Methadone and buprenorphine prescribing and referral practices in US prison systems: results from a nationwide survey.

BACKGROUND: More than 50% of incarcerated individuals have a history of substance use, and over 200,000 individuals with heroin addiction pass through American correctional facilities annually. Opiate replacement therapy (ORT) with methadone or buprenorphine is an effective treatment for opiate dependence and can reduce drug-related disease and recidivism for inmates. Provision of ORT is nevertheless a frequently neglected intervention in the correctional setting. OBJECTIVE AND METHODS: We surveyed the 50 state; Washington, District of Columbia (DC); and Federal Department of Corrections' med

Psychiatric disorders and repeat incarcerations: the revolving prison door.

OBJECTIVE: A number of legal, social, and political factors over the past 40 years have led to the current epidemic of psychiatric disorders in the U.S. prison system. Although numerous investigations have reported substantially elevated rates of psychiatric disorders among prison inmates compared with the general population, it is unclear whether mental illness is a risk factor for multiple episodes of incarceration. The authors examined this association in a retrospective cohort study of the nation's largest state prison system. METHOD: The study population included 79,211 inmates who began

Adult correlates of early behavioral maladjustment: a study of injured drivers.

OBJECTIVE: To establish whether a history of school suspension (HSS) predicts adult driver behavior. METHODS: 323 injured drivers were interviewed as part of a study of psychoactive substance use disorders (PSUD) and injury. Drivers with a HSS were compared to those without HSS in relation to demographics, SES, PSUD, risky behaviors, trauma history and driving history using student's t test and chi-square. Multiple logistic regression models were constructed to adjust for demographics, SES and PSUD. RESULTS: HSS drivers represented 31% of the population and were younger, more likely to be ma

Client and program factors associated with dropout from court mandated drug treatment.

To examine why court mandated offenders dropout of drug treatment and to compare their characteristics, treatment experiences, perceptions, and outcomes with treatment completers, we analyzed self-reported and administrative data on 542 dropouts (59%) and 384 completers (41%) assessed for Proposition 36 treatment by thirty sites in five California counties during 2004. At intake, dropouts had lengthier criminal histories, lower treatment motivation, more severe employment and psychiatric problems, and more were using drugs, especially heroin. Relatively fewer dropouts received residential trea

Polysubstance abuse in liver transplant patients and its impact on survival outcome.

BACKGROUND: Alcohol-related end-stage liver disease was the most common reason for liver transplant in the 1990s. Currently, hepatitis C virus (HCV) is the most common reason for transplant. The major HCV risk factor is intravenous drug abuse, which often includes other forms of substance abuse. It is important to understand posttransplant survival outcomes in patients with multiple substance abuse and pretransplant factors that predict relapse. METHODS: The medical records of patients referred to the transplant psychiatrist were retrospectively reviewed to identify posttransplant patients wi

Predictors of prison-based treatment outcomes: a comparison of men and women participants.

The purpose of this study was to examine differences between men and women entering prison-based therapeutic community (TC) treatment and to explore the relationship of those differences to posttreatment outcomes (i.e., aftercare participation and reincarceration rates). Extensive treatment-intake interview data for 4,386 women and 4,164 men from 16 prison-based TCs in California were compared using chi-square analyses and t-tests. Logistic regression analyses were then conducted separately for men and women to identify gender-specific factors associated with post-treatment outcomes. Prison in

Adverse drug reactions/interactions in maintenance therapy.

This paper begins with a brief review of the early onset, recidivism, and multiple consequences of recurrent major depressions. Increasing attention to these factors underscores the importance of long-term maintenance therapy. Successful maintenance rests on optimizing the risk:benefit profile for the patient. Literature is reviewed that the serotonin selective reuptake inhibitors exhibit a significantly lower discontinuation rate due to adverse events than do other conventional antidepressants. Adverse events clearly detract from patient compliance and, in turn, ultimately contribute to the r

Using physiological biomarkers in forensic psychiatry: a scoping review.

UNLABELLED: Forensic psychiatry aims to reduce criminogenic risks and enhance societal safety. While effective at a macro level, crime recidivism rates among forensic patients remain substantial. This underlines the need for innovation, with growing interest in the use of physiological biomarkers. To identify the extent (size), range, (variety), and nature (characteristics) of evidence on the use of physiological biomarkers in forensic psychiatry, a systematic scoping review was conducted following JBI methodology and PRISMA-ScR guidance. Data on study characteristics and results were extracte

Randomized Trial Shows Smartphone Support App for DWI Offenders and Their Families Reduced Alcohol Use and Ignition Interlock Device Lockouts.

