Outcome of distal clavicular fracture separations and dislocations in immature skeleton.

BACKGROUND: There are only a few studies with long-term follow-up of distal clavicular injuries in children and their treatment is not clearly defined. The purpose of our study is to suggest a new classification system according to the fracture pattern and the degree of the displacement, to evaluate the long-term follow-up and also to propose an algorithm for treatment. METHODS: We conducted a retrospective study from 1983 to 2008. Seventy-five children and adolescents, age ranging from 3 to 16 years (46 >8 years), were treated in our department. We classified all these injuries into five gro

Failure of coracoclavicular artificial graft reconstructions from repetitive rotation.

PURPOSE: To assess how suture type and suture construct in an augmented Weaver-Dunn reconstruction affect coracoclavicular sling failure and rotary stability. METHODS: Fifteen cadaveric shoulders were tested in rotation about the long axis of the clavicle with 10 lb of simulated arm weight. The clavicle was rotated 50 degrees about its long axis, and the applied torque was recorded. Next, modified Weaver-Dunn reconstruction was conducted. Two types of coracoclavicular sling (opposed drill holes through the clavicle and complete loop around the clavicle) were tested by use of 3 different sutur

Indications for Arthroscopic Subacromial Decompression. A Level V Evidence Clinical Guideline.

Since the introduction of acromioplasty by Neer in 1971 and arthroscopic subacromial decompression (SAD) by Ellman in 1987, the outcomes have been reported to be consistently good. Recently it was suggested that supervised physical therapy is comparable with SAD, which was contested by other studies claiming that SAD is clearly superior to nonoperative treatment. Before consideration for treatment, the diagnosis of impingement with an intact rotator cuff must be determined by clinical history, a detailed and structured clinical examination, and appropriate imaging. In favor of SAD are publishe

Patient-controlled lidocaine analgesia for acromioplasty surgery.

Twenty-four consecutive patients undergoing shoulder acromioplasty were given postoperative analgesia with a new method in which a patient-controlled continuous infusion of lidocaine infiltrated the subacromial space. Seventeen of the acromioplasties were done with arthroscopy, whereas 7 were performed with an open procedure. A 2% solution of lidocaine without epinephrine was used for both a continuous dose of 2 cc/h and patient-controlled interval doses of 1 cc administered at 15-minute intervals. The catheter was left in place for 72 hours. We prospectively studied complications, the patient

Physical examination tests of the shoulder: a systematic review with meta-analysis of individual tests.

OBJECTIVE: To compile and critique research on the diagnostic accuracy of individual orthopaedic physical examination tests in a manner that would allow clinicians to judge whether these tests are valuable to their practice. METHODS: A computer-assisted literature search of MEDLINE, CINAHL, and SPORTDiscus databases (1966 to October 2006) using keywords related to diagnostic accuracy of physical examination tests of the shoulder. The Quality Assessment of Diagnostic Accuracy Studies (QUADAS) tool was used to critique the quality of each paper. Meta-analysis through meta-regression of the diag

Aneurysmal Bone Cyst of the Clavicle: A Rare Orthopedic Entity.

INTRODUCTION: Clavicle neoplasms account for only 1.01% of bone tumors and the possibility of it being a primary aneurysmal bone cyst is incidentally low. Although rare in incidence, most of the cases reported were treated with en bloc resection either as a primary procedure or during recurrence. The rarity of the disease and the lack of medical literature poses a diagnostic dilemma for the treating orthopedic surgeon, radiologist, and pathologist, that could potentially lead to the lesions being diagnosed at a stage when options to salvage the bone can no longer be considered. In our case, ea

[Morphometry of the sub-acromial space and its clinical relevance].

Rupture of the rotator cuff is one of the commonest (30%) pathologic findings in elderly dissecting room subjects. Together with the degenerative tendopathy associated with increasing age, this is due to mechanical damage to the distal parts of the tendons or the soft tissues lying in the subacromial space between the acromion, coracoacromial ligament, coracoid process and humeral head. The object of this investigation was to examine the influence of the decisive metrical parameters of the subacromial space in the macerated scapula. For this purpose, defined distances and angles of 343 macerat

Is rotator cuff repair appropriate in patients older than 60 years of age? prospective, randomised trial in 103 patients with a mean four-year follow-up.

BACKGROUND: The objective of this randomised trial was to compare surgical rotator cuff repair to simple decompression by acromioplasty and biceps tenotomy in patients older than 60 years of age with a mean follow-up of 4 years. HYPOTHESIS: Tendon repair produces better functional outcomes than simple decompression and prevents progression towards cuff tear arthropathy in the longer term. PATIENTS AND METHOD: Of 130 initially included patients older than 60 years of age and having rotator cuff tears deemed amenable to surgical repair, 103 (79%) were evaluated after a mean of 4 years. These p

Extra-articular knowles pin fixation for unstable distal clavicle fractures.

BACKGROUND: Unstable distal clavicle fractures may benefit from surgical treatment. This report described a new method with transacromial extra-articular Knowles pin fixation to treat these unstable fractures. The hypothesis is that the results would be improved. METHODS: Twenty-five patients with unstable distal clavicle fractures were treated by transacromial extra-articular Knowles pin fixation, coracoclavicular ligament repair and augmentation. The mean clinical and radiographic follow-up was 51 months. Functional outcomes were evaluated by a modified Constant-Murley score. RESULTS: Nine

Shoulder diagnoses in secondary care, a one year cohort.

BACKGROUND: Shoulder pain is common in the general population. Reports on specific diagnoses in general populations are scarce and only from primary care. The diagnostic distribution of shoulder disorders in secondary care is not reported. Most of the clinical research in the shoulder field is done in hospital settings. The aim of this study was to identify the diagnoses in a 1-year cohort in a hospital-based outpatient clinic using standardized diagnostic criteria and to compare the results with previous studies. METHODS: A diagnostic routine was conducted among patients referred to our phys
