Surgical treatment of double and triple heart valve disease through a limited single-access right minithoracotomy.

Heart valve disease can be extensive and may include double (mitral-aortic, mitral-tricuspid), or triple (mitral, aortic, and tricuspid) valvular regurgitation. The surgical correction of significant valvular regurgitation usually consists of the repair or replacement of all valves affected by a pathologic process. The median full-length sternotomy still serves as a classic approach for single, double, and triple valve operations in most patients. Here, we present a minimally invasive approach for the surgery of double and triple heart valve disease through a limited single-access right minith

Translocation of the supra-aortic vessels and stent grafting of the aortic arch.

This is a report on the combined surgical and endovascular treatment of a very large aortic arch aneurysm. In the first step, total arch rerouting was performed with an inversed bifurcated Dacron prosthesis. Brachiocephalic trunk and left subclavian artery were inserted into the two branches of the prosthesis via an end-to-end anastomosis and left common carotid was reinserted into the branch to the left subclavian. In the second step, thoracic endovascular aortic repair was performed. By this combined concept a stable and durable result could be achieved.

Surgery for tetralogy of Fallot - absent pulmonary valve syndrome. Technique of anterior translocation of the pulmonary artery.

Patients with absent pulmonary valve syndrome (APVS) and tetralogy of Fallot represent a particular management's challenge. The distinctive feature of APVS is the airway obstruction caused by tracheobronchial compression that results from massive dilatation of the main pulmonary artery (PA) and its first- and second-order branches. Consequential tracheomalacia and bronchomalacia determine the timing and severity of respiratory compromise, as well as the morbidity and mortality of these patients. Although several techniques have been proposed for decompression of the airways, the method of choi

Postpneumonectomy syndrome.

Postpneumonectomy syndrome is a rare postoperative complication whereby mediastinal shifting toward the pneumonectomy space results in bronchial compression between the pulmonary artery, aorta, and vertebral column. This syndrome is more common after right pneumonectomy; other risk factors include young age and female sex. Imaging studies consistently reveal massive mediastinal shifting and document airway compromise. Bronchoscopy and flow-volume loops are helpful in confirming the diagnosis. Other causes of dyspnea, including cancer recurrence, should be excluded. Definitive treatment involve

Surgery for malposition of the great arteries: the REV procedure.

The REV procedure was introduced in 1980 to treat transposition of the great arteries with ventricular septal defect (VSD) and pulmonary stenosis and malpositions similar to transposition of the great arteries (TGA). It aims at overcoming the drawbacks and limitations of the classic Rastelli operation, such as subaortic stenosis, late ventricular deterioration, arrhythmias and sudden death. In particular, the resection of the infundibular septum allows for the placement of a straighter, smaller ventricular patch, bulging much less in the right ventricular cavity. The extensive mobilization of

Colon interposition for oesophageal replacement.

The choice of the colon as an oesophageal substitute results primarily from the unavailability of the stomach. However, given its durability and function, colon interposition keeps elective indications in patients with benign or malignant oesophageal disease who are potential candidates for long survival. The choice of the colonic portion used for oesophageal reconstruction depends on the required length of the graft, and the encountered colonic vascular anatomy, the last being characterised by the near-invariability of the left colonic vessels, in contrast to the vascular pattern of the right

Laparoscopic Nissen fundoplication.

The dissection is started performing a crural dissection with visualization and preservation of both vagus nerves, followed by circumferential dissection of the esophagus at the gastro-esophageal junction. Adequate intra-mediastinal mobilization of the esophagus is performed to obtain 3-4 cm of intra-abdominal esophagus without undue downward traction on the cardia or stomach. The gastric fundus is then mobilized through adequate short gastric vessel division. The left and right pillars of the right diaphragmatic crus are approximated using interrupted sutures. A short (≪2 cm), floppy 360° fun

Nikaidoh operation for transposition of the great arteries with a ventricular septal defect and pulmonary stenosis.

The optimal surgical management of patients with transposition of the great arteries with a ventricular septal defect and pulmonary stenosis remains challenging. When compared to other surgical options, the Nikaidoh procedure results in a 'more normal' anatomic result, with better alignment of the right and left ventricular outflow tracts. Also, the pulmonary outflow is less likely to be compressed by the sternum, a major issue associated with the Rastelli repair. This technique is especially useful in the presence of an inlet or restrictive ventricular septal defect, a hypoplastic right ventr

Ebstein's anomaly: the Broussais approach.

Ebstein's anomaly is rare, but it is the most frequent cause of congenital tricuspid valve anomaly. For many years valve replacement was performed. Conservative techniques are now preferred due to improvement of the results. The goals of surgery are to restore a normal tricuspid valve function, to preserve the right ventricular contractility and to decrease the risk of rhythm disturbances. Basically, the technique is based on mobilization of the anterior leaflet and longitudinal plication of the right ventricle. A bidirectional cavo pulmonary shunt is used in severe cases. Results are correlat

Innominate artery cannulation.

Innominate artery cannulation is indicated mainly for surgery which involves aortic root and aortic arch when pathology precludes standard cannulation of the ascending aorta. Thus, it can be used as the procedure of choice in cases of ascending aortic aneurysm, type A aortic dissection, porcelain aorta and reoperation for coronary or aortic surgery. It is performed through a standard median sternotomy, hence avoiding the complications of a second incision. It provides antegrade systemic and cerebral flow that is associated with improved distal organ perfusion and reduced rate of retrograde cer

Reconstruction of the intercostal arteries with small-branched grafts in patients with thoracoabdominal or descending aortic aneurysms.

