Does mechanical bowel preparation have a role in preventing postoperative complications in elective colorectal surgery?

BACKGROUND: mechanical bowel preparation (MBP) consists of orthograde fluid preparation to clean the bowel. MBP is considered to prevent postoperative complications. METHODS: meta-analysis of prospective randomised clinical trials (RCT) evaluating MBP versus no MBP. RESULTS: following a medline search we retrieved 15 prospective trials of which only 5 where RCT comparing MBP versus no MBP in elective colorectal surgery. For the randomised studies, there were respectively 186, 179, 149, 267 and 380 patients, including all type of resections. The total number of patients in these 5 studies was

Prevalence and contributing factors of eating disorder behaviors in active duty service women in the Army, Navy, Air Force, and Marines.

Eating disorders continue to be studied among civilian women. Gross disturbances in eating behaviors characterize the condition of anorexia nervosa (AN), currently seen among 1 to 2% of non-active duty women. Bulimia nervosa (BN) is prevalent among 2% of the female population, and both disorders have a female-to-male ratios of 10:1. Another category of eating disorders known as not otherwise specified (NOS) occurs in 3 to 35% of individuals in the reported literature. This study examined the prevalence of AN, BN, and NOS among a large sample of active duty women currently serving in the Army,

[Analgesics and laxatives as risk factors for cancer in the efferent urinary tract--results of the Berlin Urothelial Carcinoma Study].

A retrospective case-control study (1990-1995), the Berlin Urothelial Cancer Study (BUS), examined analgesics and laxatives as risks for the induction of urothelial cancer in renal pelvis, ureter and bladder. Especially for renal pelvis cancer could observe substance and dose specific risk of compound analgesics. The analgesic substances Phenacetin, Paracetamol, Acetylsalicylic acid (ASA) and Pyrazolones were assessed. Besides a risk of contact laxatives (chemical or anthranoide ingredients) for urothelial cancer was found, not yet described. The highest risk shows the anthranoide plant Senna.

A comparative trial of magnesium citrate (Citramag) and Picolax for barium enema bowel preparation.

It has previously been suggested that a single dose of magnesium citrate produces adequate cleansing of the bowel for barium enema examination. To determine whether such a regimen has any advantage over the widely used two doses of Picolax, a prospective randomized blinded trial was performed with over 100 patients allocated to each of the bowel preparation regimens. Radiographs were assessed for quality of mucosal coating, faecal residue and overall quality of bowel preparation. Patient compliance was good with both regimens. Normal daily activities were inconvenienced significantly more by t

Comparison of sennoside A + B and sodium picosulfate-based regimens for bowel preparation: a prospective randomized trial.

AIM: Adequate bowel preparation is essential for high-quality colonoscopy and directly affects key quality indicators such as adenoma detection and cecal intubation. Although low-volume, nonpolyethylene glycol (PEG) regimens are widely used in clinical practice, real-world comparative data between stimulant-based preparations and sodium picosulfate-based combinations remain limited. This study aimed to compare the effectiveness and tolerability of sennoside A + B versus sodium picosulfate/magnesium oxide/citric acid (PM/Ca) using the Boston Bowel Preparation Scale (BBPS). METHODS: This prospe

Mechanical bowel preparation plus oral antibiotics reduces surgical site infection and anastomotic leak rates in elective colorectal cancer surgery: A systematic review and meta-analysis of randomised controlled trials.

BACKGROUND: Surgical site infection (SSI) and anastomotic leak (AL) are major contributors to postoperative morbidity in elective colorectal surgery. Although the addition of oral antibiotics (oAB) to mechanical bowel preparation (MBP) is recommended by several professional societies, uncertainty remains about its effectiveness and hence uptake globally is inconsistent, particularly in patients undergoing surgery for colorectal cancer (CRC). METHODS: We conducted a systematic review and meta-analysis (PROSPERO CRD420251055720) of randomised controlled trials comparing MBP plus oAB with MBP al

Postoperative outcomes following mechanical bowel preparation before proctectomy: a meta-analysis.

AIM: Previous meta-analyses of mechanical bowel preparation (MBP) before colorectal surgery have grouped colon and rectal resection together. An increased postoperative morbidity has been suggested in the absence of MBP following proctectomy. The current study used meta-analytical techniques to evaluate the comparative outcome of patients who received MBP prior to proctectomy. METHOD: A comprehensive search was performed for published studies examining the effect of MBP before proctectomy on patient outcome. Random effects methods were used to combine data. RESULTS: Eleven studies including

Development of expertise in the detection and classification of non-polypoid colorectal neoplasia: Experience-based data at an academic GI unit.

At its core, quality improvement in gastrointestinal (GI) practice relies on continuous training, education, and information among all health care providers, whether gastroenterologists, GI trainees, endoscopy nurses, or GI pathologists. Over the past few years, it became clear that objective criteria are needed to assess the quality of colonoscopy, such as cecum intubation rate, quality of bowel preparation, withdrawal time, and adenoma detection rate. In this context, development of competence among practicing endoscopists to adequately detect and treat non-polypoid colorectal neoplasms (NP-

Toxicity of phosphate enemas - an updated review.

INTRODUCTION: Enemas containing phosphate are widely prescribed and may cause important adverse effects. A systemic review published in 2007 reported the literature on the adverse effects of phosphate enemas from January 1957 to March 2007 and identified 12 deaths. These were thought due to electrolyte disturbances, heart failure and kidney injury. These data raised concerns about the use of phosphate enemas in routine practice. Newer osmotic-based enema alternatives are now available that do not contain absorbable ions. We sought to review the literature since this review and evaluate the lat

Educational Colonoscopy Video Enhances Bowel Preparation Quality and Comprehension in an Inner City Population.

BACKGROUND: Quality of bowel preparation and patient knowledge remains a major barrier for completing colorectal cancer screening. Few studies have tested unique ways to impact patient understanding centering on interactive computer programs, pictures, and brochures. Two studies explored instructional videos but focused on patient compliance and anxiety as endpoints. Furthermore, excessive video length and content may limit their impact on a broad patient population. No study so far has studied a video's impact on preparation quality and patient understanding of the colonoscopy procedure. MET
