Evaluating the incidence of nephrolithiasis surgery before and after parathyroidectomy.

PURPOSE: Primary hyperparathyroidism (PHPT) patients who undergo parathyroidectomy (PTX) have significantly reduced rates of kidney stone events based on imaging and patient-reported history. However, there has been limited knowledge on the impact of PTX on stone disease requiring surgical intervention. The aim of this study was to compare the incidence of kidney stone surgery before and after PTX. METHODS: PHPT patients who underwent PTX from 2015-2023 at our institution were retrospectively reviewed. We evaluated the incidence of kidney stone treatment by shockwave lithotripsy (SWL), ureter

Patients treated for uric acid stones reoccur more often and within a shorter interval compared to patients treated for calcium stones.

INTRODUCTION: We aimed to investigate the association between stone composition and recurrence rate in a well-characterized group of patients. METHODS: From our prospectively assembled database of 1328 patients undergoing ureteroscopy and percutaneous nephrolithotomy (PCNL) between 2010 and 2015, we identified 457 patients who met the inclusion criteria: a minimum of two years' followup, stone-free status following surgery, normal anatomy, and Fourier transform infrared (FT-IR) stone analysis results. Stone recurrence was identified by kidney-ureter-bladder (KUB) or an ultrasound (US). All sy

Endoscopic and histologic findings in a cohort of uric acid and calcium oxalate stone formers.

OBJECTIVE: To characterize the endoscopic and histologic renal papillary lesions in a cohort of uric acid (UA) stone formers (SF). METHODS: Data were prospectively obtained during percutaneous nephrolithotomy between 2009 and 2013. Renal papillae were endoscopically analyzed to quantitate surface area occupied by plaque or plug, and biopsies were obtained. UA SF were compared with non-SF controls and patients with >50% calcium oxalate (CaOx) in the absence of UA. RESULTS: There were 23 UA SF; of which 19 stones (83%) were admixed with CaOx and 4 (17%) were pure. Compared with CaOx SF and con

Variation in urine composition in the human urinary tract: evidence of urothelial function in situ?

PURPOSE: An increased awareness of the concept that the urothelium has a significant transport function led us to question whether urine composition changes as it passes along the human lower urinary tract. MATERIALS AND METHODS: Urine samples from the bladder and renal pelvis were collected from 30 adults who underwent percutaneous nephrolithotomy (27) or ureteral stent insertion before lithotripsy (3). Urine was obtained from the 2 renal pelves (operative and contralateral sides) in 6 patients (24%). Urine pH was measured using an ultra-thin glass pH electrode. Urinary osmolality, Na and K

Randomized, single-blind comparison of sidehole and end-hole v end-hole ureteral catheters.

BACKGROUND AND PURPOSE: Flexible 5F, open-ended ureteral catheters are currently available only with an end hole, which can lead to difficulties during retrograde pyelography and when aspirating urine for culture or cytology, as the hole frequently sucks up urothelium or debris and becomes occluded. We compared the traditional end-hole ureteral catheter with a catheter having a sidehole as well. PATIENTS AND METHODS: Twenty-two patients requiring aspiration of urine from the renal pelvis for cytology study or culture or to empty the collecting system during retrograde pyelography or prior to

The natural history of asymptomatic calyceal stones.

OBJECTIVES: To evaluate the outcomes of patients with incidentally detected asymptomatic calyceal stones on active surveillance, and to identify risk factors for stone-related adverse events (AEs). PATIENTS AND METHODS: In this retrospective case series, we identified all renal units with non-contrast computed tomography diagnosed asymptomatic calyceal stones in a single reference centre between August 2005 and August 2016. Primary endpoints were spontaneous stone passage and need for stone-related surgical intervention. The secondary endpoints were stone-related symptoms and AEs. Cox proport

A prospective comparative study of different nephrolithometric scoring system for nephrolithiasis to predict stone free status and complications in supine PCNL.

The significant rise in stone disease has led to an increased popularity of percutaneous nephrolithotomy for the management of substantial stone burdens. Despite ongoing advancements in technology and surgical methods, overall complication rates of PCNL have gone up. For the best decision-making and well-informed patient counselling, a precise assessment of treatment success is essential. CT scans offer high-resolution spatial imaging that precisely characterizes stone size and distribution, pelvicaliceal anatomy, anomalies, and anatomical relationships. With these measurable stone and patient

Does a Novel Lead-Free Radiation Shield Improve Surgeon Protection Compared with Lead Apron Alone?

