Advances in paediatric urology.

Paediatric urological surgery is often required for managing congenital and acquired disorders of the genitourinary system. In this Series paper, we highlight advances in the surgical management of six paediatric urological disorders. The management of vesicoureteral reflux is evolving, with advocacy ranging from a less interventional assessment and antimicrobial prophylaxis to surgery including endoscopic injection of a bulking agent and minimally invasive ureteric reimplantation. Evidence supports early orchidopexy to improve fertility and reduce malignancy in boys with undescended testes. A

Age at orchidopexy in the UK: has new evidence changed practice?

INTRODUCTION: Research suggesting progressive deterioration in an undescended testis has led to the reduction in the target age for orchidopexy to 6-12 months of age. This age was selected as normal testicular descent is unlikely after 3 months of age and it is timed to prevent early gonadocyte developmental delay as suggested by Hutson and Hasthorpe. This study aims to determine the current age at orchidopexy in one UK training centre and whether changing targets have altered practice. METHODS: The demographics of orchidopexies performed at a single unit between 1998 and 2011 were reviewed.

An atypical presentation of testicular torsion: a case report.

Testicular torsion is an emergent condition requiring prompt surgical intervention to avoid permanent testicular compromise and death. As its symptoms overlap with other disease processes, diagnosis is sometimes difficult. When considering the presenting symptoms, one must be careful not to ignore unusual causes of torsion and misdiagnose the patient. In this case report we describe an unusual etiology of testicular torsion (traumatic) with an atypical patient presentation (mildly painful) which presented many obstacles before proper diagnosis and treatment.

Serum AMH concentration as a marker evaluating gonadal function in boys operated on for unilateral cryptorchidism between 1st and 4th year of life.

The aim of this study was to measure the serum AMH (anti-Mullerian hormone) concentrations in a group of boys with or without cryptorchidism, evaluation of karyotypes, testicular position, morphology, and major length of the undescended testes. Fifty boys who were 1-4 years old (median = 2.4 years) with unilateral cryptorchidism were evaluated. All of them underwent orchidopexy in 2010. Prior to the procedure, all of the subjects had undergone karyotyping to exclude chromosomal abnormalities. Fifty healthy boys within the same age range (median = 2.1 years) admitted for planned inguinal hernia

Laparoscopic management of persistent mullerian duct syndrome.

Persistent mullerian duct syndrome (PMDS), characterized by the presence of mullerian structures in a virilized male, frequently presents as undescended testis, either intraabdominal or within a hernial sac. We describe a 10-month-old infant with PMDS successfully managed by the laparoscopic approach. At the age of 1.5 months, the patient presented with a left inguinal hernia and bilateral nonpalpable gonads in another center and underwent left inguinal exploration. The uterus and a gonadlike structure along with the hernia sac were found in the inguinal canal. Left inguinal herniotomy was per

Laparoscopic management of transverse testicular ectopia with spermatic cord fusion: a case report.

BACKGROUND: Transverse testicular ectopia (TTE) is a rare congenital anomaly where both testes descend through the same inguinal canal and are located on the same side. Cases with fused spermatic cords are particularly rare and present unique surgical challenges. CASE DESCRIPTION: We report the case of an 8-month-old male infant with bilateral cryptorchidism and an empty scrotum since birth. Preoperative ultrasonography identified two testicular-like structures near the right internal inguinal ring. Laparoscopic exploration revealed bilateral patent processus vaginalis, with both testes locat

Transverse testicular ectopia with an inguinal hernia: A rare case report.

INTRODUCTION: Transverse testicular ectopia (TTE) is a rare congenital condition characterized by migration of both testes through the same inguinal canal and often presents with an inguinal hernia. TTE is associated with various genitourinary anomalies. CASE PRESENTATION: A three-year-old boy presented with a non-palpable right testis and a palpable undescended left testis in the left inguinal area. Ultrasound (US) indicated the presence of both testes in the left inguinal canal. In surgery, the two testes were found with separated cord and one hernia sac which was dissected and ligated thus

An unusual presentation of cryptorchidism: A case report of Spigelian-cryptorchidism syndrome.

INTRODUCTION: Spigelian hernia is an uncommon hernia presenting as a protrusion of abdominal contents through the spigelian fascia, lateral to the rectus abdominis. In some rare cases, Spigelian hernia can occur alongside cryptorchidism, which forms a recognized syndrome found in male infants with Spigelian hernia. This is a relatively unreported syndrome with very limited literature available regarding it, none of which is reported in Pakistan in adults. PRESENTATION OF CASE: We report a case of a 65-year-old male with right sided obstructed spigelian hernia along with the rare finding of te

A case of splenogonadal fusion in the left testis.

Splenic tissue outside the normal anatomical site can be collectively referred to as ectopic spleen. Clinically, the commonest causes of ectopic spleen include accessory spleen, splenic tissue implantation, and splenogonadal fusion (SGF). Accessory spleen is mostly caused by congenital dysplasia, is mostly located near the spleen, and may be supplied by the splenic artery. Splenic implantation is mostly caused by autologous spleen tissue transplantation caused by trauma or surgery. SGF is the abnormal fusion of the spleen with the gonad or with the mesonephric derivatives. As a rare developmen

Seminoma arising in splenogonadal fusion: a case report and literature review.

BACKGROUND: Splenogonadal fusion (SGF) is a rare congenital malformation in which the spleen is abnormally connected to the gonads or to the mesonephric derivatives. There is no obvious causality between SGF and testicular neoplasm. However, cryptorchidism, which is a well-known risk factor of testicular germ cell tumors, are the most frequent malformations associated with SGF. To our knowledge, there are only four reported cases of SGF associated with testicular neoplasm so far. Herein, we reported a patient of this condition, and briefly reviewed the related literature. CASE PRESENTATION: A

Atypical symptoms of testicular torsion in a 7-year-old child: A case report.