An extent II thoracoabdominal aortic aneurysm of 60 mm diameter was exposed through the left 6(th) intercostal space and retroperitoneal approach. The partial cardiopulmonary bypass was initiated through the femoral arterial and venous cannulation. A knitted Dacron graft of 22 mm with four spatially orientated branches for the abdominal viscera and five branches for the intercostal arteries was utilized. The thoracoabdominal aorta was replaced with staged segmental aortic clamping. The proximal aorta, just distal to the left subclavian artery, was completely transected and anastomosed to the g

Neonatal aortic arch surgery.

Surgical repair of the aortic arch is entailed in the neonatal period of patients with: hypoplastic left heart syndrome, interrupted aortic arch, hypoplastic aortic arch and complex aortic coarctation. Aortic arch surgery requires a period of circulatory arrest and deep hypothermia. Cerebral selective perfusion has recently been introduced as an alternative to circulatory arrest with the aim of reducing mortality and neurological complications. Moreover, the arch reconstruction phase can be safely performed under moderate hypothermia and with cerebral and myocardial perfusion (on beating heart

Total anomalous pulmonary venous connection.

Total anomalous pulmonary venous connection (TAPVC) is subdivided into four types: supracardiac, cardiac, infracardiac, and mixed. The principle of operative repair is to establish an unobstructed communication between the pulmonary veins and the left atrium, interrupt the connections with the systemic venous circulation, and remove intracardiac shunting. The specific repair is dependent on the type of anomalous connection. Pulmonary venous obstruction is a significant and complex complication following TAPVC repair. The introduction of the sutureless pericardial marsupialization technique has

Endomyocardial biopsy - jugular/subclavian vein approach.

Endomyocardial biopsy (EMB) is a diagnostic procedure mainly to survey the sufficiency of immunosuppressive therapy after cardiac transplantation. Other indications for EMB remain controversial. After insertion of an introducer sheet in Seldinger's technique, four to six biopsies are taken from the right ventricle by fluoroscopic guidance. EMB is a very safe operation with a low complication rate which can be rapidly performed with little inconvenience for the patient if performed by a skilled surgeon.

Orthotopic heart transplantation for congenital heart defects: situs inversus.

Although rare, visceroatrial situs inversus is not exceptional in patients with complex congenital heart defects. Achieving orthotopic heart transplantation using a graft harvested in a donor with situs solitus is surgically demanding. Technical problems can, however, be overcome by adequate harvesting of donor heart and by the use of innovative reimplantation techniques. These include: separate right and left pulmonary venous anastomoses, creation of an atrio-pericardial tunnel for reimplantation of the inferior vena cava, extracardiac reconstruction of the superior vena cava pathway using th

The right gastroepiploic artery graft.

This is the presentation of coronary artery bypass grafting with gastroepiploic artery (GEA) in both skeletonized GEA harvesting technique and anastomosis: After median sternotomy and harvesting internal thoracic artery, the median incision is extended less than one inch. Following laparotomy, dissection of GEA is started from two third distal of the great curvature of the stomach and ended above the pylorus. The GEA is passed into pericardial cavity through a small tunnel in the diaphragm. The most common site of the anastomosis is distal right coronary artery (postero-descending or atriovent

Lobectomy of the right lower lobe for lung cancer.

Lobectomy is the treatment of choice for primary non-small cell lung cancer (NSCLC), provided that the patient is fit enough to undergo surgery, the primary tumour is confined to one lobe and there are no distant metastases. Other indications for lobectomy include metastatic disease (with multiple nodules in one lobe or central localisation of metastasis), centrally located benign tumours (such as hamartoma), extensive infectious diseases (such as lung abcess, bronchiectasis), and congenital anomalies (such as congenital cystic adenomatoid malformation-CCAM). A lobectomy of the right lower lob

Resection of intrathoracic and subdiaphragmatic hydatid cysts.

The goal of surgical therapy in pulmonary hydatid disease is to remove the cyst while preserving as much lung tissue as possible. The surgical method may be different in the intact (simple) and ruptured (complicated) cysts. The operation has two steps: a) removal of the germinative layer, b) management of the residual pulmonary cavity. Simple cysts are generally removed after needle aspiration or enucleation without needle aspiration. Enucleation cannot be performed in ruptured cysts. The lung cavity that remains after removal of the cyst may be left as it is or obliterated by sutures from wit

Extended-interval dosing of rituximab/ocrelizumab is associated with a reduced decrease in IgG levels in multiple sclerosis.

The potential benefits of extended-interval dosing (EID) of rituximab (RTX) or ocrelizumab (OCR) in mitigating the reduction of immunoglobulin levels and decreasing the risk of infection in persons with relapsing-remitting multiple sclerosis (pwRRMS) remain largely unknown. We retrospectively analyzed two structured data collections including pwRRMS who were prescribed RTX/OCR using different interval dosing regimens, a 6-month standard-interval dosing (SD) or EID. The SD and EID cohorts included 88 and 271 pwRRMS, respectively, with a mean (SD) treatment duration of 3.5 (1.3) and 4.4 (1.5) ye

Mobile phone text messaging for medication adherence in secondary prevention of cardiovascular disease.

BACKGROUND: Cardiovascular diseases (CVDs) are the leading cause of death globally, accounting for almost 18 million deaths annually. People with CVDs have a five times greater chance of suffering a recurrent cardiovascular event than people without known CVDs. Although drug interventions have been shown to be cost-effective in reducing the risk of recurrent cardiovascular events, adherence to medication remains suboptimal. As a scalable and cost-effective approach, mobile phone text messaging presents an opportunity to convey health information, deliver electronic reminders, and encourage beh