OBJECTIVE: Driving while intoxicated (DWI) remains a preventable source of morbidity and mortality in the United States. Ignition interlock devices (IIDs) are used to prevent DWI offenders from driving while intoxicated during a mandated installation period and are effective during that time. Once IIDs are removed, DWI rates are similar to levels for offenders who had no IID. This study tested the efficacy of a smartphone app (B-SMART) for DWI offenders with an IID and concerned family members (CFMs), with the goal of reducing IID lockout events and alcohol consumption. METHOD: Four B-SMART a

Peer support services for individuals with health-related needs reentering the community after incarceration: a scoping review of program elements and outcomes.

BACKGROUND: Peer support services (PSS) have become increasingly formalized in many healthcare settings. While use of PSS in the criminal-legal space is expanding, no comprehensive reviews of program designs and outcomes are available to characterize current approaches. This scoping review sought to identify PSS models for individuals with health-related needs transitioning from incarceration to the community and summarize key features and outcomes related to the models. RESULTS: A search of PubMed, PsycInfo, Web of Science, National Criminal Justice Reference Service, Criminal Justice Abstra

Expanding End-of-Life Options for Incarcerated Individuals.

The aging prison population in the United States presents urgent challenges for providing appropriate end-of-life (EOL) care. Currently, there are two primary approaches to deliver this in a compassionate manner: medical release programs (MRPs) and prison-based end-of-life care programs (EOLCPs). Using New York State as a case study, we argue expanded MRPs represent the most ethical path forward, while acknowledging the continuing need for prison-based EOLCPs. Our analysis reveals significant barriers to accessing medical release, including lack of knowledge about the application processes, pr

Effects of Limited English Proficiency on Outcomes Following Burn Injury.

Studies indicate that surgical patients with limited English proficiency (LEP) experience longer hospital stays and higher emergency department (ED) recidivism. Burn care requires complex coordination for both inpatient and outpatient services, regardless of language preference. This study hypothesized that burn patients with LEP would have longer hospital stays, fewer follow-ups, and more ED visits. Following IRB approval, a retrospective chart review analyzed burn patients admitted between January 2018 and December 2019. Collected data included demographics, burn characteristics, outcomes, p

Imaging guidance for cholesteatoma surgery using tissue autofluorescence.

SIGNIFICANCE: Cholesteatoma is an expansile destructive lesion of the middle ear and mastoid, which can result in significant complications by eroding adjacent bony structures. Currently, there is an inability to accurately distinguish cholesteatoma tissue margins from middle ear mucosa tissue, causing a high recidivism rate. Accurately differentiating cholesteatoma and mucosa will enable a more complete removal of the tissue. AIM: Develop an imaging system to enhance the visibility of cholesteatoma tissue and margins during surgery. APPROACH: Cholesteatoma and mucosa tissue samples were exc

Participants in Court-mandated Intervention Programs for Intimate Partner Violence Perpetrators with Substance Use Problems: A Systematic Review of Specific Risk Factors.

Men with alcohol and/or other drug use problems (ADUPs) court-mandated to attend intervention programs for intimate partner violence (IPV) perpetrators have been identified as a high-risk, highly resistant group of IPV perpetrators, as they present lower treatment adherence and higher dropout and recidivism rates. Previous research suggests that IPV perpetrators with ADUPs may require tailored interventions to address their specific risk factors. The present systematic review was conducted using PRISMA guidelines to identify the specific risk factors in men with and without ADUPs on entry to c

Psilocybin use is associated with lowered odds of crime arrests in US adults: A replication and extension.

BACKGROUND: The United States boasts the largest prison population in the world, conferring significant direct and indirect costs (e.g. lost wages for the incarcerated, increased morbidity/mortality, etc.) to society. Recidivism rates are high for the imprisoned and most interventions to reduce criminality are minimally effective. Thus, in addition to the need for criminal justice reform, there is a need to better understand factors linked to lowered criminal behavior. AIM: The aim of this study was to assess the relationships between the use of classic psychedelic substances (psilocybin, LSD
