The utility of transbronchial cryobiopsy performed under conscious sedation for interstitial lung diseases in a resource constrained setting.

BACKGROUND: Transbronchial biopsy (TBB) with a cryoprobe, also known as transbronchial lung cryobiopsy (TBLC), has become a well established modality for sampling lung parenchyma. TBLC is performed under general anaesthesia in the majority of centres, utilising rigid or flexible bronchoscopy. In resource-constrained settings, however, most diagnostic bronchoscopies, including TBB, are performed under conscious sedation with flexible bronchoscopy without the presence of a specialist anaesthetist. OBJECTIVES: Given the paucity of evidence on TBLC performed under conscious sedation for interstit

Clinical and pathological characteristics of CRS with nasal polyps in patients with asthma-COPD overlap.

OBJECTIVE: Bronchial asthma (BA) and Chronic Obstructive Pulmonary Disease (COPD) have been considered as independent diseases, but in recent years, Asthma and COPD Overlap (ACO), which combines characteristics of BA and COPD, has been defined. Chronic rhinosinusitis (CRS) with nasal polyps and BA, both caused by similar type 2 inflammation, often coexist and are considered to have a "one airway, one disease" relationship. In ACO, airway inflammatory cells of BA (eosinophils and CD4+ lymphocytes) and COPD (neutrophils, CD8+ lymphocytes and macrophages) coincide, theoretically leading to steroi

Safety of ultrasound-guided small-bore chest tube insertion in patients on clopidogrel.

BACKGROUND: The safety of small-bore chest tubes insertion with ultrasound (US) guidance has been well demonstrated in patients not receiving antiplatelet therapy. Given the current widespread use of these agents, pulmonologists frequently encounter patients on this therapy and requiring drainage of the pleural space. The use of these agents clearly increases the risk of bleeding, but it is not always possible to stop this therapy before the procedure, especially in patients requiring urgent drainage and those with coronary stents. The purpose of this study is to report our experience on the s

Outcome of medical thoracoscopy.

BACKGROUND: Medical thoracoscopy is a common procedure for pulmonologists working in Europe but is still unpopular in South East Asia with few medical centers in Thailand performing this procedure. We report our outcome of medical thoracoscopy based upon 10 years experience. MATERIAL AND METHOD: Medical thoracoscopy was first performed in our unit in 1998. The early indication was undiagnosed pleural effusion. Other indications which were empyema, pneumothorax and talc pleudrage were carried on for the last 3 years. The patients' demographic data, indication for medical thoracoscopy, procedur

Clinical profile of pulmonary involvement in rheumatological disorders: a cross-sectional study in a rheumatology-associated lung disease clinic.

Rheumatological diseases (RDs) are characterized by autoimmune-mediated organ damage, with the lungs being an important target organ. Almost all compartments of the pulmonary system may be involved, with considerable heterogeneity in the frequency, severity, and spectrum of pulmonary manifestations. This heterogeneity may result in delayed diagnosis and treatment, leading to adverse outcomes. The objective of this study was to characterize the clinical, demographic, serological, and radiological features of pulmonary manifestations across a spectrum of rheumatic diseases. This cross-sectional

Efficacy and safety of radial probe endobronchial ultrasound-transbronchial lung biopsy in peripheral pulmonary lesions without guide sheath and fluoroscopy.

BACKGROUND: Radial probe endobronchial ultrasound-transbronchial lung biopsy (rEBUS-TBLB) enables accurate and safe sampling of small peripheral pulmonary lesions (PPLs) inaccessible to conventional flexible bronchoscopy. Although widely adopted and effective, the standard approach using fluoroscopic guidance and a guide sheath (GS) has limitations, including reduced instrument flexibility, greater procedural complexity, and higher demands for equipment and radiation safety. OBJECTIVE: This study aimed to evaluate the diagnostic efficacy and safety of rEBUS-TBLB technique performed without th

Remote Spirometry, Education, and Action for COPD Diagnosis and Management in Rural and Medically Underserved Settings in South Carolina: Protocol for an Observational Feasibility Study.

UNLABELLED: Individuals living in rural or medically underserved areas (MUA) with chronic obstructive pulmonary disease (COPD) face significant barriers to specialised pulmonary care, including pulmonologists, diagnostic spirometry, and pulmonary rehabilitation. Remote spirometry for diagnostic screening and disease monitoring may mitigate access barriers and contribute to improved COPD management in this population. This study protocol describes the proposed implementation of a Mobile Health (mHealth) intervention using Bluetooth-enabled portable spirometry combined with a mobile disease mana

Severe restrictive lung disease and vertebral surgery in a pediatric population.