RATIONALE: Testicular torsion accounting for 25% of acute scrotal disease, is an acute surgical condition. Atypical presentations of testicular torsion may lead delay diagnosis. PATIENT CONCERNS: A 7-year-old boy was admitted to the pediatric emergency department with continuous and progressive left scrotal pain for 2 days, associated symptoms and signs included left scrotal swelling and erythema. The pain started 4 days ago as left lower abdominal pain which then migrated to the left scrotum. DIAGNOSES: Physical examination showed left scrotum skin redness, swelling, local heat, tenderness,

Mean platelet volume is the most valuable hematologic parameter in differentiating testicular torsion from epididymitis within the golden time.

BACKGROUND: We aimed to assess the diagnostic value of hematologic parameters in the differential diagnosis of testicular torsion and epididymitis within and after the golden time. METHODS: We retrospectively reviewed the records of 250 patients aged <25 years who were diagnosed with epididymitis (n=119) or testicular torsion (n=131). The characteristics and hematologic parameters of patients in the two groups were analyzed. Receiver operating characteristic (ROC) curves were used to assess the validity of hematologic parameters as differential diagnostic tools with respect to the golden time

Transverse testicular ectopia associated with persistent Mullerian duct syndrome in infertile male: two case reports and literature review.

Transverse testicular ectopia (TTE) associated with persistent Mullerian duct syndrome (PMDS) is a rare form of male pseudohermaphroditism usually unexpectedly found at surgery for cryptorchidism or inguinal hernia in children. Its etiology and prevalence are unclear, although defects in the gene that encodes anti-Mullerian hormone (AMH) or AMH receptor has been generally considered as the major cause. Adult cases of TTE associated with PMDS are even more peculiar, as the adult patients usually present more complex medical history, require more comprehensive medical examination and management.

Alcohol binge drinking during pregnancy and cryptorchidism.

BACKGROUND: Recent studies have suggested gestational weeks 8-14 as a time window of particular importance to the intrauterine development of the male genitalia, and prenatal exposure to alcohol is under suspicion as a risk factor for cryptorchidism. We examined if prenatal exposure to alcohol, and especially binge drinking, during the suggested programming window is associated with an increased risk of cryptorchidism. METHODS: The authors used data on 41 268 live born singleton boys of mothers who were enrolled into the Danish National Birth Cohort in 1996-2002. During early childhood, 1598

Testicular histology in boys with Prader-Willi syndrome: fertile or infertile?

PURPOSE: Prader-Willi syndrome is associated with hypogonadism. Cryptorchidism is found in 93% of cases and considered a phenotypic criterion. Men with Prader-Willi syndrome are thought to be infertile. To study the fertility probability in boys with Prader-Willi syndrome we analyzed testicular histology in 8 prepubertal boys and 1 man. MATERIALS AND METHODS: Eight boys 16 months to 14 years old with a proven molecular diagnosis of Prader-Willi syndrome, including 6 with a deletion on chromosome 15 and 2 with uniparental maternal disomy of chromosome 15, underwent orchiopexy and the man under

The true incidence of cryptorchidism in Denmark.

PURPOSE: Considerable controversy exists regarding the recently published high prevalence of cryptorchidism and the actual orchiopexy rate in Denmark. Therefore, we carried out another prevalence study. MATERIALS AND METHODS: The study population included 1,094 consecutive males delivered alive at Glostrup University Hospital during 2002, with a 4-year review of patient data and telephone contact with primary physicians. RESULTS: At birth the overall frequency of cryptorchidism was 2.4% (26 of 1,088 cases, 23% bilateral). The frequency of cryptorchidism in singletons was 2.4% (24 of 1,012 pa

Vascular anatomy of normal and undescended testes: surgical assessment of anastomotic channels between testicular and deferential arteries.

OBJECTIVES: To demonstrate the vascular anatomy of the normal and undescended testes, emphasizing the surgical importance of the anastomotic channels between the testicular and deferential arteries in the undescended testis. METHODS: This study was performed on four normal testes obtained from adult cadavers and the undescended testes taken from men undergoing unilateral orchiectomy. To show the arteries and their connections, we used the injection-corrosion casting technique. RESULTS: The normal and undescended testes measured 36.3 +/- 9.2 mm x 21.2 +/- 4.3 mm x 14.3 +/- 5.4 mm and 19 x 16

Comparison of the LMA-ProSeal and LMA-Classic in children.

BACKGROUND: The LMA-ProSeal is a new laryngeal mask airway with a rear cuff and drainage tube that allows a higher seal pressure than the LMA-Classic for the same intra-cuff pressure, and it permits drainage of gastric secretions and access to the alimentary tract. The LMA-ProSeal can be used in children but it does not have a rear cuff. This study compared the LMA-ProSeal and the LMA-Classic in children for ease of insertion, airway sealing pressure and fibre-optic visualization. METHODS: Sixty ASA I-II children undergoing herniorrhaphy, orchiopexy or myringotomy were included. The patients

The ascending testis and the testis undescended since birth share the same histopathology.

PURPOSE: The etiology of the ascending testis is controversial. We propose that ascending testis, defined as a testis previously thought to be descended and later noted to be out of the scrotum, is due to mild hypogonadotropic hypogonadism affecting both testes. The diagnosis of these low types of true undescended testes is difficult to make clinically in children since they are frequently confused with retractile testes. In this study we compared testicular biopsies in a group of boys with ascending testes with those in boys who had an undescended testis since birth (primary undescended testi