Introduction: The RADPAD is a novel lead-free shield that can be positioned directly in the surgical field to block scatter radiation. The primary objective of this study was to assess the effectiveness and added protection of the RADPAD compared with conventional lead during simulated percutaneous nephrolithotomy (PCNL). Methods: To simulate a PCNL, a radiographical phantom patient and a mannequin surgeon were utilized. Using a real-time dosimeter, radiation doses were measured in µSv at seven surgeon locations: head, neck, chest, abdomen, pelvis, forearm, and leg. Two shielding strategies we

Focus on Upper Urinary Tract Stones Combined with Parenchymal Infiltrative Renal Pelvis Cancer.

INTRODUCTION: Upper urinary tract stones combined with parenchymal infiltrative renal pelvic cancer are challenging to detect on imaging and to evaluate the differential diagnosis. CASE PRESENTATION: The symptoms and diagnoses in three cases of parenchymal infiltrative renal pelvic cancer and upper urinary tract stones that occurred between June 2019 and June 2022 were reviewed. Primary symptoms of lumbar discomfort and hematuria were evident in all 3 patients. Preoperative computed tomography (CT) abdominal imaging revealed that all three cases had hydronephrosis along with renal stones, whi

Incidental squamous cell carcinoma of renal pelvis presenting as skin invasion: a case report.

INTRODUCTION: Squamous cell carcinoma of the renal pelvis is a rare neoplasm, accounting for less than 0.8% of malignant renal tumors. Chronic irritation is believed to be the primary pathogenic cause for squamous cell carcinoma of the renal pelvis. The most frequently reported cases of squamous cell carcinoma of the renal pelvis generally present with hydronephrosis, pyelonephritis, or nephrolithiasis. The skin of the flank is a very uncommon site of clinical presentation. Here, we report an exceedingly rare case of squamous cell carcinoma of the renal pelvis presenting as skin invasion of th

Recurrent Nephrolithiasis in Congenital Cystinuria With a Complete Duplex Collecting System: A Case Report.

Cystinuria is a rare autosomal recessive genetic disorder that affects the reabsorption of cystine in the renal tubules. Although cysteine lithiasis is a rare form of nephrolithiasis, it typically presents early in life and is associated with a highly recurrent course and frequent need for repeated surgical interventions. The presence of a duplicated collecting system adds complexity, as it can hinder drainage and make endoscopic access more difficult. We report the case of a 16-year-old male patient with congenital cystinuria and a complete bilateral duplex collecting system. He presented wit

No Injury Observed in Kidneys Treated with Burst Wave Lithotripsy in Therapeutically Anticoagulated Pigs.

Introduction: Current American Urological Association guidelines recommend that patients with acute obstructive kidney stone requiring continuous anticoagulation/antiplatelet therapy should not be treated by shockwave lithotripsy or percutaneous nephrolithotomy because of the risk of catastrophic renal hemorrhage possible with those techniques. Currently, ureteroscopy is the only recommended surgical treatment. We evaluated if burst wave lithotripsy (BWL) could be used in these cases by treating pigs with BWL while undergoing anticoagulation therapy. Materials and Methods: Six pigs (31-37 kg)

Management of Kidney Stones in Pregnancy: A Worldwide Survey of Practice Patterns.

Introduction: Kidney stones in pregnant is not a common emergency, but it is one that is extremely challenging to manage. There exists no previous survey, which maps the different practice patterns adopted. Our aim was to deliver a survey to evaluate the current status of practice patterns across different parts of the world regarding the management of stone disease in pregnancy. Methods: Through an iterative process, 19-item survey was devised. This contained the following five sections: (1) Demographics, (2) General items, (3) Diagnosis and Imaging, (4) Initial management, (5) Surgery. It wa

Low Urologist Density Predicts High-Cost Surgical Treatment of Stone Disease.

Introduction and Objectives: Lack of access to urologic specialists is approaching crisis levels as the number of urologists is decreasing, while the demand for urologic care is increasing. The financial implications of this have not been explored. The objective of this study is to examine the impact of access and other patient factors on cost to treat urolithiasis. We hypothesized that markers of poor access would associate with higher costs of surgical encounters for patients presenting with urolithiasis. Methods: A retrospective review of prospectively collected data from the Registry for S

Revisiting the morphology of pelvicalyceal system in human cadaveric kidneys with a systematic review of literature.

OBJECTIVE: Renal collecting system macroscopically consists of minor calyx, major calyx, renal pelvis and ureter. Stone in renal collecting system is a common presentation in everyday urological practice. The prevalence of renal calculi ranges from 4% to 20% in different geographical distribution. Anatomical variation in renal collecting system plays a significant role in formation of calculi in its parts. The large extra renal pelvis leads to stagnation of urine for longer durations and formation of stones. The stone free rate after percutaneous nephrolithotomy and extra corporeal shock wave