The aim of this study is to describe the outcome of surgical treatment for pediatric patients with forced vital capacity (FVC) <40% and severe vertebral deformity. Few studies have examined surgical treatment in these patients, who are considered to be at a high risk because of their pulmonary disease, and in whom preoperative tracheostomy is sometimes recommended. Inclusion criteria include FVC <40%, age <19 years and diagnosis of scoliosis. The retrospective study of 24 patients with severe restrictive lung disease, who underwent spinal surgery. Variables studied were age and gender, pre- an

Comparison of X-ray image quality and scattered radiation among portable, mobile, and stationary X-ray machines.

Chest radiography is a sensitive method for tuberculosis (TB) detection, and portable X-ray systems offer a practical solution for community-based screening. However, whether the image quality of portable systems is diagnostically comparable to that of mobile and stationary systems remains insufficiently addressed. This study compared image quality and ambient dose equivalent H*(10) per radiographic exposure across one portable, three mobile, and three stationary X-ray systems. Image quality was evaluated using a test object for radiography (TOR) contrast-detail phantom at acrylic thicknesses

Characterizing the Asthma Phenotype of Military Personnel.

INTRODUCTION: Asthma is the most common diagnosis in military personnel who endorse chronic dyspnea. Service members have unique occupational risk factors, and there is concern that airborne exposures in the deployed environment as well as other occupational exposures may contribute to the development of asthma or exacerbate pre-existing disease. Asthma phenotyping with clinical biomarkers such as serum immunoglobulin E (IgE) levels and eosinophil (EOS) counts is useful in defining treatment strategies for the management of asthma. This study sought to characterize the phenotype of medically s

Chronic hypersensitivity pneumonitis in the southeastern United States: an assessment of how clinicians reached the diagnosis.

BACKGROUND: Chronic hypersensitivity pneumonitis (cHP) is a disease caused by exposure to inhaled environmental antigens. Diagnosis of cHP is influenced by the awareness of the disease prevalence, which varies significantly in different regions, and how clinicians utilize relevant clinical information. We conducted a retrospective study to evaluate how clinicians in the Southeast United States, where the climate is humid favoring mold growth, diagnosed cHP using items identified in the international modified Delphi survey of experts, i.e., environmental exposure, CT imaging and lung pathology,

Rule-based and machine learning algorithms identify patients with systemic sclerosis accurately in the electronic health record.

BACKGROUND: Systemic sclerosis (SSc) is a rare disease with studies limited by small sample sizes. Electronic health records (EHRs) represent a powerful tool to study patients with rare diseases such as SSc, but validated methods are needed. We developed and validated EHR-based algorithms that incorporate billing codes and clinical data to identify SSc patients in the EHR. METHODS: We used a de-identified EHR with over 3 million subjects and identified 1899 potential SSc subjects with at least 1 count of the SSc ICD-9 (710.1) or ICD-10-CM (M34*) codes. We randomly selected 200 as a training s

Conventional transbronchial needle aspiration with 23 gauge needle: a preliminary study.

BACKGROUND: Conventional transbronchial needle aspiration (cTBNA) is a safe and minimally invasive procedure with a high yield for the diagnosis of large lymph nodes (LNs) in favourable locations (LNs >1.5 cm in stations #4R and/or #7). However, it is usually underutilized by pulmonologist. One of the main reasons given for not performing cTBNA is the risk of puncturing vascular structures of the mediastinum. Recently, with the twofold objective of minimize the risk of bleeding and reduce the cTBNA cost, a thinner and less expensive needle has been commercialized. It is a 23 gauge (G) needle t

The histopathology of pulmonary reactions to drugs.

Many pathologists and pulmonologists have trained and practiced in an era during which therapeutic options for patients have increased manyfold with regard to pharmacologic agents. Toxicity to the lung may be recognized as data accumulate in the form of case reports and clinical reviews to document the clinical and pathologic manifestations. For the histopathologist, paradoxically the more we learn the more difficult problems of interpretation become. Drug reactions previously described extensively at the autopsy now can be relatively easily interpreted in living patients with tissue obtained

Pulmonary venoocclusive disease/pulmonary capillary hemangiomatosis: Inadequacy of clinico-radiologic features in diagnosis.

Pulmonary venoocclusive disease (PVOD)/pulmonary capillary hemangiomatosis (PCH) belongs to the WHO group 1 pulmonary hypertension (PH), labeled as pulmonary arterial hypertension (PAH), though it mainly involves pulmonary veins, venules and capillaries. PVOD/PCH may occur as a pure form, with other types of PH, or with fibrotic interstitial lung diseases (ILDs). Recognition of PVOD/PCH is crucial as vasodilators used in PAH patients may cause severe pulmonary edema in PVOD/PCH patients. Since biopsy is relatively contraindicated in PAH given the high risk of complication, high resolution comp
