Osteoarthrosis of the atlanto-axial joints. Long-term follow-up after treatment with arthrodesis.

We evaluated the results for fifteen patients in whom symptomatic osteoarthrosis of the atlanto-axial joints had been treated with an atlanto-axial or occipitocervical arthrodesis between 1973 and 1990. Thirteen patients had long-term follow-up (average duration, seven years and two months; range, four years and two months to nineteen years and two months). The two remaining patients had died: one, four days postoperatively, from a cardiopulmonary arrest, and the other, one year and eight months postoperatively, from complications related to bladder cancer. Preoperatively, all fifteen patients

Plate fixation of odontoid fractures without C1-C2 arthrodesis: practice of a novel surgical technique for stabilization of odontoid fractures, including the opportunity to extend the fixation to C3.

OBJECTIVE: Surgical treatment of odontoid fractures that do not allow interfragmentary fracture compression involves either posterior atlantoaxial arthrodesis or additional anterior stabilization using a plate construct. The purpose of this study was to determine the clinical and radiographic outcome after anterior plate fixation of odontoid fractures that were not suitable for anterior screw fixation. METHODS: We reviewed the clinical and radiographic records of 9 patients with an average age of 54 years at the time of surgery who had undergone anterior plate fixation of an odontoid fracture

Late recovery following spinal cord injury. Case report and review of the literature.

The authors of this prospective, single-case study evaluated the potential for functional recovery from chronic spinal cord injury (SCI). The patient was motor complete with minimal and transient sensory perception in the left hemibody. His condition was classified as C-2 American Spinal Injury Association (ASIA) Grade A and he had experienced no substantial recovery in the first 5 years after traumatic SCI. Clinical experience and evidence from the scientific literature suggest that further recovery would not take place. When the study began in 1999, the patient was tetraplegic and unable to

Posterior atlantoaxial facet screw fixation in rheumatoid arthritis.

OBJECT: This retrospective review was conducted to determine the efficacy of transarticular screw fixation in a group of patients who were treated for rheumatoid atlantoaxial instability. METHODS: Thirty-six patients (mean age 63 years) with rheumatoid atlantoaxial instability were treated with posterior atlantoaxial transarticular screw fixation supplemented with an interspinous C1-2 strut graft-cable construct to provide immediate three-point fixation to facilitate bone fusion. Previous attempts at fusions by using bone grafting with wire fixation at other institutions had failed in six of

Overstrain on the longitudinal band of the cruciform ligament during flexion in the setting of sandwich deformity at the craniovertebral junction: a finite element analysis.

BACKGROUND CONTEXT: In the setting of "sandwich deformity" (concomitant C1 occipitalization and C2-3 nonsegmentation), the C1-2 joint becomes the only mobile joint in the craniovertebral junction. Atlantoaxial dislocation develops earlier with severer symptoms in sandwich deformity, which has been hypothesized to be due to the repetitive excessive tension in the ligaments between C1 and C2. PURPOSE: To elucidate whether and how the major ligaments of the C1-2 joint are affected in sandwich deformity, and to find out the ligament most responsible for the earlier development and severer symptom

[Clinico-radiologic correlations in common neck pain].

Patients with spontaneous neck pain, headache, dizziness and/or pain to the upper limbs are frequently observed. Common cervicodynia, due to the involvement of arthromuscular structures of the cervical spine, was diagnosed in the patients with these symptoms in the absence of trauma or neurologic signs. The authors investigated the clinical-radiologic correlation in a series of 130 symptomatic patients and considered it a metameric disorder. The frequent association of many radiographic signs at the same level often prevents single radiographic signs from being assessed individually; therefore

Microsurgical Unroofing to Secure a Safe Corridor for C1 Screws in Bilateral Ponticulus Posticus: A Case Report.

Ponticulus posticus (PP) is a bony bridge arising from the posterior arch of C1 toward the superior articular process or lateral mass; when complete, it forms an arcuate foramen over the C1 vertebral artery (VA) groove and may convert the VA path into an osseous canal. This anatomy can narrow the usual corridor for C1 lateral mass screw (LMS) insertion. We report a 68-year-old man who sustained an upper cervical cord injury (ASIA Impairment Scale grade [AIS] B) after a fall. Computed tomography (CT) revealed continuous ossification of the posterior longitudinal ligament and anterior longitudin

Crowned dens syndrome combined with cervical disc herniation: A case report and literature review.

