Comparison of ultra-ultrabrief and ultrabrief pulse widths in right unilateral electroconvulsive therapy: A randomized trial.

BACKGROUND: Ultrabrief stimulation in electroconvulsive therapy (ECT) using a 0.25 or 0.30 ms pulse width markedly reduces the charge required to reach the seizure threshold (ST) and cognitive side effects. It is not known whether further reduction of pulse width to 0.15 ms is advantageous. METHODS: Thirty-seven patients were randomized to ST titration at the first session applying right unilateral (RUL) ECT with either a 0.15 or 0.30 ms pulse width and were titrated again in the second session using the alternative pulse width. All subsequent treatments used the pulse width applied in the se

The effects of treatment, clinical and demographic factors on recovery of orientation after ECT: A care network study.

BACKGROUND: Time to reorientation after electroconvulsive therapy (ECT) has been shown to predict retrograde amnesia and is a useful measure for monitoring patients over the acute treatment course. This study investigated the effects of treatment, clinical and demographic factors on the recovery of orientation after ECT. METHODS: Data from 555 ECT patients across two different clinical CARE Network sites were analysed. The main outcome variable was recovery of orientation on the 10-Item Orientation Questionnaire assessed after every ECT treatment. A linear mixed-effects repeated measures mode

Autobiographical Memory Specificity in Major Depression Treated With Electroconvulsive Therapy.

OBJECTIVE: Autobiographical memory in major depression is characterized by reduced specificity, which reflects the tendency to summarize categories of events rather than recall specific instances of events situated in a time and place. This widely studied cognitive marker for depression has not been extensively examined in patients treated with electroconvulsive therapy (ECT). METHODS: We conducted a retrospective chart review of a naturalistic cohort of patients receiving a course of brief-pulse predominantly bitemporal ECT for a major depressive episode. Patients completed the recent life s

Visual imagery deficits, impaired strategic retrieval, or memory loss: disentangling the nature of an amnesic person's autobiographical memory deficit.

Conclusions about the duration of hippocampal contributions to our autobiographical record of personal episodes have come under intense scrutiny in recent years. Interpretation is complicated by such factors as extent and site of lesions as well as test sensitivity. We describe the case of an amnesic person, K.C., with large, bilateral hippocampal lesions who figured prominently in the development of theories of remote memory due to his severely impoverished autobiographical memory extending across his entire lifetime. However, the presence of lesions in higher-order visual cortex raises the p

[A case of cerebral infarction presenting as retrosplenial amnesia].

We report a 73-year-old right-handed female who presented with an acute amnesic syndrome. On November 18, 1991, she was admitted to a local hospital complaining of sudden-onset vertigo and nausea, but immediately after the admission she developed an amnesic syndrome. On November 27, she was transferred to our hospital for further assessment of her memory disturbance. Neurologically she was normal except for mild right hemianopsia and increased deep tendon reflexes in the extremities. Neuropsychological assessments were performed over 3 weeks. She was always alert, attentive, and cooperative. S

Acute psychological symptom profiles in high school athletes following sport-related concussion.

OBJECTIVE: Psychological symptoms following a sport-related concussion may affect recovery in adolescent athletes. Therefore, the aims of this study were to 1) describe the proportion of athletes with acute psychological symptoms, 2) identify potential predictors of higher initial psychological symptoms, and 3) determine whether psychological symptoms affect recovery in a cohort of concussed high school athletes. METHODS: A retrospective cohort study of high school athletes (14-18 years of age) who sustained a sport-related concussion from November 2017 to April 2022 and presented to a multid

Head-up tilt testing in older syncope patients: a systematic review.

BACKGROUND: In older syncope patients, medical histories are often less reliable due to retrograde amnesia and cognitive impairment. Therefore, additional tests may be needed to reach a diagnosis. We conducted a systematic review to evaluate positivity rates and safety of head-up tilt testing (HUTT) in these patients. METHODS: We searched Medline and Embase for HUTT positivity rates and diagnoses in older syncope patients (mean age ≥ 65 years) vs. younger patients. Secondary outcomes were time to syncope (TtS) and adverse events (AEs). Risk of bias was assessed with a modified version of the

Amnesia after Repeated Head Impact Is Caused by Impaired Synaptic Plasticity in the Memory Engram.

Subconcussive head impacts are associated with the development of acute and chronic cognitive deficits. We recently reported that high-frequency head impact (HFHI) causes chronic cognitive deficits in mice through synaptic changes. To better understand the mechanisms underlying HFHI-induced memory decline, we used TRAP2/Ai32 transgenic mice to enable visualization and manipulation of memory engrams. We labeled the fear memory engram in male and female mice exposed to an aversive experience and subjected them to sham or HFHI. Upon subsequent exposure to natural memory recall cues, sham, but not

In search of the 'self': Holistic rehabilitation in restoring cognition and recovering the 'self' following traumatic brain injury: A case report.

