sign and double bone bruise, meniscal loss, and revision of previous bon

Opioid Prescribing After Surgery in the United States, Canada, and Sweden.

IMPORTANCE: Small studies and anecdotal evidence suggest marked differences in the use of opioids after surgery internationally; however, this has not been evaluated systematically across populations receiving similar procedures in different countries. OBJECTIVE: To determine whether there are differences in the frequency, amount, and type of opioids dispensed after surgery among the United States, Canada, and Sweden. DESIGN, SETTING, AND PARTICIPANTS: This cohort study included patients without previous opioid prescriptions aged 16 to 64 years who underwent 4 low-risk surgical procedures (i

Inhibition of Nrf2/HO-1 signaling leads to increased activation of the NLRP3 inflammasome in osteoarthritis.

INTRODUCTION: Osteoarthritis (OA) is an inflammatory disease of the joints that causes progressive disability in the elderly. Reactive oxygen species (ROS) play an important role in OA development; they may activate the NLRP3 inflammasome, thereby inducing the secretion of proinflammatory IL-1β and IL-18, leading to the aggravation of the downstream inflammatory response. Nrf2 is a key transcription factor that regulates the expression of antioxidant enzymes that protect against oxidative stress and tissue damage. We aimed to explore the underlying mechanism of OA development by investigating

Micro CT and Micro MR imaging of 3D architecture of animal skeleton.

Quantitative assessment of three-dimensional (3D) trabecular structural characteristics may improve our ability to understand the pathophysiology of osteoporosis, to test the efficacy of pharmaceutical intervention, and to estimate bone biomechanical properties. Considerable progress has been made in advanced imaging techniques for noninvasive and/or nondestructive assessment of 3D trabecular structure and connectivity. Micro computed tomography (microCT) has been used to measure 3D trabecular bone structure in rats, both in vivo and in vitro. It can directly quantify 3D trabecular bone struct

High return to sport rate and few re-ruptures at long term in professional footballers after anterior cruciate ligament reconstruction with hamstrings.

PURPOSE: Anterior cruciate ligament (ACL) ruptures are considered high burden injuries in sports with high pivotal activity, especially for professional footballers. A lack of evidence exists about long-term follow up of professional elite athletes who underwent ACL reconstruction. The purpose of the study is to analyze the return to play and the career of professional footballers who underwent ACL reconstruction with hamstrings, to evaluate re-rupture and reoperation at either indexed and contralateral knee, and to assess the long-term clinical subjective outcomes and satisfaction. METHODS:

The Role of Crutches and Bracing in Preventing Secondary Meniscal Tears After Anterior Cruciate Ligament Injury in Pediatric Patients.

BACKGROUND: Secondary meniscal tears are a well-described sequela of delay in anterior cruciate ligament (ACL) reconstruction (ACLR) after ACL injury. This study aimed to evaluate whether preoperative conservative interventions (ie, crutches, bracing, and physical therapy) are associated with a reduced risk of secondary meniscal pathology in pediatric patients. HYPOTHESIS: Preoperative conservative interventions of the ACL-deficient knee would be associated with fewer secondary meniscal tears in pediatric patients undergoing delayed (≥8 weeks postinjury) ACLR. Risk factors for hospital admission in patients undergoing outpatient anterior cruciate ligament reconstruction: A national database study.

INTRODUCTION: Anterior Cruciate Ligament Reconstructions (ACLR) are routinely performed in an outpatient setting with low 90-day readmission rates (2.3%); however, admissions rates in the immediate perioperative period have been previously reported as high as 13.1%. Despite the surprisingly high number of patients requiring immediate perioperative admission, there has been a lack of recent literature specifically examining the associated risk factors for admission. METHODS: Using the American College of Surgeons National Surgical Quality Improvement Program (ACS-NSQIP) database, a query for p

A Novel Repair Method for Radial Tears of the Medial Meniscus: Biomechanical Comparison of Transtibial 2-Tunnel and Double Horizontal Mattress Suture Techniques Under Cyclic Loading.

BACKGROUND: Complete radial tears of the medial meniscus have been reported to be functionally similar to a total meniscectomy. At present, there is no consensus on an ideal technique for repair of radial midbody tears of the medial meniscus. Prior attempts at repair with double horizontal mattress suture techniques have led to a reportedly high rate of incomplete healing or healing in a nonanatomic (gapped) position, which compromises the ability of the meniscus to withstand hoop stresses. HYPOTHESIS: A newly proposed 2-tunnel radial meniscal repair method will result in decreased gapping an

Self-assembled meniscus-specific ECM membrane promotes cell migration and vascular ingrowth for meniscus repair and reconstruction.

BACKGROUND: The knee meniscus has limited intrinsic repair and regenerative capacity. Safe and effective biotherapeutic products remain lacking, and enhancing cell migration and angiogenesis remains a major challenge. This study aimed to develop a novel self-assembled meniscus-specific extracellular matrix membrane (saECM-M) and evaluate its performance in scaffold-free meniscus repair and scaffold-guided whole-meniscus reconstruction, in comparison with chemically processed decellularized ECM (cpd-ECM). METHODS: Meniscal fibrochondrocytes (MFCs) were cultured under membrane-inducing conditio

Are Altered Knee Joint Biomechanics Associated with Future Post-Traumatic Osteoarthritis Outcomes? A Systematic Review and Meta-Analysis of Longitudinal Studies.

BACKGROUND: Post-traumatic knee osteoarthritis affects an estimated one in two people within a decade of traumatic knee injury. While altered biomechanics in older adults are associated with the onset and progression of insidious-onset knee osteoarthritis, the relationship between biomechanics and post-traumatic osteoarthritis is less clear. OBJECTIVE: We aimed to evaluate associations between knee biomechanics and future structural and symptomatic outcomes post-surgery for traumatic knee injuries.