RATIONALE: Crowned dens syndrome (CDS) is an unusual and frequently underdiagnosed cause of intense neck pain in older people due to the lack of awareness. The clinical presentation often mimics intracranial infections, which may result in unnecessary invasive procedures and the inappropriate use of antibiotics. The patient had a long-standing history of chronic neck pain, and the acute exacerbation was initially misdiagnosed as cervical disc herniation with suspected intracranial infection before the diagnosis of CDS was established. PATIENT CONCERNS: We report the first case of an 80-year-o

A biomechanical study of unilateral posterior atlantoaxial transarticular screw fixation.

The purpose of this study was to investigate the fixation of C1-C2 instability with the use of a unilateral screw. Transarticular screw placement across C1-C2 may be contra-indicated in up to 20% of specimens on at least one side because of anatomic variations or other pathological processes. Hence the current study looks into unilateral screw fixation of C1- C2 instability. Eight cervical spine specimens, C1 through C5, were harvested from fresh human cadavers (4 male and 4 female) of average age 67 years (54-80). C1 and C2-C5 vertebrae were potted to allow motion only at the C1-C2 articulati

Outcomes of C1 and C2 posterior screw fixation for upper cervical spine fusion.

To achieve stable fixation of the upper cervical spine in posterior fusions, the occiput is often included. With the newer techniques, excluding fixation to the occiput will retain the occiput-cervical motion, while still allowing a stable fixation. Harms's technique has been adapted at our institution and its effectiveness for indications such as C2 complex fractures and tumors using C1 or C2 as endpoints of a posterior fixation are reviewed. Fourteen cases were identified, consisting of one os odontoideum; four acute fractures and four non-unions of the odontoid; three tumors and two complex

Discrepancies of MRI findings between recumbent and upright positions in atlantoaxial lesion. Report of two cases.

Two cases of atlantoaxial (A-A) instability that showed different MRI findings between supine and upright positions are presented. The upright MRI represented the findings corresponding to their symptoms. In A-A lesions, conventional MR images taken in the supine position do not always explain the pathophysiological consequences. The MR images taken in the upright position disclose the actual spinal pathophysiology with gravitational effects.

Vertical translocation: the enigma of the disappearing atlantodens interval in patients with myelopathy and rheumatoid arthritis. Part I. Clinical, radiological, and neuropathological features.

This statistical comparison between patients with cervical myelopathy secondary to horizontal atlantoaxial subluxation and those with vertical translocation is designed to elucidate the mechanisms responsible for cranial settling and the effect of translocation on the development of spinal cord compression. In a 10-year study of a cohort of 256 patients, 186 suffered from myelopathy and 116 (62%) of these exhibited vertical translocation according to the Redlund-Johnell criteria. Vertical translocation occurred after a significantly longer period of disease than atlantoaxial subluxation (p < 0

Radiographic and anatomic basis of endoscopic anterior craniocervical decompression: a comparison of endonasal, transoral, and transcervical approaches.

OBJECTIVE: To evaluate surgical access to the craniocervical junction using 3 endoscopic approaches: endonasal, transoral, and transcervical. METHODS: Nine cadaveric specimens were used. Image guidance was used in 1 specimen for each approach; fluoroscopy was used in every case. The Vitrea imaging station (Vital Images Inc., Minnetonka, MN) was used to evaluate the angles and distances to the target of the approach, centered on the tip of the odontoid. The entry site was defined as: 1) the endonasal approach (inferior midline of the nasal bone), 2) the transoral approach (the tip of the upper

Calcium pyrophosphate dihydrate crystal deposition in the craniovertebral junction.

OBJECT: Calcium pyrophosphate dihydrate (CPPD) deposition is a rare cause of retroodontoid mass lesions in elderly individuals. However, this condition may be severely underdiagnosed if sufficient attention is not paid to imaging characteristics and clinical presentation. The authors sought to evaluate the decision-making process in both the diagnosis and surgical treatment of CPPD. METHODS: A retrospective review of University of Iowa medical records and radiographs accumulated between 1977 and 2006 was performed. The inclusion criterion was histopathological findings consistent with pseudog