BACKGROUND: Following mild-moderate traumatic brain injury (TBI), an individual experiences a range of emotional changes. It is often difficult for the patient to reconcile with their post-injury persona, and the memory of pre-injury personhood is particularly painful. Insight into one's cognitive deficits subsequent to injury can lead to an existential crisis and a sense of loss, including loss of self. OBJECTIVE: Restoration of cognitive functions and reconciliation with loss of pre-traumatic personhood employing a holistic method of neuropsychological rehabilitation in a patient suffering

The benefits and costs of changing treatment technique in electroconvulsive therapy due to insufficient improvement of a major depressive episode.

BACKGROUND: Electroconvulsive therapy (ECT) technique is often changed after insufficient improvement, yet there has been little research on switching strategies. OBJECTIVE: To document clinical outcome in ECT nonresponders who were received a second course using high dose, brief pulse, bifrontotemporal (HD BP BL) ECT, and compare relapse rates and cognitive effects relative to patients who received only one ECT course and as a function of the type of ECT first received. METHODS: Patients were classified as receiving Weak, Strong, or HD BP BL ECT during three randomized trials at Columbia Un

Consolidation of spatial memory in the rat: Findings using zeta-inhibitory peptide.

Whether or not spatial memories reorganize in the rodent brain is an unanswered question that carries the importance of whether the rodent provides a suitable animal model of human retrograde amnesia. The finding of equally impaired recent and remote spatial memory could reflect the continued importance of the hippocampus for spatial memory or a performance deficit (for example, hippocampal lesions may impair the rat's ability to use distal spatial cues to navigate to a specific point in space). In the current study, we tested recent and remote spatial memory in rats following hippocampal ZIP

Objective cognitive performance associated with electroconvulsive therapy for depression: a systematic review and meta-analysis.

BACKGROUND: Electroconvulsive therapy (ECT) is the most acutely effective treatment for depression, but is limited by cognitive side effects. However, research on their persistence, severity, and pattern is inconsistent. We aimed to quantify ECT-associated cognitive changes, specify their pattern, and determine progression. METHODS: MEDLINE, EMBASE, PsycArticles, PsychINFO, PsychLIT, and reference lists were systematically searched through January 2009. We included all independent, within-subjects design studies of depressed patients receiving ECT where cognition was assessed using standardiz

Effects of pulse width and electrode placement on the efficacy and cognitive effects of electroconvulsive therapy.

BACKGROUND: While electroconvulsive therapy (ECT) in major depression is effective, cognitive effects limit its use. Reducing the width of the electrical pulse and using the right unilateral electrode placement may decrease adverse cognitive effects, while preserving efficacy. METHODS: In a double-masked study, we randomly assigned 90 depressed patients to right unilateral ECT at 6 times seizure threshold or bilateral ECT at 2.5 times seizure threshold, using either a traditional brief pulse (1.5 ms) or an ultrabrief pulse (0.3 ms). Depressive symptoms and cognition were assessed before, duri

Evolving the SCAT5 for Ruling Out Higher-Severity Traumatic Brain Injuries-Can Decision Rules Developed for Emergency Settings Help?

BACKGROUND: Decision rules (eg, Canadian computed tomography head rule [CCHR] for adults and Pediatric Emergency Care Applied Research Network [PECARN] rule for children/adolescents) are used in emergency settings (emergency room [ER] rules) to assess traumatic brain injuries (TBIs). The rules have a high-sensitivity and near-perfect negative predictive value that help to rule out more severe TBI. CLINICAL QUESTION: Which criteria should be added to the Sport Concussion Assessment Tool 5 (SCAT5) to reach the sensitivity of the ER rules and improve the utility of the SCAT5 for screening for hig

The Korsakoff syndrome.

BACKGROUND: Investigations of the Korsakoff syndrome by researchers from different disciplines have proliferated in recent years, making it apposite to review the various findings. METHOD: This review is based on the author's knowledge of reports in the major clinical and neuropsychological journals, supplemented by Medline searches to update particular subtopics. RESULTS: The Korsakoff syndrome is defined as a disproportionate impairment in memory, relative to other aspects of cognitive function, resulting from a nutritional (thiamine) depletion. Autobiographical memory in mild cognitive impairment and Alzheimer's disease: a comparison between the Levine and Kopelman interview methodologies.

Previous studies have produced inconsistent results concerning the two components of autobiographical memory--personal semantic memory and episodic memory. Results in subjects with mild cognitive impairment (MCI) and dementia of Alzheimer's type (DAT) have varied concerning the existence of a temporal gradient in retrograde amnesia. These results have important theoretical implications regarding multiple trace theory versus standard consolidation models of long-term memory (LTM). We investigated whether this variability arises from differences in the methods used in assessing autobiographical

The effects of hypertonic saline and nicotinamide on sensorimotor and cognitive function following cortical contusion injury in the rat.

Hypertonic saline (HTS) is an accepted treatment for traumatic brain injury (TBI). However, the behavioral and cognitive consequences following HTS administration have not thoroughly been examined. Recent preclinical evidence has suggested that nicotinamide (NAM) is beneficial for recovery of function following TBI. The current study compared the behavioral and cognitive consequences of HTS and NAM as competitive therapeutic agents for the treatment of TBI. Following controlled cortical impact (CCI), bolus administrations of NAM (500 mg/kg), 7.5% HTS, or 0.9% saline Vehicle (1.0 mL/kg) were gi
